Occupational and individual risk factors for dysphonia in teachers.

BACKGROUND: In recent decades several groups of researchers have been interested in describing and understanding vocal morbidity in teachers in order to explain the large number of teachers diagnosed with dysphonia and account for the absenteeism attributed to vocal disability. AIMS: To determine the proportion of teachers who reported a diagnosis of dysphonia and measure associations between individual and contextual factors and the event of interest. METHODS: Teachers were recruited from the city of Belo Horizonte and invited to complete a web-based institutional intranet questionnaire. R

Individualizing hormone-therapy according to cardiovascular risk.

There is growing consensus that the benefit to risk profile for hormone therapy is high for healthy, low-risk women initiating therapy within 10 years of menopause or under age 60. However, special considerations are needed for women who are outside those boundaries or for those that have risk factors for cardiovascular disease.

Culture-negative hand abscesses in immunocompetent individuals.

Gonococcal infection is a common sexually-transmitted infection in the older male population in our local setting. It is caused by Neisseria gonorrhoeae, and results in fever, dysuria and a foul-smelling discharge from the external urethral meatus. Occasionally, it may also present with disseminated gonococcal infection - dermatitis, septic arthritis and even meningitis or endocarditis. We present two unusual cases, where the primary presentation was that of multiple subcutaneous hand and wrist abscesses. This illustrates the need for competent history-taking, especially in culture-negative pa

Prenatal application of the individualized fetal growth reference.

The individualized reference for defining small for gestational age (SGA) at birth has gained popularity in recent years. However, its utility on fetal assessment has not been evaluated. The authors compare an individualized with an ultrasound reference in predicting poor perinatal outcomes. Data from a large clinical trial in predominantly white US women (1987-1991) with singleton pregnancies (n = 9,526) were used. The individualized reference classified fewer SGA fetuses than the ultrasound reference, but the risks of adverse outcomes were similar between fetuses classified by both reference

Symptomatic duodenal cryptococcosis in HIV-infected individuals.

Gastrointestinal involvement is an uncommon manifestation of cryptococcosis and, consequently, there are very few reports that have described symptomatic duodenal involvement. We present three cases of Cryptococcus-associated duodenitis in HIV-positive patients and review the literature.

Genetic testing and screening of individuals at risk of NF2.

Genetic testing and management of the at-risk individual for neurofibromatosis type 2 (NF2) is complicated by the well-documented risk of mosaicism that causes a milder later onset more asymmetrical disease course. Risks of NF2 were derived from genetic testing of over 1000 individuals through the Manchester NF2-testing service. Individuals are at risk of NF2 or have 'potential' NF2 if they have features of the disease that fall short of diagnostic criteria or are the first-degree relative of someone with NF2 or suspected NF2. The present protocol devised for the Nationally Commissioned Group

Training rapid stepping responses in an individual with stroke.

BACKGROUND AND PURPOSE: Compensatory stepping reactions are important responses to prevent a fall following a postural perturbation. People with hemiparesis following a stroke show delayed initiation and execution of stepping reactions and often are found to be unable to initiate these steps with the more-affected limb. This case report describes a targeted training program involving repeated postural perturbations to improve control of compensatory stepping in an individual with stroke. CASE DESCRIPTION: Compensatory stepping reactions of a 68-year-old man were examined 52 days after left he

Individual differences in behavioral estimates of cochlear nonlinearities.

Psychophysical methods provide a mechanism to infer the characteristics of basilar membrane responses in humans that cannot be directly measured. Because these behavioral measures are indirect, the interpretation of results depends on several underlying assumptions. Ongoing uncertainty about the suitability of these assumptions and the most appropriate measurement and compression estimation procedures, and unanswered questions regarding the effects of cochlear hearing loss and age on basilar membrane nonlinearities, motivated this experiment. Here, estimates of cochlear nonlinearities using te

Gastric cancer in individuals with Li-Fraumeni syndrome.

PURPOSE: Li-Fraumeni syndrome is a rare hereditary cancer syndrome associated with germline mutations in the TP53 gene. Although sarcomas, brain tumors, leukemias, breast and adrenal cortical carcinomas are typically recognized as Li-Fraumeni syndrome-associated tumors, the occurrence of gastrointestinal neoplasms has not been fully evaluated. In this analysis, we investigated the frequency and characteristics of gastric cancer in Li-Fraumeni syndrome. METHODS: Pedigrees and medical records of 62 TP53 mutation-positive families were retrospectively reviewed from the Dana-Farber/National Cance

A pharmacogenomic method for individualized prediction of drug sensitivity.

Identifying the best drug for each cancer patient requires an efficient individualized strategy. We present MATCH (Merging genomic and pharmacologic Analyses for Therapy CHoice), an approach using public genomic resources and drug testing of fresh tumor samples to link drugs to patients. Valproic acid (VPA) is highlighted as a proof-of-principle. In order to predict specific tumor types with high probability of drug sensitivity, we create drug response signatures using publically available gene expression data and assess sensitivity in a data set of >40 cancer types. Next, we evaluate drug sen

Effect of posture on deglutitive biomechanics in healthy individuals.

The purpose of this study was to assess displacement of oropharyngeal structures, particularly the hyoid bone and velum, and variations in timing of the pharyngeal stage of swallowing in the upright versus the supine position. Twelve Caucasian adult subjects between 19 and 27 years of age participated. Subjects were recorded swallowing 7 cc of liquid barium in the upright and supine positions. The hyoid bone had a significantly greater amount of anterior displacement while in the supine position compared to that of the upright position (p < 0.01). While in the upright position, the velum comes

Assistive technologies for visually impaired individuals in Turkey.

A phone survey was conducted with 80 visually impaired people in Turkey to understand what types of assistive products are used and to what extent. The perceived utility of selected products and reasons for nonuse were investigated. Descriptions of products that would be useful for the participants but may not be available on the market were also obtained. The three most used assistive products were found to be computer screen readers (46%), talking watches (26%), and screen readers for cellular phones (21%). Cellular phones with screen reading capability are the most desired assistive product

Variable individual sensitivity to cannabis in patients with schizophrenia.

There is now compelling evidence that cannabis consumption might precipitate psychosis onset. The objective of the present study was to assess the role of individual sensitivity to the psychotogenic effect of cannabis in male patients with schizophrenia. The lifetime diagnosis, disease and substance-use history were determined using a standardized interview in 190 patients with schizophrenia. Of patients with lifetime cannabis use (n=121), 44 were characterized as Cannabis-sensitive (CS) patients if the onset of psychotic symptoms occurred within 1 month following the initiation of cannabis co

Speech rate treatments for individuals with dysarthria: a tutorial.

Research confirms the long-standing clinical observation that patients with dysarthria exhibit variability in speech rate. Thus, modifying speech rate has been documented as one of the best treatment options for these patients. In this tutorial, several published rate control interventions for dysarthric speakers are presented. Studies discussed utilized various interventions, including pacing boards, alphabet board supplementation, visual and auditory feedback, cueing and pacing strategies, and delayed auditory feedback. Interventions discussed represent a hierarchy from "rigid" strategies, w

Characteristics of individuals with hydrocephalus and their dental care needs.

Hydrocephalus is considered a congenital disease affecting the central nervous system, and is characterized by accumulation of cerebrospinal fluid in the lateral ventricles of the brain causing progressive ventricular dilatation. This review paper reports on the oral characteristics of persons with hydrocephalus and discusses their dental care needs. It has been reported that patients with hydrocephalus can present chronological changes in the eruption of their teeth, changes in the occlusion, greater accumulation of plaque, and have a higher caries prevalence. Regular dental visits are needed
