Of all the spontaneous fistulas that occur between the extrahepatic biliary system and the intestine, a choledochoduodenal fistula is rarely seen. When it does occur, it is most often secondary to a perforated duodenal ulcer, choledocholithiasis, or cholelithiasis. It may also be seen following complications related to iatrogenic injury or tuberculosis. Generally, choledochoduodenal fistulas are asymptomatic, but may present with vague abdominal pain, fever, and other symptoms related to cholangitis. As a result, they can be difficult to diagnose clinically before imaging is obtained. We prese

OBJECTIVE: There is a paucity of data on the sonographic criteria for the diagnosis of intestinal tuberculosis. The purpose of this study was to further characterize the potential sonographic signs of intestinal tuberculosis and to increase the diagnostic sensitivity and specificity of ultrasound.
MATERIAL AND METHODS: Seven patients with a final diagnosis of gastrointestinal tuberculosis and a control group of 18 patients suffering from tuberculosis limited to the lungs were prospectively evaluated for sonographic criteria of intestinal tuberculosis and the findings were compared with those

In some patients with Crohn's disease the anorectal complications are the major cause of symptoms and morbidity. Anorectal Crohn's disease may be present in patients with intestinal Crohn's disease, may be the initial manifestation of the disease, or rarely occurs without involvement of Crohn's disease elsewhere in the intestinal tract. The pathogenesis of these anorectal complications remains to be clarified. The anorectal examination is very important in the assessment of patients with suspected or documented inflammatory bowel disease. Meticulous physical examination, examination under anae

Total parenteral nutrition was given to 10 patients with acute regional enteritis for 3--12 weeks. The rationale of this therapy is elimination of potential food allergens and decrease of intestinal irritation. Complete remission of the disease could be achieved in 8 patients, thus surgical removal of the intestine involved could be avoided. One entero-cutaneous and two perinal fistulas healed spontaneously; two other perianal fistulas improved so much, that local surgical treatment was sufficient. If well planned, total parenteral nutrition can be performed without disturbance of metabolism a

Gallstone ileus is an uncommon cause of bowel obstruction that involves cholecystoenteric fistulation and resultant passage of gallstones into the bowel. In the vast majority of cases, the fistula forms between the gallbladder and duodenum leading to small bowel obstruction. We report a case of cholecystocolic fistulation and subsequent large-bowel obstruction in a 75-year-old woman who presented acutely after taking a bowel preparation for an outpatient colonoscopy during the course of an investigation of anaemia and nonspecific abdominal pain. Preintervention imaging revealed a giant gallsto

The authors report 7 cases of gallstone ileus out of 452 intestinal obstructions corresponding to an incidence of 0.04%. The late references on the subject show that gallstone ileus is an unusual condition which gives rise to surgical approach problems and operating mortality since it is a typical seniors' disease. Some Authors support the "one stage" repair for cholecystoduodenal fistula and gallstone ileus, whereas others suggest firstly to resolve the gallstone ileus with the enterolithotomy and then to perform the operation for gallstone disease. The authors treated six cases with the ente

A patient with cholecystoduodenocolic fistula and gallstone ileus is described. Barium enema and barium meal and follow through demonstrated the passage of the gallstone from the gallbladder region to the small bowel. The clinical features and operative management are discussed in the light of four previously recorded cases.

The authors, after a short reference to considerations of general order, report 6 cases of gallstone-ileus. The number of cases, studied with the traditional X-ray procedure (with and without contrast means) are quite various for the occlusion at different levels of the small bowel, and exactly from the duodenal bulb to the Bowin valve, and for the X-ray symptomatology complete and incomplete. At last it must be underlined the high percentage where the stone of the gallbladder was ascertained, and which is to be considered responsible of intestinal occlusion; this ascertainment has been a very

A rare case of gastrointestinal fistula secondary to gastric cancer is presented. Fluoroscopic examination visualized the passage of contrast medium from the stomach to the small intestine. Gastric fiberscopy revealed cancer on the greater curvature of the corpus perforating into the jejunum. The scope could be inserted readily into jejunum through this perforation. Laparotomy showed that the cancer infiltrated and perforated from the stomach to the jejunum at about 10 cm. from the ligament of Treitz. Distal gastrectomy and resection of 10 cm. of the perforated jejunum were performed. The rese…

Positive urine and blood assays of methylene and leukomethylene blue were obtained from 9 volunteers with normal colonic and renal function after a 100 ml, methylene blue enema (50 mg.). The study conclusively demonstrates that methylene blue is absorbed by rectal mucosa and excreted by the kidneys as a colored dye in the urine. Thus, intrarectal methylene blue cannot be used to confirm the diagnosis of a colovesical fistula.
