Evaluating perfusion and metabolic responses of microvascular free flaps to ischemia, reperfusion and fluid resuscitation during septic shock in a large animal model.

Microvascular free flap transfer is an essential procedure in head and neck reconstructive surgery, with total flap loss representing the primary complication. Sepsis has been identified as a critical risk factor contributing to flap failure. Here we aimed to evaluate the impact of septic shock on free flap vitality using a large animal model and further investigated the flap-specific effects of different fluid resuscitation strategies on flap viability and metabolic response. A prospective, randomized trial was conducted on 31 juvenile male pigs to assess the effects of 3 fluid resuscitation

Editorial Commentary: Epinephrine Solution in Arthroscopic Irrigation Fluid Is Marginally Superior to Intravenous Tranexamic Acid, but Should We Focus Instead on Using Ringer's Lactate Instead of Normal Saline for Arthroscopic Clarity?

Using a double-blinded randomized technique, a matched cohort of shoulder arthroscopic surgeries was compared and evaluated for visual clarity of the surgical field. Epinephrine in a 0.9% saline irrigation solution was marginally superior to an intravenous infusion of tranexamic acid. It is unknown whether 0.9% saline is inferior to a solution of Ringer's lactate irrigation when evaluating loss of clarity as a result of intraoperative bleeding during arthroscopy.

Time to First Culture Positivity for Gram-Negative Rods Resistant to Ceftriaxone in Critically Ill Adults.

BACKGROUND: The optimal timing for the de-escalation of broad-spectrum antibiotics with activity against Pseudomonas aeruginosa and resistant Gram-negative rods (GNRs) in critically ill adults remains unknown. RESEARCH QUESTION: We tested the hypothesis that cultures will identify GNRs that ultimately demonstrate resistance to ceftriaxone within 48 hours, potentially allowing safe de-escalation at this time point. STUDY DESIGN AND METHODS: We conducted a secondary analysis of data from the Isotonic Solutions and Major Adverse Renal Events Trial: a pragmatic, cluster-randomized, multiple-cros…

Resuscitation for Hypovolemic Shock.

Hemorrhage is the leading cause of preventable deaths in trauma patients. After presenting a brief history of hemorrhagic shock resuscitation, this article discusses damage control resuscitation and its adjuncts. Massively bleeding patients in hypovolemic shock should be treated with damage control resuscitation principles including limited crystalloid, whole blood or balance blood component transfusion to permissive hypotension, preventing hypothermia, and stopping bleeding as quickly as possible.

Individual patient data meta-analysis of hydroxyethyl starch 130/0.4-0.42 versus crystalloid for fluid resuscitation in patients with severe sepsis: a statistical analysis plan.

BACKGROUND: The Crystalloid versus Hydroxyethyl Starch Trial (CHEST) and the Scandinavian Starch in Severe Sepsis/ Septic Shock (6S) trial reported that 6% hydroxyethyl starch (HES) is associated with increased use of renal replacement therapy and death in critically ill patients. Data collection was harmonised between the two trials in order to facilitate a preplanned individual patient data meta-analysis (IPDMA) of patients with severe sepsis. OBJECTIVES AND RATIONALE: To publish a statistical analysis plan (SAP) for an IPDMA of patients with severe sepsis enrolled in the 6S trial and the C

An unexpected cause of cardiac arrest during laparoscopy in an infant with supravalvar aortic stenosis.

Patients with congenital supravalvar aortic stenosis (SVAS) with associated biventricular outflow tract obstruction and coronary artery abnormalities have a tenuous myocardial oxygen supply/demand relationship. They are at increased risk of acute myocardial ischemia and sudden death, especially during anesthesia. Furthermore, resuscitation during cardiac arrest is frequently unsuccessful. We report a case of perioperative cardiac arrest due to an unexpected cause in a 2 month old with SVAS during a laparoscopic Nissen fundoplication.

[Basic study on the mechanism of postanesthetic recovery acceleration caused by infusion of bicarbonated Ringer's solution].

BACKGROUND: Effect of bicarbonated Ringer's solution (BRS) on the liver function of rats with metabolic acidosis was compared with that of lactated Ringer's solution (LRS) and Ringer's solutions (RS). Furthermore, the effect of acidosis on the plasma protein binding ratio of propofol was examined. METHODS: Partial hepatectomized rats were divided into three groups, and BRS, LRS or RS was intravenously administered at 20 ml x kg(-1) x hr(-1) with propofol (45 mg x kg(-1) hr(-1)) for 90 min. Immediately after the administration, indocyanine-green (ICG) was infused. Blood sampling was done at 30

Pre-operative intravenous fluid therapy with crystalloids or colloids on post-operative nausea & vomiting.

BACKGROUND & OBJECTIVE: Post-operative nausea and vomiting (PONV) is a frequent complication and may be a reason for increased morbidity and cost of treatment. Following elective surgery, it is believed to result from gut ischaemia consequent to hypovolemia from overnight fasting. This study was carried out to study the effects of pre-operative intravenous fluid supplementation, either crystalloids or colloids, on PONV. METHODS: In this prospective randomized clinical trial, 60 female patients undergoing elective open cholecystectomy were randomly allocated to three equal groups A, B and C. A

Albumin versus crystalloid therapy in the management of hepatorenal syndrome: a model for using meta analysis in cost effectiveness studies and the design of clinical trials.

We applied traditional methods of gathering, integrating and summarizing findings of current literature, with new approaches for assessing the cost effectiveness of two treatments for hepatorenal syndrome (HRS). Findings of this cost effectiveness study are used to form a proposal for a multi-center prospective clinical trial, to assess the economic and clinical benefits of albumen versus crystalloid therapy in the care of these patients. Our initial findings suggest that albumin therapy is superior to standard crystalloid therapy, in the treatment of HRS patients. The number of survival days

Colloids versus crystalloids and tissue oxygen tension in patients undergoing major abdominal surgery.

Источник пометил публикацию как отозванную

UNLABELLED: The effects of intravascular volume replacement regimens on tissue oxygen tension (ptiO(2)) are not definitely known. Forty-two consecutive patients scheduled for elective major abdominal surgery were prospectively randomized to receive either 6% hydroxyethyl starch (HES) (mean molecular weight 130 kd, degree of substitution 0.4, n = 21) or lactated Ringer's solution (RL, n = 21) for intravascular volume replacement. Fluids were administered perioperatively and continued for 24 h on the intensive care unit to keep central venous pressure between 8 and 12 mm Hg. The ptiO(2) was meas

HCO3- potentiates the cAMP-dependent secretory response of the human distal colon through a DIDS-sensitive pathway.

We used the Ussing short-circuit technique to investigate the role of HCO3- in the adenosine 3',5'-cyclic monophosphate (cAMP)-dependent secretory response of the human distal colon. In HCO3(-)-free 4-(2-hydroxyethyl)-1-piperazineethanesulphonic acid (HEPES)-Ringer's, forskolin (10 mumol l-1 mucosal and serosal) evoked a sustained increase in short-circuit current (Isc) (delta Isc = 24 +/- 3 microA cm-2, n = 57). However, this was only 25% of the forskolin-stimulated Isc response in HCO3(-)-Ringer's (delta Isc = 84 +/- 8 microA cm-2, n = 57). The reduced response to forskolin in HCO3(-)-free H

[Decrease in serum propofol concentrations after acute autologous blood letting].

A change in serum propofol concentrations associated with acute autologous blood letting during anesthesia was investigated in seven scheduled surgical patients. Anesthesia was induced with propofol 2 mg.kg-1 and maintained with infusion of propofol 6 mg.kg-1.hr-1 at a constant rate. After achieving a stable anesthesia, about 10 g.kg-1 of autologous blood was withdrawn in about 15 minutes and 20 ml.kg-1 of acetated Ringer's solution was infused to manage the hypotension caused by withdrawal. A blood sample each 4 ml was taken before and 0, 5, 15, 30 minutes after blood withdrawing. Another 7 p

Effect of volume expansion induced by Ringer's solution or plasma on the natriuretic response of the kidney.

Studies were carried out in anaesthetized dogs, to determine the effects of volume expansion induced by Ringer's solution, plasma taken from non-hydrated or previously volume expanded animals on the natriuretic response of the kidney. When the animals were infused with Ringer's solution (0.25 ml/kg/min iv.) water and sodium excretion increased gradually reaching a peak value at about 70 min. after starting the infusion (3.01 +/- 0.21 ml/min and 329 +/- 23.9 mmol/min, respectively). Infusing the dogs with plasma taken from non-hydrated animals (0.25 ml/kg/min iv.) water and sodium excretion sho
