Histochemical analysis of frozen, thin sections revealed the distribution of alkaline phosphatase (ALPase) in 47 primary intracranial neoplasms in humans. The cytoplasm of meningioma cells exhibited the strongest ALPase reactivity. Pretreatment of these materials by levamisol indicated that the isozymes of ALPase had the characteristic liver-bone-kidney form. Immunohistochemical findings are reported from an endocrinologically active tumor of the liver hilus in a 39 year old woman. Clinical findings were dominated by difficult to control hypokalemia, cholelithiasis and mild hypoglycemia. Staging was undertaken in 118 patients with primary lymph node neoplasms; the sensitivity of computer tomography in the paraaortic region was 80%, that of lymphography 89%. Specificity of computer tomography was 93%, of lymphography 95%. In the iliac region, sensitivity was 81% (CT) and 90% (lymphography), and specificity was 90% (CT) and 97% (Lymphography). The value of computer tomography should, however, be stressed, since it can demonstrate lymph nodes not shown by lymphography, including those in the mediastinum, as well as lesions in the spleen, liver and lungs.

Twnty-six children suspected of having malignant diseases were examined by Gallium-67 scintigraphy. The scintigrams were pathologic in the 20 children with final diagnoses of malignancy. The lesions visualized by scintigraphy were 12 primary tumors and eight cases of local recurrence or dissemination of cancer. Gallium-67 did not accumulate in a hematoma of the liver, and scintigraphy was normal in five children with no clinical evidence of tumor recurrence. Several cases are reported and the clinical value of 67Ca scintigraphy for detection and control of childhood neoplasms is emphasized.

Ten cases of spontaneous perforation of subcapsular bile ducts in patients with opisthordiasis are analysed. In 8 of them perforation of the bile duct developed against the background of purulent cholangitis. Epstein-Barr virus-associated smooth-muscle tumors (EBV-SMTs) are unique and rare neoplasms described in immunocompromised patients. The case describes a nine-year-old female with a history of acute lymphoblastic leukemia with relapse and subsequent allogeneic bone marrow transplantation who presented with multiple EBV-SMTs of the liver. EBV utilizes the mammalian target of rapamycin (mTOR) pathway for tumor growth, and sirolimus, a mTOR inhibitor, has shown to result in a short-term response. We now report an extended treatment response with sirolimus in a pediatric patient with an EBV-SMT.

Hepatic tumors are rare childhood neoplasms with uncertain etiology. We report the cooccurrence of hepatic tumors in 2 boys with fragile X syndrome, one with hepatoblastoma and another with desmoplastic nested spindle cell tumor of liver. The pathogenesis of fragile X syndrome involves silencing of the fragile X mental retardation 1 gene and consequent loss of FMR1 protein. We speculate regarding molecular pathways that might explain the cooccurrence of the 2 conditions. Further examination of a possible functional link between hepatic neoplasia and loss of FMRP is warranted.

Hereditary neuroblastoma in a pair of identical twins is reported. In one of the twins the tumor occurred in fetal life and caused death of the twin before birth; in the other one, the tumor occurred at two months of life as IV-S neuroblastoma in the liver with primary tumor undetected. The unusual clinical course of the latter case is described: twice there was tumor regression, but in spite of treatment, fatal uncontrolled growth occurred. The family history, characterized by neoplasms of dissimilar cell types but without additional cases of neuroblastoma, is also detailed.
