A large measles epidemic occurred in 2002 in Campania, a region of southern Italy with inadequate vaccination coverage. We evaluated the burden of the outbreak in children <15 years of age using different data sources. The measles standardized incidence rate was 5,757/100,000, corresponding to 63,368 estimated cases (95% CI: 59,544--67,373). Measles virus strains were identified as belonging to the D7 genotype. A measles vaccination coverage of between 50 and 60% has been achieved in Kinshasa, Zaire, from 1980-1985. The live attenuated measles vaccine is extremely effective in preventing measles and induces mucosal immunity in the respiratory tract; however, the mechanism is not known. We show that live attenuated measles virus (LAMV) RNA is frequently detected in the respiratory tract 7-21 days after subcutaneous measles-mumps-rubella (MMR) vaccination in healthy children (n = 5/20) and macaques (n = 6/8). A case of Friedreich's ataxia of the spine with intranuclear bodies in the cerebrum and stem was found in a 43-years old man. The disease is possibly the result of measles contracted when the patient was seven. This is the first observation of this type known to us; Friedreich's spinal ataxia has until now been believed to exist only hereditary ("hereditary spinal ataxia").
