Role of second look laparotomy in multidisciplinary treatment and in the follow up of advanced ovarian cancer.

Surgical re-examination after first line surgery followed by chemotherapy is an absolutely necessary step in the course of integrated treatment of advanced ovarian epithelial tumours. Our personal experience of surgical re-staging and reductive surgery in 48 patients affected by ovarian tumours stage III and IV (FIGO), by laparotomic second look is reported. Repeated surgery succeeds in further reducing the neoplastic deposits or radicalizing the first intervention in 33% of the overall series; in 52% of the cases, the second look was negative for neoplasia; in 16% only, despite tumour finding

Are Fewer Damage Control Laparotomies Associated with a Decrease in the Rate and Severity of Surgical Site Infections?

BACKGROUND: Damage control laparotomy (DCL) rates have declined and remained low. Given DCL's association with surgical site infections (SSIs), we hypothesize that SSI rates and severity, as per Clavien-Dindo (CD) classification, have decreased concurrently. PATIENTS AND METHODS: A retrospective study was performed on patients (≥16 y) who underwent trauma laparotomy from January 2011 to December 2020. Patients who survived ≤48 hours were excluded. Data were divided into period 1 (January 2011-December 2015, DCL rates declined) and period 2 (January 2016-December 2020, low DCL rates were susta…

The weight of risk: BMI and postoperative outcomes in older adults with sepsis undergoing emergency laparotomy.

BACKGROUND: Older adults with sepsis undergoing emergency laparotomy face high postoperative risk. We aimed to evaluate the association between body mass index and postoperative outcomes in this high-risk population. METHODS: We used American College of Surgeons National Surgical Quality Improvement Program 2013-2017 to identify patients aged ≥65 years who underwent emergency exploratory laparotomy with preoperative sepsis. Body mass index was categorized into 6 standard groups. The primary outcome was 30-day mortality. Secondary outcomes included in-hospital mortality, respiratory complicati…

Abdominal computed tomography prior to trauma laparotomy: a roadmap or detour? A pro-con debate.

The use of abdominal computed tomography (CT) for trauma has grown tremendously in the past several decades, to the point that it is now a regular part of the trauma work-up. Increased utilization of CT has reduced non-therapeutic laparotomy for trauma, which is associated with significant morbidity. Traditionally, findings on physical examination including hemodynamic instability, evisceration, and peritonitis were indicators to proceed with laparotomy without preoperative imaging. However, growing use of and comfort with CT imaging has led to a shift in practice during which patients who mee…

Conversion to laparotomy during laparoscopic hysterectomy: a meta-analysis of prevalence and key risk factors.

BACKGROUND: This meta-analysis aimed to estimate the prevalence and identify risk factors for conversion to laparotomy during laparoscopic hysterectomy (LH) for both benign and malignant gynecologic conditions. METHODS: A comprehensive search of PubMed, Embase, and the Cochrane Library was conducted to identify studies published between January 2000 and September 2024. Eligible studies reported the prevalence and risk factors for conversion to laparotomy in patients undergoing LH. Studies were assessed for quality using the Newcastle-Ottawa Scale (NOS), and data were extracted on patient demo…

The underestimated burden of postoperative pulmonary complications in emergency laparotomy: a propensity score-matched analysis.

INTRODUCTION: Emergency laparotomy (EL) remains one of the highest-risk procedures in general surgery, characterized by substantial postoperative morbidity and mortality. Despite major advances in perioperative medicine, current enhanced-recovery and infection-prevention protocols are largely derived from elective surgical cohorts and are seldom validated in emergency settings. This lack of EL-specific data represents a critical gap in establishing evidence-based perioperative guidelines for this uniquely vulnerable patient population. Among postoperative complications, pulmonary complications…

Maternal laparotomy to fetoscopic myelomeningocele repair in patients with Class 2 obesity is safe and feasible.

BACKGROUND: There is minimal to no data on fetal surgery outcomes in the setting of body mass index (BMI) ≥ 35. Theoretical concerns exist for increased maternal risks from surgery and increased surgical difficulty due to maternal body habitus. OBJECTIVE: To evaluate the pregnancy and operative outcomes with maternal laparotomy to 3-port fetoscopic myelomeningocele (MMC) repair in the setting of pre-pregnancy BMI 35-40 compared with pre-pregnancy BMI < 35. METHODS: This was a cohort study at a single maternal fetal care center in which maternal laparotomy to 3-port fetoscopic surgery for MMC…

Management of open abdomen complication after laparotomy in an ovarian cancer patient with intraperitoneal metastasis.

This case report highlights the management of complications from an open abdomen following surgery for ovarian mucinous adenocarcinoma, a rare subtype of ovarian cancer. A 63-year-old female underwent extensive surgery, including single-port laparoscopic total bilateral salpingo-oophorectomy, right hemicolectomy, small bowel resection, cholecystectomy, and jejunostomy. Postoperatively, she experienced bile leakage, leading to significant skin and fascial damage and an abdominal skin defect. Early detection and multidisciplinary management were crucial. Treatment involved vacuum-assisted closur

Development and validation of a prognostic model for death 30 days after adult emergency laparotomy.

The probability of death after emergency laparotomy varies greatly between patients. Accurate pre-operative risk prediction is fundamental to planning care and improving outcomes. We aimed to develop a model limited to a few pre-operative factors that performed well irrespective of surgical indication: obstruction; sepsis; ischaemia; bleeding; and other. We derived a model with data from the National Emergency Laparotomy Audit for patients who had emergency laparotomy between December 2016 and November 2018. We tested the model on patients who underwent emergency laparotomy between December 20…

Incidence of wound dehiscence in patients undergoing laparoscopy or laparotomy: a systematic review and meta-analysis.

Surgical wound dehiscence (SWD) is a serious complication-with a 40% estimated mortality rate-that occurs after surgical intervention. Since the implementation of advanced recovery protocols, the current global incidence of SWD is unknown. This systematic review and meta-analysis estimated the worldwide incidence of SWD and explored its associated factors in general surgical patients. Eligible full-text cross-sectional, cohort and observational studies in English, between 1 January 2010 to 23 April 2021, were retrieved from MEDLINE, CINAHL, EMBASE and the Cochrane Library. The Effect of Sarcopenia on Postoperative Outcomes Following Emergency Laparotomy: A Systematic Review and Meta-Analysis.

Emergency laparotomy procedures have high rates of postoperative mortality and morbidity in older patient. Sarcopenia is associated with poor postoperative outcomes in elective surgeries and there is growing evidence for its use as a risk predictor in the emergency setting. The study aimed to evaluate the effect of sarcopenia on postoperative mortality and morbidity following emergency laparotomy. Five electronic databases were systematically searched (MEDLINE, EMBASE, CINAHL, Cochrane Central Register of Controlled Trials, and Web of Science) from conception until the 14th February 2022. All …

A Rare Case of an Exploratory Laparotomy to Treat a Liver Abscess Secondary to Foreign Body Ingestion.

A 49-year-old female presented to the hospital with complaints of generalized weakness, subjective fevers, and chills. In the emergency depa … uodenoscopy (EGD) was performed, but no evidence of the FB was found. This warranted an exploratory laparotomy (EL) to ensure the successful removal of the FB. Upon gross visualization by surgery, the FB was revealed to be a bone that the patient did not recall ingesting. However, surgical pathology evaluation revealed that the FB was actually a plastic stic …

Failure-to-rescue and mortality after emergent pediatric trauma laparotomy: How are the children doing?

INTRODUCTION: Emergent trauma laparotomy is associated with mortality rates of up to 40%. There is a paucity of data on the outcomes of emergent trauma laparotomies performed in the pediatric population. The aim of our study was to describe the outcomes, including mortality and FTR, among pediatric trauma patients undergoing emergent laparotomy and identify factors associated with failure-to-rescue (FTR). METHODS: We performed a one-year (2017) retrospective cohort analysis of the American College of Surgeons Trauma Quality Improvement Program dataset. All pediatric trauma patients (age <18 y…
