[Long term prognosis after massive small bowel resection in children].

Eight survivors after massive small bowel resection were reviewed for a period of 2 to 19 years to assess the long term prognosis of patients. The primary diseases of short bowel syndrome were congenital intestinal atresia (6) and midgut volvulus (2). The length of the residual small intestine ranged between 27 and 75 cm and ileocecal valve was also resected in 3 cases. All cases had been already weaned from parenteral nutrition and six children tolerate normal meals and two have still enteral formula at home now. Near normal somatic growth was achieved in cases which received intensive nutrit…

The role of amniotic fluid in influencing neonatal birth weight.

OBJECTIVE: We studied 123 neonates with atresias of the gastrointestinal tract at different levels to assess the role of amniotic fluid in the growth and development of the gastrointestinal tract. Our hypothesis was that significant differences in birth weight between groups otherwise comparable would suggest an intrauterine nutritive role for amniotic fluid. Our second hypothesis was that there is a trophic role of amniotic fluid on intestinal epithelium affecting its functional maturity. Establishing earlier tolerance of enteral nutrition in neonates with distal as compared with proximal atr

Gastroschisis: A State-of-the-Art Review.

Gastroschisis, the most common type of abdominal wall defect, has seen a steady increase in its prevalence over the past several decades. It is identified, both prenatally and postnatally, by the location of the defect, most often to the right of a normally-inserted umbilical cord. It disproportionately affects young mothers, and appears to be associated with environmental factors. However, the contribution of genetic factors to the overall risk remains unknown. While approximately 10% of infants with gastroschisis have intestinal atresia, extraintestinal anomalies are rare. Prenatal ultrasoun…

Perioperative and Long-Term Outcome in Patients Treated for Jejunoileal Atresia.

OBJECTIVES: Jejunoileal atresia is a common cause of neonatal intestinal obstruction. Results of long-term outcome are very limited. The aim of this study was to describe perioperative and postoperative outcome, and to evaluate long-term gastrointestinal quality of life (QoL) in patients treated for jejunoileal atresia. METHODS: We conducted a combined retrospective and cross-sectional observational study of patients treated for jejunoileal atresia during 2001-2019. Perioperative data were registered, and complications were classified according to the Clavien-Dindo classification. To evaluate

Should open Kasai portoenterostomy be performed for biliary atresia in the era of laparoscopy?

Kasai portoenterostomy has been the treatment of choice for neonates with biliary atresia since its introduction. With the advance in laparoscopic techniques, a few centers have reported the feasibility of performing laparoscopic Kasai portoenterostomy. However, the outcome of this new technique is not known. Here, we aim to evaluate, as the only referral center for liver transplantation, our experience with patients referred for transplantation after failed Kasai portoenterostomy. A retrospective study was carried out between October 1996 and September 2005. The records of all patients with t

Splenic infarct with polysplenia syndrome and situs inversus.

A rare case of spontaneous splenic infarction with polysplenia and situs inversus is presented. The diagnosis was made by ultrasound and confirmed by CT which showed multiple aberrant right-sided spleens with situs inversus and one of the spleen showing low attenuation areas representing infarct. Polysplenia syndrome is a rare entity associated with heterotaxy syndromes, congenital cyanotic heart diseases, billiary atresia, intestinal malrotation, immotile cilia syndrome, annular or short pancreas, preduodenal portal vein and vena caval anomalies.

Evolution of sonographic findings in a fetus with ileal atresia.

We report a case of a meconium pseudocyst secondary to ileal atresia and midgut volvulus. Initially, a single anechoic cyst was detected on prenatal sonography. The cyst gradually increased in size during the second trimester and eventually appeared as a large mass in the lower abdomen with echogenic content and associated with bowel dilatation. This case indicates that until the mid third trimester a single sonolucent cyst without signs of bowel obstruction may be associated with congenital intestinal obstruction.

Hydrocolpos with peritonitis in the newborn.

Two cases of hydrocolpos are described presenting soon after birth. One infant who died on the first day had an intrauterine peritonitis caused by compression of the caecum on the pelvic brim. The other surviving infant developed a caecal perforation secondary to Hirschsprung's disease. Neither infant had rectal atresia, which is the commonest cause of intestinal obstruction in these infants. The association of hydrocolpos with polydactyly in one of these cases may represent an autosomal recessive trait.

TTC7A mutations disrupt intestinal epithelial apicobasal polarity.

Multiple intestinal atresia (MIA) is a rare cause of bowel obstruction that is sometimes associated with a combined immunodeficiency (CID), leading to increased susceptibility to infections. The factors underlying this rare disease are poorly understood. We characterized the immunological and intestinal features of 6 unrelated MIA-CID patients. All patients displayed a profound, generalized lymphocytopenia, with few lymphocytes present in the lymph nodes. The thymus was hypoplastic and exhibited an abnormal distribution of epithelial cells. Patients also had profound disruption of the epitheli…

Prenatal closure of abdominal defect in gastroschisis: case report and review of the literature.

With the routine use of fetal imaging studies during prenatal care, increased numbers of unusual intrauterine events are now detected. Prenatal closure of the abdominal defect in gastroschisis is an example. We report a 34 5/7-week stillborn who had prenatal closure of a ventral abdominal wall defect, which had been seen earlier on fetal ultrasound examination. Two ultrasound examinations performed at 15 1/7 weeks and 19 1/7 weeks showed a mass of exteriorized bowel that herniated through the abdominal defect, just to the right of the umbilical cord. At 30 1/7 weeks, no exteriorized bowel was

Developmental and Genetic Considerations in a Neonate With Pectus Excavatum, Bilateral Hydroceles, Indirect Inguinal Hernia, and Type 2 Ileal Atresia.

This case study describes a combination of congenital anomalies observed in a full-term male neonate, including pectus excavatum, bilateral hydroceles, an indirect inguinal hernia, and type 2 ileal atresia. The neonate, delivered by cesarean section at 39 weeks and one day, presented with pectus excavatum and bilateral hydroceles, confirmed both prenatally and postnatally. Postpartum, the neonate experienced feeding difficulties and failed to pass meconium within 24 hours of birth. Diagnostic imaging revealed dilated bowel loops consistent with intestinal obstruction, necessitating surgical in

Determining oncogenic patterns and cancer predisposition through the transcriptomic profile in Mitchell-Riley syndrome with heterotopic gastric mucosa and duodenal atresia: a case report.

BACKGROUND: Homozygous mutations in the transcription factor RFX6 are the cause of the Mitchell-Riley syndrome (MRS) associating neonatal diabetes, congenital digestive system, such as biliary atresia, pancreatic hypoplasia, duodenal and/or jejunal atresia, intestinal malrotation, gallbladder aplasia, cholestasis. A constitutive inactivation of RFX6 leads also to gastric heterotopia. Application of RNA-seq in human diseases may help to better understand pathogenic mechanism of diseases and to predict the risk of developing chronic disorders and personalizing their prevention and treatment. Minimally conjoined omphalopagus twinning: a case report.

In most omphalopagus twinnings, joined structures include the gastrointestinal tract, liver, biliary tree and bladder. In some instances of omphalopagus, joined attachments are limited to the intestine and bladder. Eight cases like these, that are also called minimally conjoined twinning, have been reported before. This article describes a set of twins who were joined by an omphalocele sac and small bowel. There were several loops of bowel in the conjoined omphalocele sac without any evidence of liver. One of the twins had atresia of the colon, rectum and anus and the other had a cloacal anoma

Intestinal obstruction in the newborn.

Any newborn who continues to vomit in the first few days of life, particularly if the vomitus contains bile and if the abdomen is distended, should have immediate investigation because intestinal obstruction in the newborn is a fatal condition unless promptly recognized and surgically corrected. The most common cause of obstruction at this age is atresia and the simplest possible surgical procedure which adequately corrects this deformity should be done. It is also possible to successfully correct the obstruction caused by other congenital deformities such as annular pancreas and meconium ileu

Interdisciplinary management of pediatric intestinal failure: a 10-year review of rehabilitation and transplantation.

Management of children with intestinal failure is optimized by interdisciplinary coordination of parenteral and enteral nutrition support, medical management of associated complications, surgical lengthening procedures, and intestinal transplantation. Three hundred eighty-nine pediatric patients have been referred to our center for interdisciplinary assessment of intestinal failure since 1996 (median age=1 year; range 1 day-28.8 years). Factors predictive of weaning from parenteral nutrition without transplantation included increased mean bowel length for patients with gastroschisis (44 vs. 23

Combined diagnostic imaging in a patient with suspected biliary atresia.

The case of a newborn infant of 40 days of age with persistent jaundice and blood chemistry values suggestive of cholestasis is discussed. Liver and bile duct US documented missed visualization of the gallbladder. In the suspicion of biliary atresia, hepatobiliary scintigraphy was performed; it showed preserved hepatocyte function, missed visualization of the gallbladder and absence of intestinal bile flow which confirmed the diagnostic hypothesis. Based on possible diagnostic alternatives, signs perceived and interpreted, are analyzed.

Gastroschisis complicated by midgut atresia, absorption of bowel, and closure of the abdominal wall defect.

We present a case of gastroschisis that was associated with progressive resorption of the extra-abdominal bowel loops and dilation of intra-abdominal bowel loops. After preterm delivery at 32 weeks, a small paraumbilical remnant was present. There was complete atresia of most of the jejunum, ileum, cecum, and the proximal half of the transverse colon. At laparotomy, the jejunum was anastomosed to the transverse colon. The neonate developed short gut syndrome and eventually received a liver and intestinal transplant.

Congenital hernia of the abdominal wall: a differential diagnosis of fetal abdominal wall defects.

A 28-year-old woman was referred at 33 weeks of gestation with suspected fetal intestinal atresia. Sonography showed a large extra-abdominal mass on the right of the normal umbilical cord insertion. Following Cesarean section at 36 weeks and immediate surgical treatment, the malformation was not definable either as an omphalocele or as gastroschisis. This reported case involves a previously undocumented malformation of the fetal abdominal wall described as a 'hernia' of the fetal abdominal wall.
