Imaging in sports-medicine--knee.

Since the last decade, the introduction of Magnetic Resonance Imaging has provided a powerful new tool for the clinician to diagnose sports-related knee injuries. The main objective of this paper is to familiarise the relevant specialists with the proven clinical indications and semiology of MRI of the knee. At the present time, the knee joint is the area in which the advantages of MRI are most spectacularly apparent. The MR appearances of meniscal tears in particular, are first discussed in detail, referring to recent literature and personal experience based on more than ten thousands MR exam…

[Revision surgery of knee endoprosthesis].

Thirty-one revisions for aseptic or septic loosening of knee arthroplasty have been performed between 1983 and 1995. In 18 cases we had loosening of uni- or bicondylar prosthesis and in 13 cases a tricompartmental revision arthroplasty. With an average of 53 months (1.5 to 13 years) after the last operation 21 patients could be examined. Main reasons for failure of uni- and bicondylar prosthesis were as well a proceeding of the arthritis in other compartments, instability, incorrect alignment and other reasons depending on the surgical technique. We found similar reasons in aseptic loosening o…

Knee in congenital short femur.

The abnormalities of the knee associated with congenital short femur (CSF) and proximal femoral focal deficiency (PFFD), have been recognized for many years. The emergence of new and more effective lengthening techniques that can correct major congenital leg length discrepancies has made the abnormalities of the knee much more relevant. Some investigators have reported a high incidence of knee subluxation and dislocation during lengthening procedures in patients with CSF, attributing the incidence to knee anomalies. We assessed knee function in a group of 24 patients with CSF. Detailed evaluat…

Ipsilateral hip and knee dislocation.

Hip and knee dislocations individually are two of the relatively limited orthopaedic emergencies. Long-term results of treatment of these two types of injury are associated with avascular necrosis of the femoral head, knee instability, and knee stiffness. Correct early diagnosis, including arteriography in the case of knee dislocation, is crucial. Prompt treatment consisting of closed or open reduction of the dislocations is necessary. Additional treatment involving knee ligament reconstruction is warranted to maximize knee function in healthy, active patients. Simultaneous occurrence of ipsil…

Noncemented revision total knee arthroplasty.

From 1985 to 1991, 16 patients underwent 17 revision total knee arthroplasties using porous-coated tibial or femoral components, or both, without cement. Eight knees had both components implanted without cement, 4 the femoral component without cement and 5 the tibial component without cement. Fifteen revisions were performed for aseptic loosening, 1 for instability, and 1 for infection. Patients were evaluated by clinical examination, radiographs, and the Hospital for Special Surgery Knee Rating Scale. Two patients died, leaving 15 revision total knee arthroplasties for followup. The average a…

Knee laxity in symptomatic osteoarthritis.

Twenty-two patients with primary osteoarthritis (OA) of the knee were studied to determine the effects of OA on laxity of the knee joint. Laxity was measured with a Genucom Knee Analysis System. Ten knees had mild OA (> 50% preservation of joint space). Fifteen knees had moderate OA (some preservation of joint space, but < 50%). Eighteen knees had severe OA (no joint space). A group of 18 knees from 9 healthy (asymptomatic) subjects of ages similar to those of the OA patients were used as controls. Compared to control knees, severe OA knees had less total anteroposterior (AP) translation (12.2…

Late knee problems in myelomeningocele.

This study was undertaken to assess the incidence and etiology of knee problems in a long-term follow-up of myelomeningocele patients. Of the 72 community ambulators reviewed, 17 (24%) had significant knee symptoms. A specific gait pattern was identified in symptomatic patients with low lumbar lesions, which may be explained on the basis of hip abductor and calf muscle weakness. These patients have a characteristic gait, which places abnormal stress on the knee, leading to medial and anteromedial rotary instability and eventual degenerative change. It is likely that disability resulting from k…

Temperature elevation during knee arthroplasty.

The temperature in the tibia and in the saw blade were recorded during 30 knee arthroplasty operations. In the saw blade the median maximum temperature was 68 degrees C (45-100 degrees C). The corresponding bone temperatures at two levels approximately 2 and 3 mm below the cutting surface were 47 degrees C and 42 degrees C, respectively. Irrigation of the saw area with physiologic saline solution had only a minimal effect. During the cement-curing process, the temperature was 37 degrees C (31-50 degrees C) at the cement-bone interface. We conclude that the cutting procedure generates heat abov…

Hematogenous infection after knee arthroplasty.

Twenty-five hematogenously infected knee arthroplasties in 20 patients (17 with rheumatoid arthritis and 3 with arthrosis) were followed for 3 years. Staphylococcus aureus was the major infecting organism. Three patients with four arthroplasties died of sepsis. Two patients had removal of the arthroplasty, one of which resulted in an above-the-knee amputation. Four out of five arthrodeses fused. Two knees healed after early debridement and two healed without surgery. Ten knees had successful revision arthroplasty. Rheumatoid arthritis and constrained prostheses increase the risk of hematogenou…

Knee flexion contractures in myelodysplasia.

From 1976 to 1982, 12 children with myelodysplasia had surgery at our institution for knee flexion contractures (21 affected knees). Follow-up was a minimum of 18 months and averaged 47 months. Soft tissue lengthening of the hamstring tendons and release of the posterior capsule with or without femoral osteotomy yielded uniformly excellent results; femoral osteotomy alone produced 40% unsatisfactory results. Based on these data, we no longer recommend osteotomy alone as an effective treatment for fixed knee flexion contractures in children with myelodysplasia.

[Complications following total knee replacement].

Complications of total knee replacement were studied by following up 218 cases operated in our clinic. The average follow-up time ranged from 24 months to 101 months (mean 44.3 months). The three most serious types of complication were: significant loosening in 8 knees; deep infection in 5 knees; and fracture in 7 knees. Patellar dislocation and extension mechanism rupture were found in 13 knees, restriction of ROM and instability in 20 knees, and other complications in 7 knees. The incidence of complications in the rheumatic group tended to be greater than that in the osteoarthritic group. Th…

Arthroscopy in acute knee injuries.

The diagnostic value of arthroscopy was evaluated in 148 patients with acute hemarthrosis and/or instability of the knee. The treatment planned after clinical examination was compared with the treatment given after arthroscopy. Seventy-nine per cent of the patients had ligamentous injuries; 59 per cent of tears were combined with other injuries, and 71 per cent were complete ruptures. Stability testing under anesthesia was most inaccurate for the anterior cruciate ligament, with 13 per cent false positive and 30 per cent false negative results. The planned treatment was altered as a consequenc…

Bilateral hip and knee arthroplasty.

Twenty-six severely deformed patients with prosthetic arthroplasty of both hips and both knees because of rheumatoid arthritis (16), juvenile rheumatoid arthritis (6), ankylosing spondylitis (2), osteoarthritis (1), and inflammatory arthritis, cause unknown (1), were followed for a mean of 6.8 years. Severe pain was the primary indication for surgery, and consistent and substantial pain relief was achieved. Most patients also had limited ambulatory and functional improvement. Computer analysis showed that this improvement was related only to preoperative walking and function and a combined hip…

[Treatment of knee joint empyema].

This report deals with 42 patients suffering from purulent infections following aseptic operations, perforating soft tissue injuries or implantation of endoprostheses. By early revision of the joint in combination with gradually increasing physiotherapy (under medical supervision!) in all cases except endoprostheses the infection could be brought under control and a satisfactory functional result achieved. In all cases of infected endoprostheses the prosthesis could not be preserved, the infection of the knee-joint could only be cured by removing the foreign body and arthrodesis.

Below-knee amputee running gait.

The running ability of ten unilateral below-knee amputees was examined by analyzing the temporal and length characteristics of their running strides and the angular displacement patterns of their intact and prosthetic limbs. Six of the individuals tested were able to run and in several cases their locomotor patterns were similar to those of non-amputees. The running gait of three of the amputee runners, however, did contain one particular undesirable element namely an excessively straight residual knee during at least a portion of support on the prosthesis. Although several explanations could …

[Allotendinoplasty in knee joint injuries].

The experience of surgical treatment of 51 patients with fresh and old injuries of the knee joint is analyzed. The operation consisted in plastic of the integrity of the injured patella, extension apparatus, cruciate ligaments of the knee joint. Allogenic tendinous transplants of musculus peroneus longus, extensor of the foot toes, flexor of fingers conserved by freezing were used. Good results of the treatment were obtained in 43 patients, satisfactory-in 5 and unsatisfactory-in 3 patients.

Knee ligament injuries in children.

Nine children who were less than fourteen years old and had open physes and ligament injuries of the knee were studied. Despite an initial thorough physical and roentgenographic evaluation, the full extent of the lesion was determined only at operation in seven of the nine patients. The intercondylar eminence of the tibia was avulsed in five patients, four of whom had associated collateral ligament injuries and a positive anterior drawer sign. Despite surgical repair, all nine patients demonstrated some degree of postoperative ligament instability. This was increased in those patients in whom …

The Stanmore hinged knee arthroplasty.

The Stanmore hinged total knee replacement was introduced in 1969 for severe destructive arthropathy of the knee, and the results of one hundred consecutive operations are presented after an average interval of two and a half years. Insertion of the prosthesis relieved pain in 94 per cent, improved the range of movement in 67 per cent, invariably restored stability and corrected valgus or varus deformity. Gross flexion contractures were improved but not always fully corrected. Serious complications were few, though of three cases of deep infection two came to amputation and one to fibrous anky…
