Cell quality affects clinical outcome after MACI procedure for cartilage injury of the knee.

The aim of our study was to test the hypothesis that in early follow up after matrix guided autologous chondrocyte implantation (MACI), clinical results do not correlate with radiological and histological results, and that MACI as first line procedure and treatment of traumatic cartilage defects leads to better results compared to second line treatment and treatment of degenerative defects. Six and twelve months after MACI, patients IKDC-score was analysed, as well as the results of MRI-examinations. Specimens of the scaffold were histologically assessed at the time of implantation. The IKDC-s

[Dislocation and fractures around the knee with popliteal artery injury: A retrospective analysis of 54 cases].

A retrospective multicentric series of 54 cases of knee trauma with acute ischemia by popliteal artery injury were analyzed. These high-energy traumas involved 25 dislocations and 29 fractures, 11 involving distal femur, 15 the proximal tibia and 3 a floating knee. There were 45 men and 7 women, mean age 42 years. Thirty-three patients suffered multiple injuries. The knee injury was open in 25 cases and associated with sciatic paralysis in 32. Vascular repair was almost always achieved with bypass surgery. An external fixator was used in 39 patients. Vascular repair was unsuccessful in three c

Extensor spasms triggered by imposed knee extension in chronic human spinal cord injury.

Extensor spasms, which are a significant component of spasticity in spinal cord injury (SCI), are still incompletely understood. In this study, contributions of knee proprioceptors to the origination of extensor spasms were examined in fifteen subjects with SCI. Ramp and hold knee extension perturbations were imposed to one leg while the hip and ankle were held in an isometric position using an instrumented leg brace. Isometric joint torques of knee, ankle and hip, and electromyograms (EMGs) from six muscles of the leg were recorded following controlled knee extension at four different velocit

Acute injury of the articular cartilage and subchondral bone: a common but unrecognized lesion in the immature knee.

OBJECTIVE: We studied the prevalence of injuries of the articular cartilage and subchondral bone after acute trauma in skeletally immature knees using high-resolution MRI. MATERIALS AND METHODS: We reviewed knee MRIs of 126 young children and adolescents suspected to have internal knee derangement, including 82 with open physes and a control group of 44 who were skeletally mature. High-resolution proton density and T2-weighted pulse sequences were used in all patients. The prevalence of common injuries in the two groups was compared using chi-square analysis. Levels of interobserver agreement

Of needles and skinned knees: children's coping with medical procedures and minor injuries for self and other.

Children participated in four role-plays designed to assess what the children themselves would do and what they would suggest a friend should do when encountering a medical procedure and a minor injury. Open-ended responses were coded into an empirically derived continuum suggested by past research. Similar responses were given to cope with medical procedures and injuries. However, children suggested more reactive coping strategies (e.g., cry, pull away) for themselves and more proactive responses (e.g., think of something fun, take deep breaths) for friends. This finding questions the assumpt

Handedness as a predictor of increased risk of knee, elbow, or shoulder injury, fractures and broken bones.

Several recent studies suggest that left-handers have higher susceptibility to accidental injury. Two studies investigated the suggested link between sinistrality and accidental injury by assessing the prevalence of joint problems, bone breaks, and fractures, which may be the sequelae of such mishaps. Study 1 used retrospective responses on medical history checklists completed by 1064 males on active duty with the United States Air Force. It found that left-handers were at significantly higher risk for knee problems (RR = 2.74), elbow and shoulder problems (RR = 3.03), and also swollen and pai

Neuromuscular characteristics of agonists and antagonists during maximal eccentric knee flexion in soccer players with a history of hamstring muscle injuries.

BACKGROUND: Muscle strain injuries (MSIs) in the hamstrings are among the most prevalent injuries in elite soccer. We aimed to examine the relation between biomechanical maladaptation in eccentric strength and neuromuscular factors separated by their time and frequency domains. METHODS: 20 elite soccer players with a previous history of unilateral MSI in the M. biceps femoris (BF) long head and 20 without MSI participated. Knee flexion torques, rate of torque development (RTD) and electromyographic signals (EMG) of the BF, the M. semitendinosus (SMT) and knee extensors were obtained during un

Injuries to the posterolateral aspect of the knee accompanied by compression fracture of the anterior part of the medial tibial plateau.

We present 12 cases of patients with injury to the posterolateral aspect of the knee accompanied by a compression fracture of the anterior part of the medial tibial plateau. There were 11 male patients and 1 female patient with an average age of 26 years (range, 17 to 44 years). There were 4 cases of posterolateral rotatory instability and 8 cases of straight lateral instability of the knee. The size of the compression fracture was classified into 2 types, small (8 cases) and large (4 cases). Although the mechanism of injury was considered to be hyperextension and varus force, the pattern of c

Anatomic Multiligament Knee Reconstruction With a Medial Meniscal Repair in the Setting of a Valgus Hyperextension Injury.

BACKGROUND: Simultaneous anterior cruciate ligament (ACL), posterior cruciate ligament (PCL), and medial collateral ligament (MCL) tears represent a severe form of multiligament knee injury (MLKI). This combination of injuries often results from high-energy trauma and leads to gross instability, impaired joint biomechanics, and long-term degenerative changes if not addressed appropriately. Untreated or improperly treated MLKIs may result in chronic pain, instability, and early onset osteoarthritis. INDICATIONS: Combined ACL, PCL, and MCL reconstruction is indicated in patients with complete t

Time to First Injury After Knee Surgery in U.S. Army Soldiers: A Survival Analysis.

BACKGROUND: Knee injuries, including those requiring surgical intervention, are a leading cause of disability in U.S. Army soldiers. Previous studies investigating the risk of injury after surgery have primarily focused on civilian populations and lack specific information on the timing of new injuries. OBJECTIVE: The purpose of this study was to identify the windows of peak injury risk after knee surgery in active-duty soldiers. We hypothesized that injury risk would peak around return to duty and remain elevated beyond completion of post-operative rehabilitation. METHODS: U.S. Army adminis…

Intra-articular Entrapment of the Common Peroneal Nerve in a Multiligamentous Knee Injury: A Case Report.

CASE: A 25-year-old severely obese woman sustained a knee injury with foot drop after an electric scooter crash. Initially diagnosed with a knee sprain and advised to follow-up with her primary care physician, she was later diagnosed with chronic multiligamentous knee instability and complete common peroneal nerve (CPN) palsy. Surgery 7 months postinjury demonstrated a severed intra-articular CPN scarred to the anterior cruciate ligament stump, necessitating arthroscopic-assisted extrication. CONCLUSION: Clinicians should maintain high suspicion for ligament injury and peroneal palsy after lo

Refined Detection and Classification of Knee Ligament Injury Based on ResNet Convolutional Neural Networks.

Currently, medical imaging has largely supplanted traditional methods in the realm of diagnosis and treatment planning. This shift is primarily attributable to the non-invasive nature, rapidity, and user-friendliness of medical-imaging techniques. The widespread adoption of medical imaging, however, has shifted the bottleneck to healthcare professionals who must analyze each case post-image acquisition. This process is characterized by its sluggishness and subjectivity, making it susceptible to errors. The anterior cruciate ligament (ACL), a frequently injured knee ligament, predominantly affe
