Mucus retention cyst of the maxillary sinus: the endoscopic approach.

OBJECTIVE: To present our experience of endoscopic surgery for symptomatic mucus retention cyst of the maxillary sinus. DESIGN: Retrospective study. SETTING: Teaching hospital, Israel. PATIENTS: 60 patients with 65 symptomatic cysts of the maxillary sinus who were operated on endoscopically. Only patients with large cysts that filled at least 50% of the sinus space were included. INTERVENTION: A rigid nasal endoscope was used in all cases; most of the cysts were removed through the natural sinus ostium. RESULTS: Cysts recurred in only two patients during the first postoperative year. Ther…

Lupus pernio with involvement of nasal cavity and maxillary sinus.

Histological demonstration of sarcoidosis affecting the paranasal sinus is rare in the clinical practice. In this paper, we present a case with a diagnosis of lupus pernio with extensive involvement of the nasal cavity and the right maxillary sinus. Long before being diagnosed as having lupus pernio, the patient had had a long history of nasal obstruction and had been treated surgically without success on 3 occasions. The literature on the topic is reviewed.

The radiological appearance of the post-Caldwell-Luc maxillary sinus.

A review of the radiographic findings in 19 patients who had undergone unilateral or bilateral Caldwell-Luc procedures for chronic maxillary sinusitis revealed improvement in the radiographic appearance of the sinuses in 13 and lack of improvement, progression or recurrence of disease in six. Eight of the patients had changes which could be attributed directly to surgery. These included fibro-osseous proliferation in five, antral contraction in four and compartmentalisation in two. Follow-up studies, including pluridirectional tomography and computed tomography, can reliably distinguish post-o…

Computed tomographic features in muco-pyoceles of the maxillary sinus.

Radiological images of muco-pyoceles of the maxillary sinus often simulate those of malignancy. This presents a diagnostic dilemma for rhinologists. The authors studied the computed tomographic findings of 13 cases of muco-pyoceles of the maxillary sinus by obtaining two different images, one of the soft tissues and the other of the bony structures. The bony sinus walls are distended and thinned in many cases by long-standing intramural pressure. The most reliable finding in the diagnosis of muco-pyoceles is the smooth clear-cut margins of bone erosions occurring in the sinus walls.

Zygomycosis of the maxillary sinus and palate caused by Basidiobolus haptosporus.

Basidiobolus haptosporus is known to cause subcutaneous zygomycosis in tropical Africa and Asia. We cared for a 49-year-old hyperglycemic, asplenic man who had never traveled outside the United States and who was seen initially for a painless palatal ulcer with cutaneous hypesthesia of the right cheek and upper lip. An invasive process involved the right middle nasal turbinate, maxillary antrum, maxillary division of the trigeminal nerve, and bony palate. Histological examination of biopsy tissue showed necrotizing granulomata with broad, nonseptate hyphae. Basidiobolus haptosporus was culture

Evaluation of the effect of direct sinus lift surgery on maxillary sinus volume by Mimics software.

INTRODUCTION: Sinus lift surgery allows sufficient volume of bone to be created in the posterior part of the maxilla. The aim of this study was to investigate the changes in maxillary sinus volume after a sinus lift and the rate of increase in ridge height at the site of the graft. METHODS: Eleven patients were chosen for sinus lift from among those who were referred to the radiology department for implant placement in the posterior region of the maxilla and whose bone height at the posterior of the maxilla was less than 4 mm on the cone-beam computed tomography (CBCT) image. The sinus volume…

Combining sinus plain film and sinus ultrasound as a screening tool for maxillary fungal sinusitis.

BACKGROUND: Early identification of fungal sinusitis remains a challenge. Previously, we observed a high false negative rate of using A-mode ultrasound to diagnose maxillary fungal sinusitis. This study aims to assess the accuracy of the diagnosis of fungal maxillary sinusitis using sinus plain film and ultrasound. METHODS: The screening criteria is defined as the combination of a positive sinus plain film and a false negative sinus ultrasound. We retrospectively reviewed preoperative imaging of patients with fungal sinusitis and unilateral bacterial sinusitis of the maxillary sinus undergoin…

Influence of anatomical factors related to maxillary sinus on outcomes of transcrestal sinus floor elevation.

BACKGROUND/PURPOSE: Graft bone maturation and remodeling in the maxillary sinus require adequate angiogenesis and osteoprogenitor cells migration from the surrounding bony walls and Schneiderian membrane. This study aimed to evaluate the influence of anatomical factors related to maxillary sinus on the outcomes of transcrestal sinus floor elevation using cone-beam computed tomography (CBCT). MATERIALS AND METHODS: Forty-five patients (63 elevated sites) were included in this study. CBCT were obtained preoperatively, immediately and 6 months after surgery. The anatomical features of residual b…

CBCT Study of Morphologic Variations of Maxillary Sinus Septa in Relevance to Sinus Augmentation Procedures.

BACKGROUND: Augmentation of the maxillary sinus increases the lost height of ridge in the posterior maxilla. Bony convolutions from the floor of the sinus called septa may be encountered during the procedure and significantly impact the outcome of the procedure. MATERIALS AND METHODS: For this retrospective study, 120 cone-beam computed tomography (CBCT) scans were collected from the archives of the department of oral radiology in the time period between January 2016 and August 2018. Planmeca Romexis® software version 3.2.7 was used to check for the incidence, and then among the septa, plane …

Fungal ball of the maxillary sinus and the risk of persistent sinus dysfunction after simple antrostomy.

BACKGROUND: Maxillary sinus fungal ball is a common cause of unilateral maxillary sinusitis. Fungal balls or mycetomas are primarily treated with surgery to remove the fungus. However, this assumes the pre-fungal ball sinus cavity was normal and post-surgery patients may suffer from mucostasis in the sinus cavity with persistent symptoms. It is proposed that fungal balls are potentially a feature of impaired mucus clearance as they are a pathology in their own right. METHODS: A case series of consecutive patients undergoing antrostomy for maxillary sinus fungal ball was performed. Patient fac…

Evaluation of the survival of implant placement simultaneously with sinus augmentation: relationship in maxillary sinus pathologies.

OBJECTIVES: This retrospective study aimed to evaluate the effects of existing maxillary sinus pathologies on the survival rates of dental implants placed simultaneously with sinus augmentation. METHODS: Cone-beam computed tomography images of 88 patients (34 females, 54 males), who underwent sinus augmentation, were retrospectively analyzed and the patients were divided into two groups: with (pathology group) and without pathology (control group). All maxillary sinus pathologies were recorded and categorized into mucosal thickening ( ≥ 3 mm), antral pseudocyst (polypoid mucosal thickening), …
