Release hallucinations and visual loss as first manifestations of postoperative unilateral blindness.

PURPOSE: Release hallucinations (RH) with insight into the unreality of the hallucination have been reported in association with severe vision loss. Postoperative blindness following nonocular surgery may appear associated with central retinal artery occlusion (CRAO) caused by incorrect head positioning during surgery, intraoperative hypotension, and atherosclerosis. RH may initially mask the loss of vision. METHODS: Case report. RESULTS: A 27-year-old woman who had undergone lumbar spinal surgery for right L4-L5 discal herniation developed left eye visual loss and complex visual hallucinati

Gallbladder duplication and laparoscopic management.

Gallbladder duplication can present a significant challenge to the laparoscopic surgeon, primarily due to difficulties with diagnosis and recognition. Previous reports of attempted laparoscopic cholecystectomy in patients with gallbladder duplication resulted in incomplete or staged multiple procedures. The case report of a 35-year-old woman with successful laparoscopic management of symptomatic gallbladder duplication is described, emphasizing several important considerations. Preoperatively when evaluating radiologic studies a high index of suspicion is necessary in interpreting atypical fin

Reliability of preoperative ultrasonography evaluation for vitreoretinal surgery.

To evaluate the present level of reliability of common ultrasonography prior to vitreoretinal surgery, we carried out a prospective study to compare preoperative ultrasonography data with direct intraoperative findings. A total of 228 eyes with different vitreoretinal disorders and opaque or clear media were studied. In the group of 187 eyes with tractional retinal detachment (group I), the percentage of agreement between the ultrasonography data and the surgical findings ranged from 77.1% to 92.2%, depending on the pathology. In the group of 41 eyes without retinal detachment (group II), agre

Postoperative Femoral Neuropathy After Oncological Pelvic Surgery: A Case Report and Call for Action.

Femoral neuropathy is an uncommon but potentially disabling complication that is well described after gynecologic and colorectal surgery, yet remains poorly characterized following extensive pelvic oncological procedures, despite comparable or greater cumulative risk. We describe a 62-year-old man with type 2 diabetes who developed left-sided femoral neuropathy after open cystoprostatectomy, sigmoid colectomy, cutaneous ureterostomy, and pelvic lymphadenectomy for urachal adenocarcinoma. Postoperatively, he presented with hip flexion and knee extension weakness, anterior and medial thigh/leg h

Oculocardiac Reflex during Electrode Placement in Trigeminal Schwannoma Surgery via Cavernous Sinus Approach.

The oculocardiac reflex (OCR) is well known in ophthalmic surgery, but its occurrence during neurosurgical procedures is rare and often unexpected. Mechanical stimulation of trigeminal pathways at the skull base can cause significant hemodynamic changes; however reflex activation during preparatory steps rather than tumor manipulation is rarely described. We report an intraoperative event in a 22-year-old male undergoing resection of a cystic trigeminal schwannoma via a subtemporal transcavernous approach under general anesthesia, during which periorbital needle electrodes were placed for cran

Unintentional Intra-Implant Fat Injection During Breast Fat Grafting: A Case Report and Focused Review of the Literature.

BACKGROUND: Fat grafting is commonly used in implant-based breast reconstruction to improve contour and soft-tissue quality. Cannula-related implant injury during fat grafting is considered rare and is infrequently discussed in the literature. METHODS: A focused review of PubMed and Embase identified reports describing implant puncture or rupture associated with fat grafting. In addition, we present an institutional case of delayed implant infection following fat grafting, with intraoperative identification of intraprosthetic fat. RESULTS: Four published case reports and a case discussion wi

Giant sacrococcygeal chordoma presenting with isolated urinary symptoms: A case report.

BACKGROUND: Chordomas are rare low-grade malignant tumors arising from remnants of the primitive notochord. They most commonly affect the sacrum and typically present with lower back pain and radiculopathy. Genitourinary or gastrointestinal symptoms usually occur later in the disease course and are commonly associated with neurologic deficits. CASE DESCRIPTION: We report the case of an 83-year-old Latin American male who presented with a 1-year history of intermittent dysuria, urinary frequency, nocturia, tenesmus, and dyschezia, without back pain or neurologic symptoms. Initial urologic and

The Great Mimicker: Variable Presentations of Xanthogranulomatous Salpingo-Oophoritis in a Case Series of Seven Patients From a Rural Tertiary Care Center.

Xanthogranulomatous salpingo-oophoritis (XGSO) is an uncommon, non-neoplastic chronic destructive inflammatory condition. Its clinical and radiological features frequently masquerade as advanced ovarian malignancy or refractory pelvic inflammatory disease, posing a notable diagnostic challenge. Histopathological examination remains the gold standard for diagnosis. A retrospective clinicopathological evaluation of seven distinct cases of xanthogranulomatous adnexal inflammation was performed. Clinical symptoms, diagnostic imaging, intraoperative details, surgical approaches, and gross and micro

Unilateral hyperreactio luteinalis of the ovary during pregnancy mimics ovarian malignancy: A case report.

RATIONALE: Hyperreactio luteinalis (HL) involving a unilateral ovary is extremely rare, and the clinical presentation of HL can mimic that of ovarian malignancy, resulting in potential misdiagnosis and unnecessary surgical interventions. PATIENT CONCERNS: A 34-year-old female (gravida 6, para 1), pregnant for 6 weeks according to her menstrual records, presented with nausea and abdominal distension. DIAGNOSES: Ultrasonography revealed unilateral ovarian enlargement with multilocular cysts and ascites. Cancer antigen 125 (CA125) was elevated at 337.6 U/mL, raising suspicion of ovarian maligna

Difficulties of Differentiating Primary Histiocytic Sarcoma of the Central Nervous System from Glioblastoma in Older Patients: Methylation Analysis and Review of the Literature.

Primary histiocytic sarcoma of the central nervous system is an extremely rare malignancy, with only 43 cases reported to date. It often presents diagnostic challenges due to its rarity and similarities with other central nervous system tumors, particularly glioblastoma. We report a case of primary central nervous system histiocytic sarcoma in a 72-year-old woman, one of the older patients among the reported cases. The patient presented with gait disturbance and left hemiplegia. Initial imaging and intraoperative findings suggested a high-grade glioma. However, comprehensive pathological exami

Neuromuscular Blockade Monitoring: Having It but Knowing When Not to Trust It.

Butyrylcholinesterase (BChE) is an enzyme involved in the degradation of depolarizing and non-depolarizing neuromuscular blocking agents (NMBA), such as succinylcholine and mivacurium, respectively. Its deficiency is inherited or acquired, and results in paralysis of skeletal muscles after NMBA administration. We report a case of a 32-year-old pregnant woman proposed for cesarean section. General anesthesia (GA) was induced using propofol and succinylcholine. The surgical procedure was uneventful but after 40 minutes, there was no reversal of neuromuscular block (NMB). Other differential diagn

Intracranial germinoma in the lateral ventricle with polydipsia and polyuria: a case report and literature review.

Central nervous system germ cell tumors (CNSGCTs) are rare neoplasms which usually develop in the midline structures. They are occasionally involved in off-midline structures of the brain. Here, we report an extremely rare case of an intracranial germinoma in the lateral ventricle. The patient was a 10-year-old boy with a 1-year history of polydipsia and polyuria. Brain magnetic resonance imaging (MRI) showed a relatively homogeneously enhancing lesion in the lateral ventricle, and the posterior pituitary gland was not hyperintense on T1-weighted imaging. Subependymoma was suspected, and tumor

An unusual case of intestinal obstruction due to abdominal cocoon: A case report.

INTRODUCTION AND IMPORTANCE: Abdominal cocoon (AC) or Encapsulating Peritoneal Sclerosis (EPS) is a rare cause of bowel obstruction and due to non-specific presentation, it can be misdiagnosed and often mistreated. CASE PRESENTATION: We present the case of 42 years male with a history suggestive of complete small bowel obstruction (SBO) without a history of pulmonary tuberculosis (TB) or peritoneal dialysis. CT imaging as well as the intraoperative finding of a cocoon membrane encasing the small bowel led to the diagnosis of abdominal cocoon. CLINICAL DISCUSSION: Abdominal cocoon can be idio

Computed tomography angiography study of variations of the celiac trunk and hepatic artery in 100 patients.

OBJECTIVE: To describe the main anatomical variations of the celiac trunk and the hepatic artery at their origins. MATERIALS AND METHODS: This was a prospective analysis of 100 consecutive computed tomography angiography studies of the abdomen performed during a one-year period. The findings were stratified according to classification systems devised by Sureka et al. and Michels. RESULTS: The celiac trunk was "normal" (i.e., the hepatogastrosplenic trunk and superior mesenteric artery originating separately from the abdominal aorta) in 43 patients. In our sample, we identified four types of

[Case of obstructive sleep apnea possibly having led to postoperative appearance of generalized convulsion].

Adverse surgical outcomes appear to be more frequent in patients with known obstructive sleep apnea (OSA). However, OSA patients may present for surgery without a prior diagnosis. A 37-year-old man underwent craniotomy for surgical direct neck clipping of the right ruptured internal carotid aneurysm. His intraoperative and early postoperative courses were uneventful. At night, about 48 hr after surgery, he developed sudden generalized tonic-clonic convulsion and temporary depressed consciousness resulting in marked hypercapnea (Pa(CO2)>100 mmHg). His respiration was transiently supported by PS

[Anesthesia for scoliosis surgery: preoperative assessment and risk screening of patients undergoing surgery to correct spinal deformity].

Spinal column deformity is associated with potentially serious alterations of respiratory and cardiac function. Such alterations, in association with the underlying disease that caused the deformity--such as neuromuscular disease--are risk factors that affect the prognosis for scoliosis patients undergoing anesthesia and surgery. It is important for the anesthesiologist to carry out a detailed preoperative assessment to identify patients at risk with the aim of decreasing morbidity and mortality related to surgical correction of deformities. Of paramount importance is awareness of the risk of

The keeled acromion: an aggressive acromial variant--a series of 20 patients with associated rotator cuff tears.

This study describes our experience with an unusual type of acromial spur. In the last 7 years, we have treated 20 patients with a previously undescribed "keeled" acromion. The keel refers to a central, longitudinal, downward sloping spur on the acromial undersurface. Patients with a keeled acromion are at significant risk of bursal-sided as well as full-thickness rotator cuff tears. All patients in this series had significant bursal-sided tears, and 12 of 20 patients (60%) had full-thickness rotator cuff tears associated with an acromial "keel." In addition, patients with a keeled acromion ar
