This case report describes a 77-year-old man with severe immune-related gastritis after receiving pembrolizumab for lung cancer. Despite initial steroid therapy, he showed no improvement and was diagnosed with steroid refractory immune-related gastritis. Infliximab was added to steroid therapy, resulting in significant clinical and endoscopic improvement. Tumor necrosis factor-α inhibitor (TNF-α) is frequently used for Crohn's disease and other autoimmune conditions. Increased risk of infection is an accepted adverse effect of TNF-α, and routine screening for potential infections are carried out before initiation of therapy. We report the case of a patient who developed a localized painful swelling near the injection site, which was diagnosed as acute dermato-lymphangio-adenitis due to filarial infection. This adds to the limited number of case reports on parasitic complications following TNF-α therapy.

No published data are available regarding infliximab use in Kawasaki disease patients with recent live virus vaccination. We investigated 38 acute Kawasaki disease patients who received infliximab within 90 days of receiving measles-mumps-rubella, varicella-zoster virus, or Rotavirus vaccines. There were no adverse events or serious infections, which suggests that infliximab use in acute Kawasaki disease patients with recent live viral vaccinations is safe.

BACKGROUND & AIMS: Little is known about the long-term efficacy of infliximab for patients with fistulizing perianal Crohn's disease. We evaluated outcomes and predictors of outcomes in these patients.
