Active Duty Personnel With ST Elevation Myocardial Infarctions Are Deployment Ineligible Despite Receiving Standard Management.

INTRODUCTION: ST elevation myocardial infarction (STEMI) is a high acuity diagnosis that requires prompt recognition and developed system responses to reduce morbidity and mortality. There is a paucity of literature describing active duty (AD) military personnel with STEMI syndromes at military treatment facilities (MTFs). This study aims to describe AD military members with STEMI diagnoses, military treatment facility management, and subsequent military dispositions observed. MATERIALS AND METHODS: We performed a single-center, retrospective review of all STEMI diagnoses at San Antonio Milit

Women Veterans, a Population at Risk for Fibromyalgia: The Associations Between Fibromyalgia, Symptoms, and Quality of Life.

BACKGROUND: The Institute of Federal Health Care recently published an executive summary from a round table discussion indicating that active duty and retired female military personnel are at high risk for adverse health outcomes unique to military service including complications related to post-traumatic stress disorder (PTSD), unreported sexual trauma, and musculoskeletal problems. In 2008, the Institute of Medicine began to review, evaluate, and summarize the literature on health outcomes in Gulf War-deployed and found sufficient evidence of a causal relationship with PTSD and suggestive ev

The Effect of Concussion Mechanism of Injury on Sleep Problems in Active Duty Service Members Following Deployment.

INTRODUCTION: Sleep disruption is pervasive in the military and is generally exacerbated during deployment, partially due to increases in operational tempo and exposure to stressors and/or trauma. In particular, sleep disruption is a commonly reported symptom following deployment-related traumatic brain injury (TBI), though less is known about the prevalence of sleep disturbance as a function of whether the TBI was induced by high-level blast (HLB) or direct impact to the head. TBI assessment, treatment, and prognosis are further complicated by comorbidity with posttraumatic stress disorder (P

Iraq/Afghanistan war lung injury reflects burn pits exposure.

This descriptive case series retrospectively reviewed medical records from thirty-one previously healthy, war-fighting veterans who self-reported exposure to airborne hazards while serving in Iraq and Afghanistan between 2003 and the present. They all noted new-onset dyspnea, which began during deployment or as a military contractor. Twenty-one subjects underwent non-invasive pulmonary diagnostic testing, including maximum expiratory pressure (MEP) and impulse oscillometry (IOS). In addition, five soldiers received a lung biopsy; tissue results were compared to a previously published sample fr

AI is already in the theatre of war just not on the side of the patient: clinical chatbots, telemedicine, and an agenda for "AI for medical humanity".

Artificial intelligence (AI) is already embedded in modern warfare, mainly through surveillance, targeting, and military decision-making. However, its potential to support clinicians and patients in conflict settings remains underdeveloped. Frontline medical teams must make time-critical, high-stakes decisions with scarce diagnostics, disrupted referrals, insecure communications, changing injury patterns, and irregular access to specialist expertise. Humanitarian medicine needs a proactive clinician-facing AI agenda that strengthens clinical judgment, not replaces it. We propose a two-stage mo

Labeling of deployment sexual harassment experiences among male and female veterans.

OBJECTIVE: Victims' conceptualizations of traumatic experiences can impact screening responses and decisions to utilize health care. Despite experiencing events constituting sexual harassment, many victims do not label their experiences as such. In the military, specific situational factors (e.g., occurred on-duty, higher ranking harasser) and victim appraisals of experiences (e.g., offensive, threatening) have been associated with increased labeling of sexual harassment. However, no research has examined sexual harassment labeling among those deployed in recent Afghanistan and Iraq conflicts,

Otologic assessment of blast and nonblast injury in returning Middle East-deployed service members.

OBJECTIVES/HYPOTHESIS: To determine if tympanic membrane perforation offers any protection from inner ear damage and determine the incidence and pattern of otologic blast injury in military personnel returning from deployment. STUDY DESIGN: Retrospective analysis of US service members injured in Operation Iraqi Freedom and Operation Enduring Freedom from October 2006 to October 2007. METHODS: One-hundred ten blast-injured patients were compared to 54 nonblast-injured patients returning from deployment. Hostility is related to clusters of T-cell cytokines and chemokines in healthy men.

Hostility is a risk factor for adverse health outcomes as diverse as cardiovascular disease and post-traumatic stress disorder (PTSD). Cytokines have been suggested to mediate this relationship. We investigated whether in healthy men a relation existed between hostility and T-cell mitogen-induced cytokines and chemokines. Male Dutch military personnel (n=304) were included before deployment. Eleven cytokines and chemokines were measured in supernatants of T-cell mitogen-stimulated whole blood cultures by multiplex immunoassay. Factor analysis was used to identify clusters of cytokines and chem

Heart rate variability characteristics in a large group of active-duty marines and relationship to posttraumatic stress.

OBJECTIVE: Heart rate variability (HRV), thought to reflect autonomic nervous system function, is lowered under conditions such as posttraumatic stress disorder (PTSD). The potential confounding effects of traumatic brain injury (TBI) and depression in the relationship between HRV and PTSD have not been elucidated in a large cohort of military service members. Here we describe HRV associations with stress disorder symptoms in a large study of Marines while accounting for well-known covariates of HRV and PTSD including TBI and depression. METHODS: Four battalions of male active-duty Marines (n

Pre-exposure anti-Campylobacter jejuni immunoglobulin a levels associated with reduced risk of Campylobacter diarrhea in adults traveling to Thailand.

Diarrhea history questionnaires were administered to 369 U.S. military volunteers before and after deployment to Thailand. Additionally, blood samples obtained from a subset of 221 volunteers 1-3 weeks previously and 3-4 weeks after their deployment were tested by enzyme-linked immunosorbent assay for immunoglobulin A to Campylobacter jejuni. Stool samples from personnel (including volunteers) contracting diarrhea in Thailand were cultured for enteric pathogens. Overall, 35.2% (130 of 369) of questionnaire respondents reported one or more diarrhea episodes during their trip. Volunteers with pr

Predictors of Respiratory Oscillometry Measurements in a Healthy Population.

BACKGROUND: The factors that determine variability in impulse oscillometry (IOS) are not well defined. METHODS: We used IOS data from a well-screened population of active-duty service members (ADSMs) cleared for deployment (STAMPEDE II cohort) to identify variables independently associated with IOS measurements. We constructed our own predictive models and compared them with existing reference equations when applied to postdeployment STAMPEDE II subjects and two additional ADSM IOS datasets. RESULTS: There were 775 STAMPEDE II subjects without a history of respiratory symptoms, tobacco expos

Sleep disturbance is common among servicemembers and veterans of Operations Enduring Freedom and Iraqi Freedom.

Sleep routines that develop as an adaptation or reaction to deployment can persist upon return stateside. Sleep problems intensify and are intensified by psychiatric distress. This research presents the findings of a comprehensive survey of sleep impairment in relation to demographic data, military history, combat exposure, and mental illness symptoms among a general sample of 375 servicemembers and veterans of Operations Enduring Freedom and Iraqi Freedom (OEF/OIF) at a wide range of times postdeployment. Sleep impairment was assessed with the Pittsburgh Sleep Quality Index (PSQI) and the Add

Drug use over time among never-deployed US Army Reserve and National Guard soldiers: The longitudinal effects of non-deployment emotions and sex.

Some US military service members who have never been deployed experience negative emotions related to never having been deployed, and some work shows these non-deployment emotions (NDE) are cross-sectionally associated with hazardous drinking for male, but not female, US Army Reserve/National Guard (USAR/NG) soldiers. However, it is not known if these effects extend to drug use or persist longitudinally, which is the focus of the current study. We conducted a longitudinal residual change analysis of a subset of data (N = 182 never-deployed soldiers) from Operation: SAFETY, an ongoing survey-ba

Grapheme-Color Synesthesia is Associated with PTSD Among Deployed Veterans: Confirmation of Previous Findings and Need for Additional Research.

Post-traumatic stress disorder (PTSD) is related to alteration in neuropsychological functioning, including visual and other cognitive processes. Grapheme-color synesthesia is a phenomenon in which a letter or number elicits response of a concurrent image or color perception. Since we earlier reported an association between grapheme-color synesthesia and PTSD, our objective in the current study was to validate this association among a new study group and assess risk factors. For this, we surveyed 1,730 military veterans who have been outpatients in the Geisinger Clinic, a multi-hospital system

Property prediction of energetic materials based on directional-aware graph attention network.

Energetic materials have widespread applications in military, aerospace, and other high-stakes domains. Accurate prediction of their explosive properties is critical for both material development and safe deployment. This paper proposes a Directional-Aware Graph Attention Network (DAGAN) model, which constructs node and edge representations incorporating fine-grained features such as atomic type distributions and chemical bond topological environments. A directional-aware graph attention architecture is designed and integrated with an adaptive training algorithm to enable deep mining of intrin

The effect of combat exposure on sexually transmitted diseases.

Traumatic exposures can affect beliefs and behaviors related to the spread of sexually transmitted diseases (STDs), a persistent public health problem. I leverage a natural experiment created by variation in US military deployment location assignments to estimate how combat exposure changes a surviving deployed male veteran's probability of acquiring a sexually transmitted disease. I analyze longitudinal data from 1994 to 2008 on 485 deployed veterans with information theoretic methods to reduce the sensitivity of estimates to small samples, an infrequently observed outcome, and highly correla…

Complex regional pain syndrome in an 8-year-old female with emotional stress during deployment of a family member.

INTRODUCTION: This pediatric case of complex regional pain syndrome (CRPS) illustrates the need to expand the typical age range and raise awareness of the psychological impact military deployment may have on its development. CASE: An emotional 8-year-old female, with a recently deployed father, presented with left foot pain. Over an 11-week-period, she developed symptoms, signs, and radiologic findings consistent with CRPS. DISCUSSION: Pediatric CRPS is characterized by ecchymosis, edema, allodynia, mottling, and abnormal hair growth in the region of pain after minor trauma. It occurs predom…

Evaluating the Effect of ADHD on Occupational Outcomes in the U.S. Marine Corps.

OBJECTIVE: Attention-deficit/hyperactivity disorder (ADHD) can potentially disqualify individuals for military service. However, some past research suggests the performance of individuals with ADHD in military service may be comparable to individuals without ADHD. The objective of the present research is to evaluate the effect of ADHD on occupational outcomes in the first enlistment term. We hypothesized that ADHD would be associated with weak or small performance decrements. METHOD: Study 1 (N = 51,845) linked Recruit Assessment Program (RAP) survey data collected from U.S. Marine recruits b

Vaccine effectiveness during an influenza B outbreak in a deployed military setting.

INTRODUCTION: Military deployments remain high-risk for acquisition and transmission of influenza-like illnesses. Outbreaks are associated with significant morbidity and reduced operational effectiveness. The impact of seasonal influenza vaccination uptake on vaccine effectiveness (VE) in this setting remains unclear. METHODS: VE was explored in a deployed military population during a large population transition period when a mostly unvaccinated relief cohort was introduced into a pre-existing partially vaccinated cohort in the context of an influenza B outbreak.
