Three types of expandable metallic stents were used to relieve obstructive jaundice in 59 patients. They consisted of 3 cases with benign stricture, and 56 with malignant obstruction including 28 of cholangiocarcinoma, 17 of pancreatic carcinoma, 9 of lymph node metastases, and 2 of gall bladder carcinoma. The median age of patients was 68.9 years. Of 56 cases with malignant obstruction, 51 cases were able to remove external drainage catheter. In these 51 cases, 35 patients died, and 16 are still alive. No significant difference was noted in the incidence of stent destruction or migration in t

BACKGROUND: Since it was first described in 2001, endoscopic ultrasonography-guided biliary drainage (EUS-BD) has emerged as an alternative procedure for achieving an endoscopic internal drainage in case of endoscopic retrograde cholangiopancreatography (ERCP) failure. Biliary drainage can be achieved by either a transduodenal extrahepatic approach through EUS-guided choledochoduodenostomy (EUS-CDS), or a transgastric intrahepatic approach, namely EUS-guided hepaticogastrostomy (EUS-HGS) which already holds a remarkable place in the treatment of patients with malignant biliary obstruction.

Merkel cell carcinoma (MCC) is a rare and aggressive primary neuroendocrine tumor of the skin. Gastrointestinal (GI) metastasis in MCC is uncommon. We present a case of MCC with metastasis to the stomach, duodenum, and pancreas presenting with melena and obstructive jaundice. A large, bleeding metastatic mass was identified in the duodenum. Hemostasis was achieved with coil embolization. Endoscopic retrograde cholangiopancreatography with stenting of the common bile duct was performed to relieve the obstruction. Close surveillance with positron emission tomography/computed tomography scan and

Gastric mucosal defensive factor plays an important role in the pathogenesis of various gastric diseases as well as aggressive factor such as acid. Gastric mucosal defensive factor of various gastric diseases and the relation to acid secretion were studied. The influence of operative insult and obstructive jaundice on gastric aggressive and defensive factors was also studied. Defensive factor was investigated using gastric transmucosal potential difference (PD). The results obtained were as follows: PD showed low value in the gastric diseases with the mucosal lesion such as gastric ulcer, stre

BACKGROUND: Percutaneous transhepatic biliary drainage (PTBD) is widely used in malignant obstructive jaundice (MOJ), but most series report aggregate complication rates without stratification by tumor type. This limits individualized risk counseling and hampers benchmarking across centers.
OBJECTIVE: To evaluate short-term outcomes of PTBD in a large single-center cohort, with a focus on tumor-specific complication patterns using patient-level attribution and robust statistical methods.
METHODS: We retrospectively analyzed 453 patients with MOJ undergoing PTBD (2017-2022). Complications wit

To compare the safety and efficacy of endoscopic retrograde cholangiopancreatography (ERCP) and percutaneous transhepatic cholangial drainage (PTCD) in the treatment of malignant obstructive jaundice, a systematic review and meta-analysis of published studies was undertaken to assess the differences between the two procedures in terms of efficacy and safety. From November 2000 to November 2022, the Embase, PubMed, MEDLINE, and Cochrane databases were searched for randomized controlled trials (RCTs) on the treatment of malignant obstructive jaundice with ERCP or PTCD. Two investigators independ

INTRODUCTION: Non-Hodgkin lymphoma of the duodenum with manifestation of hematemesis, melena, and obstructive jaundice is a rare occurrence.
CASE PRESENTATION: A 47-year-old Indonesian man presented with symptoms of hematemesis of 3×/day, melena, abdominal pain, decreased appetite, and a weight loss (2 kg). The patient had symptoms of weakness, jaundice, pale conjunctiva, and experienced palpable discomfort in the epigastric region (VAS of 6). An abdominal ultrasonography revealed a heterogeneous cystic tumor mass measuring 45 × 29 mm located in the pancreas head. The gastrointestinal endosco

CONTEXT: Ectopic pancreatic rest is an uncommon condition resulting in diverse clinical and pathological presentation. It results from altered development of two primitive pancreatic buds that fuse to form the uncinate-head and body-tail of normal gland. Ectopic pancreas is an anomaly where an ectopic rest develops at a place away from the normal site.
CASE REPORT: We describe a 48-year-old male patient who presented with progressive jaundice and pruritus. He was established to have a periampullary mass highly suggestive of malignancy, for which he undergo pancreaticoduodenectomy. However, hi

BACKGROUND: Obstructive jaundice poses diagnostic and therapeutic challenges to general surgeons practicing in resource-limited countries. This study was undertaken to highlight the etiological spectrum, treatment outcome of obstructive jaundice in our setting and to identify prognostic factors for morbidity and mortality.
METHODS: This was a descriptive prospective study which was conducted at Bugando Medical Centre between July 2006 and June 2010. All patients with a clinical diagnosis of obstructive jaundice were, after informed consent for the study, consecutively enrolled into the study.

CONTEXT: Obstructive jaundice in chronic pancreatitis is generally caused by stenosis of the bile duct in the pancreas. Obstructive jaundice caused by the impaction of a pancreatic stone in the papilla is markedly rare, with only seven cases reported to date.
CASE REPORT: We report a case of obstructive jaundice caused by the impaction of a pancreatic stone in the papilla. A 41-year-old male with chronic pancreatitis was admitted because of epigastric pain. Blood analysis revealed increased levels of hepatobiliary system enzymes, mild jaundice and an increase in pancreatic enzymes. Computed t

The necessity of biliary drainage before pancreaticoduodenectomy remains controversial in cases involving malignant obstructive jaundice; however, the benefits of biliary drainage have been reported in cases with severe hyperbilirubinemia. Herein, we present the case of a 61-year-old man suffering from jaundice due to distal cholangiocarcinoma. In this case, obstructive jaundice was refractory to repeat endoscopic drainage and bilirubin adsorption. Hyperbilirubinemia persisted despite successful implementation of biliary endoscopic sphincterotomy and two rounds of plastic stent placements. Ste

A man in his 60s had end-stage alcoholic cirrhosis. About six months before his death, hepatic peribiliary cysts (HPBC) rapidly increased, and he developed jaundice and liver failure. The pathological autopsy performed after his death revealed that his intrahepatic bile duct was pressured due to multiple cysts caused by HPBC, which resulted in liver failure. Some cases of HPBC have been associated with alcoholic cirrhosis;however, no other cases of increased HPBC in a short period of time have been reported. Although identifying the cause of increased HPBC in a short time is difficult in this
