[CRST syndrome and primary biliary cirrhosis].

The case of a female patient with the history of amanita poisoning, CRST syndrome and primary biliary cirrhosis (PBC) is reported. Immunological features (IgM-rheumatoid factor, antinuclear-, anticentromere- and antimitochondrial antibodies) and HLA-antigens are studied. Immunological abnormalities occurring in CRST and PBC suggest a common autoimmune base, which also makes concomitance of these two disease states more than coincidental.

Regulation of immunological and inflammatory functions by biotin.

Biotin is a water-soluble B-complex vitamin and is well-known as a co-factor for 5 indispensable carboxylases. Holocarboxylase synthetase (HLCS) catalyzes the biotinylation of carboxylases and other proteins, whereas biotinidase catalyzes the release of biotin from biotinylated peptides. Previous studies have reported that nutritional biotin deficiency and genetic defects in either HLCS or biotinidase induces cutaneous inflammation and immunological disorders. Since biotin-dependent carboxylases involve various cellular metabolic pathways including gluconeogenesis, fatty acid synthesis, and th

Immunological complications beyond rejection after intestinal transplantation.

PURPOSE OF REVIEW: Intestinal transplantation (ITx) represents a major immunological challenge as the bidirectional exchange of donor's and recipient's immune cells delivers a graft highly chimeric and immunogenic. Therefore, there is a higher risk for immunological complications. It is important to understand the prevalence, natural history and mechanisms of other immunological complications, beyond rejection, to better understand the implications of ITx. RECENT FINDINGS: In recent studies, graft-versus-host disease (GVHD) holds a high mortality (43-70%). Apart from increasing or modifying i

[Secretion and regulation of cytokines during pregnancy and gestosis].

OBJECTIVE: Pregnancy-induced hypertension is still a common cause for fetal morbidity and mortality besides causing life-threatening diseases for the mother. The aethiology of this disease is still unclear. Some research support the theory of an immunological genesis. PATIENTS AND METHODS: 32 patients were treated for gestosis or HELLP-Syndrome in our clinic. A number of data, blood samples as well as the immunological status (IL-6, Interferon-gamma. TNF-alpha und TNF-alpha-receptores p 55/p 75) of every patient were taken into consideration and compared to the results of 44 pregnant women in

Slipped Capital Femoral Epiphysis as a Presentation of Underlying Metabolic Disorders: Pseudohypoparathyroidism and Juvenile Hypothyroidism.

Slipped capital femoral epiphysis (SCFE) is an abnormality of the proximal femoral physis typically occurring in adolescents and most commonly associated with obesity, although its exact etiology is unknown. In addition to obesity, other associations and predisposing factors proposed in the literature include trauma, vascular injury or compromise, and immunologic, genetic, and metabolic conditions. While not common, metabolic conditions are known to predispose to SCFE and it is essential for radiologists to recognize SCFE as a possible initial presentation of an underlying metabolic disorder.

Chronic mucocutaneous candidiasis, a case study and literature review.

Chronic mucocutaneous candidiasis (CMC) is a clinically heterogeneous disease. Some immunologic and hormonal abnormalities have been associated with CMC. The factors that predispose host to CMC infection could be autosomal or acquisitive. The disease usually occurs in childhood. Here, we reviewed the published literature on chronic mucocutaneous candidiasis and a four years old girl is presented with CMC. She had a history of recurrent thrush and otomycosis since the age of one. Candida albicans was detected in skin scraping and biopsy samples. Serum iron was low. TSH hormone level was high an

New insights in the immunologic basis of psoriasis.

Psoriasis vulgaris is a multifactorial heritable disease characterized by severe inflammation resulting in poorly differentiated, hyperproliferative keratinocytes. Recent advances in genetic analyses have implicated components regulating the interleukin (IL)-23 and nuclear factor-kappaB pathways as risk factors for psoriasis. These inflammatory pathways exhibit increased activity in skin lesions, and promote secretion of various cytokines, such as IL-17 and IL-22. Unrestrained, the activated inflammatory cytokine network in psoriasis may trigger a vicious cycle of inflammation and cellular pro

Pathogenic autoantibodies in lupus nephritis.

Lupus nephritis is a major complication of systemic lupus erythematosus (SLE) and is associated with a high rate of morbidity and mortality. While many different immunologic and nonimmunologic factors contribute to disease expression in lupus nephritis, a large body of evidence suggests that the production of anti-DNA antibodies and the formation of glomerular immune deposits are important initial events in the pathogenesis of the disease. This review will summarize our current understanding of the differences between pathogenic and nonpathogenic autoantibodies, the mechanisms by which these a…

Localized granuloma annulare and autoimmune thyroiditis in a Saudi patient: report of a new case.

The association of granuloma annulare (GA) and autoimmune thyroiditis has been documented in the literature in 13 previous cases. However, the pathogenesis of GA remains obscure. Possible pathogenetic factors suggested include: humoral and delayed type hypersensitivity, vascular damage, metabolic disorder, or, primary collagen and/or elastin alteration mediated through an immunologic mechanism. We present herein the report of a 37-year old Saudi female who presented with autoimmune thyroiditis associated with GA. The patient was managed with clobetasol propionate, intra-lesional prednisolone a

Coexistence of HLA-B*08 and HLA-B*18 in four siblings with Lichen sclerosus.

BACKGROUND: Lichen sclerosus (LS), is characterized by localized patches of atrophy and whitening of the skin. The cause of LS remains unknown, but genetic, hormonal, immunologic factors and autoimmune mechanisms have been incriminated. There are conflicting data regarding the association between LS and human leukocyte antigens (HLA). METHODS: We have analyzed the HLA alleles of a family, in which 4 of 5 children have lichen sclerosus. RESULTS: HLA-B*08 and HLA-B*18 alleles were detected in children with LS, but not in a healthy sister. None of the patients had autoimmune disease. CONCLUSIO…

Herpes zoster ophthalmicus in patients at risk for the acquired immune deficiency syndrome (AIDS).

In a prospective investigation of 54 consecutive cases of herpes zoster ophthalmicus, conducted over a two-year period, immunologic evaluations included enumeration of T lymphocyte subsets and serum immunoglobulin levels. Herpes zoster ophthalmicus occurred with frequent ocular complications in a subgroup of adults distinguishable by their young age, the presence of AIDS-risk factors, alterations in T-cell subpopulations, and polyclonal increases of serum gammaglobulin. Over the study duration, 21% (three of 14) of the AIDS-risk subgroup patients have developed AIDS with a 14% (two of 14) mort

[Norwegian scabies: etiological grounds (author's transl)].

Norwegian scabies is a rare clinical variant of human infestation with Sarcoptes scabiei var. hominis. It is characterized by hyperkeratotic lesions of the palmar and plantar surfaces, scalp and ears which contain myriad mites. The authors have observed three cases: the first occurred in a old delibited woman with a Recklinghausen's disease, the second in a boy with Down's syndrome, the last in a child treated with immunosuppressors. Review of the literature and study of etiological grounds. The pathogenesis is yet unclear but the altered host factors appear to be the principal determinant: pr

Effects of antibiotics, hospitalisation and surgical complications on self-reported immunological vulnerability following paediatric open-heart surgery and thymectomy: a single-centre retrospective cohort study.

BACKGROUND: Partial or complete thymectomy is routinely performed in paediatric open-heart surgeries when treating congenital heart defects. Whether or not thymectomised children require systematic immunological monitoring later in life is unknown. The objective of this study was to investigate the effects of preoperatively and postoperatively used antibiotics, hospitalisation and surgical complications on self-reported immunological vulnerability in paediatric patients with early thymectomy to better recognise the patients who could benefit from immunological follow-up in the future. The role of dydrogesterone in recurrent (habitual) abortion.

The published evidence regarding the administration of dydrogesterone in the treatment of habitual abortion is summarised in this review. Habitual abortion is defined as the loss of three or more consecutive pregnancies without known maternal or foetal pathology. The immunology of early pregnancy seems to determine the rejection or non-rejection of the allogenic embryo. When peripheral mononuclear cells from recurrent aborters are incubated with progesterone or dydrogesterone in vitro, T-helper (Th)2 cytokines such as interleukin (IL)-4 and IL-6 markedly increase whereas the Th1 cytokine inter

Delineating the autoimmune mechanisms in Graves' disease.

The immunologic processes involved in autoimmune thyroid disease (AITD), particularly Graves' disease (GD), are similar to other autoimmune diseases with the emphasis on the antibodies as the most unique aspect. These characteristics include a lymphocytic infiltrate at the target organs, the presence of antigen-reactive T and B cells and antibodies, and the establishment of animal models of GD by antibody transfer or immunization with antigen. Similar to other autoimmune diseases, risk factors for GD include the presence of multiple susceptibility genes, including certain HLA alleles, and the

INTRODUCTION: Cysticercosis is a common parasitic infection caused by the larval phase of the Taenia solium, it infects humans as well as pigs. Considered an endemic parasitosis in developing countries including Latin America, Asia and Africa. Clinical manifestations of the disease can be influenced by ambient factors, host individualities and the infectious agent itself. Neurocysticercosis can be asymptomatic or present with various signs and symptoms that can vary in severity. OUTLINE: This review is focused on analyzing the various presentations of Neurocysticercosis throughout different a

Relation between serum IL-4, IL-13 and IFN-gamma levels and recurrence of wheezing episodes in infants with acute bronchiolitis.

Lower respiratory tract infections are the most important factors among various causes which trigger wheezing in the first year of life. The factors associated with episodic wheezing in children with acute bronchiolitis are still subjects of research. Infections, environmental factors, immunologic mechanisms are sorted as etiologic risk factors of episodic wheezing. We aimed to investigate the relationship between serum interleukin (IL)-4, IL-13 and gamma-interferon (IFN-gamma) levels and recurrence of wheezing episodes in infants with acute bronchiolitis. One hundred twenty infants between 3

Incidence of and risk factors for lipoatrophy (abnormal fat loss) in ambulatory HIV-1-infected patients.

To identify clinical factors associated with the incidence of HIV-1-associated lipoatrophy, HIV-1-infected patients in the HIV Outpatient Study (HOPS) were prospectively evaluated for clinical signs of lipoatrophy at two visits about 21 months apart. Development of lipoatrophy was analyzed in stratified and multivariate analyses for its relationship to immunologic, virologic, clinical, and drug treatment information for each patient. Of 337 patients with no lipoatrophy at Survey 1, 44 (13.1%) developed moderate or severe lipoatrophy between the two surveys. In multivariate analyses, significan

T-cell-derived IgE-binding factors. I. Cloned and transformed T cells producing IgE-binding factors.

In order to derive T cell clones that can produce factors that specifically regulate the IgE response, Fc epsilon receptor (Fc epsilon R)-positive T cells were isolated from two patients with hyper-IgE states (either the tropical pulmonary eosinophilia syndrome or the hyper-IgE syndrome) and transformed using the human T cell leukemia/lymphoma virus. After infection, these T4-positive, Fc epsilon R-positive T cells manifested a fourfold increase in the surface interleukin 2 receptor, a 10-fold increase in Fc epsilon R, and acquired viral markers not present in the uninfected cells.
