A retrospective study of temporomandibular joint ankylosis secondary to surgical treatment of mandibular condylar fractures.

We investigated the incidence of ankylosis of the temporomandibular joint (TMJ) after open operations for fractures of the mandibular condyl … ve ankylosis (0/51). Among the 16 patients with postoperative ankylosis, 13 had associated anterior mandibular fractures. Long-screw (bicortical screw) fixation of fractures of the condylar head seemed to be associated with a lower incidence of postoperative ankylosis than fixation by miniplate and wire or removal of the fractured fragment. The articular discs were da …

Pathological (late) fractures of the mandibular angle after lower third molar removal: a case series.

INTRODUCTION: Pathological (late) fracture of the mandibular angle after third molar surgery is very rare (0.005% of third molar removals). There are 94 cases reported in the literature; cases associated with osseous pathologies such as osteomyelitis or any local and systemic diseases that may compromise mandibular bone strength have not been included. We describe three new cases of pathological (late) fracture of the mandibular angle after third molar surgery. CASE PRESENTATIONS: The first patient was a 27-year-old Caucasian man who had undergone surgical removal of a 3.8, mesioangular varie

[Comparative efficacies of non-surgical treatment for pediatric and adult sagittal fractures of mandibular condyle].

OBJECTIVE: To compare the efficacies of non-surgical treatment for pediatric and adult sagittal fractures of mandibular condyle. METHODS: From January 1985 to December 2010, our department treated 67 cases (82 sides) sagittal fractures of mandibular condyle with occlusal splint at a height of 1-2 mm in molar region for six months or other non-surgical treatment. Their clinical and radiological remodeling of affected condyles after treatment were reviewed at six mouths, one year and each year after treatment. They were divided into 2 groups according to age. There were 28 patients (34 sides) a

Can an arch bar replace a second lag screw in management of anterior mandibular fractures?

PURPOSE: To evaluate the efficacy of using a single lag screw combined with an arch bar in the management of anterior mandibular fractures and to compare this method with the traditional application of 2 lag screws. MATERIALS AND METHODS: We designed and implemented a randomized clinical trial and enrolled a sample of patients with anterior mandibular fractures. Twenty adult male patients were randomly divided into 2 equal groups according to the number of lag screws used for fracture fixation after securing the occlusion with intermaxillary fixation. In group A, the fractures were treated us…

Mandibular fractures in British military personnel secondary to blast trauma sustained in Iraq and Afghanistan.

Blast trauma is the primary cause of maxillofacial injury sustained by British service personnel on deployment, and the mandible is the maxillofacial structure most likely to be injured in combat, but there are few reports about the effect of blast trauma on it. The Joint Theatre Trauma Registry identified all mandibular fractures sustained by British servicemen secondary to blast injury between 1 January 2004 and 30 September 2009. These were matched to corresponding hospital notes from the Royal Centre for Defence Medicine (RCDM) for those evacuated servicemen and autopsy records for those w…

Fate of developing tooth buds located in relation to mandibular fractures in three infancy cases.

The fate of developing tooth buds located in relation to mandibular fractures was investigated in three infancy cases. Three infants, 2 girls and a boy, aged from 1 year and 5-months old to 2 years and 6-months old, were treated for dislocated mandibular fracture in the symphyseal region by manual reduction and fixation with a thermoforming splint and circumferential wiring under general anesthesia. Fracture healing was uneventful in all cases. A few years later, no obvious deformity of the jaw or malocclusion was observed; however, malformation of the crown was found in one of the permanent t…

Masticatory motion after surgical or nonsurgical treatment for unilateral fractures of the mandibular condylar process.

PURPOSE: We sought to compare mandibular motion during mastication in patients treated in either an open or a closed fashion for unilateral fractures of the mandibular condylar process. PATIENTS AND METHODS: Eighty-one male patients with unilateral condylar process fractures were treated either with (n = 37) or without (n = 44) surgical reduction and rigid fixation of their condylar process fractures. At 6 weeks, 6 months, 1 year, and 2 years after treatment, the subjects' chewing cycles were recorded using a magnetic sensor array (Sirognathograph; Siemens Corp, Bensheim, Germany) while chewi

Miniplate fixation for repair of mandibular and maxillary fractures in 15 dogs and 3 cats.

Maxillofacial miniplates and screws were used for skeletal fixation in 15 dogs and 3 cats that sustained a variety of mandibular and maxillary fractures. These implants were used as neutralization or buttress fixation in 11 caudal (junction of the ramus with the mandibular body) and 2 rostral mandibular fractures, 4 maxillary fractures, and 2 zygomatic arch fractures. All but one of the fractures healed with appropriate occlusion and excellent function. In one case of a rostral mandibular fracture, soft tissue dehiscence occurred accompanied by a loss of the fixation and subsequent distraction…

Conservative management of fractures of the mandibular condyloid process in three cats and one dog.

Three cats and 1 dog with fracture of the mandibular condyloid process and concomitant fracture(s) of the rostral portion of the mandible were treated successfully by wire fixation of the rostral mandibular fracture(s) and by conservative management of the mandibular condylar fracture. All animals regained satisfactory dental occlusion and normal, pain-free motion of the jaw. Fracture of the mandibular condyloid process is an infrequently diagnosed injury that usually occurs with other mandibular fractures. If satisfactory dental occlusion can be achieved by surgical stabilization of the nonco…

Distraction osteogenesis in a juvenile rooster (Gallus gallus domesticus) with mandibular fracture malunion.

Treatment of avian malocclusion, or "scissor beak," is well-described in the literature, but reports have focused on early intervention of acute traumatic and congenital cases, and surgical correction of chronic mandibular malunion are rare. This report describes the diagnostic investigation, surgical correction, and long-term follow up of a 3-month-old Ameraucana rooster (Gallus gallus domesticus) with chronic beak malocclusion. Computed tomography was used to confirm the unilateral mandibular fracture malunion and guide surgical planning, which involved creation of a custom osteogenic distra

Staged reconstruction of panfacial trauma involving a comminuted mandibular fracture: a case report.

Comminuted mandibular fractures (CMFs) associated with panfacial trauma are challenging because restoration of skeletal continuity does not necessarily re-establish functional occlusion or facial form. We report a staged multidisciplinary rehabilitation in a 34-year-old man who sustained severe craniofacial injuries in an industrial explosion, including a CMF, multiple midfacial fractures, and extensive anterior maxillary destruction. Initial treatment consisted of hemorrhage control, top-down open reduction and internal fixation, and simultaneous orbital floor reconstruction. Because severe m…

Blood From Both Ears: A Hidden Mandibular Condyle Fracture Revealed by Otorrhagia.

Otorrhagia following mandibular trauma may appear straightforward; however, underlying external auditory canal (EAC) fractures and associated condylar injuries can be easily overlooked without targeted evaluation. We present the case of an 88-year-old man who developed bilateral ear bleeding and hearing loss after falling and striking his chin. Computed tomography revealed bilateral mandibular condyle fractures with associated fractures of the anterior walls of both EACs. The canals were filled with hematoma, and hearing loss was presumed to result from mechanical obstruction. Otoscopic examin

Revision surgery for malocclusion after open reduction of mandibular fracture: a case report.

Insufficient treatment of mandibular fractures can result in malocclusion, often necessitating revision surgery. Therefore, appropriate surgical treatment by oral and maxillofacial surgeons with adequate consideration of the occlusion is crucial. Moreover, in patients with bone defects between the fractured ends, bone grafting may be required. A 48-year-old man was referred to our department with malocclusion after initial surgery for mandibular fracture performed by plastic surgeons at another hospital. We performed revision surgery with bone grafting for a bone defect, and favorable postoper…
