Quality of Work Life Factors Among Medical Residents: A Comparative Study of Public and Private Hospitals in Mexico.

Background and objective Medical residency constitutes a crucial stage in the professional training of physicians, characterized by high clinical and academic demands that directly impact their quality of work life (QoWL). In addition, it is necessary to consider the differences between public and private hospitals, since their organizational structures, availability of resources, and institutional cultures vary significantly, although evidence on how these differences impact QoWL remains limited and inconclusive. The objective of the study was to comprehensively evaluate the factors of the ac

An ethnographic study of mobilization, basic mobility, physical activity, and exercise during acute hospitalization in patients following hip fracture surgery.

INTRODUCTION: Existing evidence highlight the need for a standardized approach in treating patients hospitalized following a hip fracture surgery. Clinical guidelines highlight rapid recovery of basic mobility post-surgery and the importance of frequent mobilization, physical activity, and exercise in rehabilitation. However, findings from the first HIP-ME-UP study show increasing physical inactivity among these patients, indicating issues with guideline implementation in clinical practice. Therefore, this study investigated clinical practice in relation to mobilization, basic mobility, physic

A Tractography Analysis Framework Using Diffusion Maps to Study Thalamic Connectivity in Traumatic Brain Injury.

Traumatic brain injury (TBI) disrupts thalamocortical connectivity, contributing to cognitive impairment and post-traumatic epilepsy (PTE). This study presents a novel tractography-based framework that leverages diffusion maps to capture microstructural and organizational changes in thalamic white matter pathways. By analyzing individual streamline characteristics, we identified significant associations between diffusion map embeddings and functional outcomes (GOSE scores), highlighting potential biomarkers for injury severity and recovery trajectories. Our findings suggest that fine-grained g

Hysterical personality and family: a clinical case.

Within the framework of family typology, the therapist can organize and orient his own observations, by confronting the characteristics of a particular family system, with the models of a "shared reality". Structural and organizational analysis of the family system, according to a general typology, can also be useful when the therapist has to deal with a defined and a not specific symptom or disease, and when a differential diagnosis is necessary for its pragmatic effects. This is often the case of hysteria, which according to many authors can show today various, confused shapes, as depression

Enhancing the Professional Work Environment Through Nursing Engagement in an Academic Medical Center.

BACKGROUND: Shared decision-making and professional governance have long been defining features of American Nurses Credentialing Center (ANCC) Magnet-designated organizations, supporting high-quality outcomes, professional autonomy, and nurse engagement. Within our multi-Magnet-designated academic medical center, part of a larger academic health system, a shared governance structure had been in place for many years. However, to respond to the evolving demands of the academic medical center, the increasing complexity of health care delivery, and the need for stronger alignment between frontline

Interpersonal and Trauma-Related Guilt moderate the relationship between intensity of combat experiences and suicidality.

Suicide rates in military-affiliated communities remain elevated since the dawn of the Global War on Terror, despite substantial efforts by clinicians and researchers. While some risk factors have been identified, mixed results need to be clarified. The current study builds on previous research by testing a structural equation model of suicide risk associated with combat experiences that by incorporates risk factors with the most empirical support (combat experiences, guilt, PTSD, depression, and the Interpersonal Psychological Theory of Suicide [IPTS] factors of Perceived Burdensomeness, Thwa

Outcomes management practices in tiered school-based speech-language therapy: A Canadian example.

BACKGROUND: Measuring, assessing and managing outcomes in school practice environments is difficult due to the complex nature of school communities as well as the recent shift in service-delivery models towards tiered approaches. In tiered approaches, multiple levels of service are offered to better match students' needs. Each level of service may require different outcomes and management techniques. Research to date on outcomes has focused on measuring outcomes in medical settings, leaving a substantive gap in the literature regarding practice in schools. AIMS: The first aim was to explore h

A policy framework for public health uses of electronic health data.

Successful implementation of a program of active safety surveillance of drugs and medical products depends on public trust. This article summarizes how the initial pilot phase of the FDA's Sentinel Initiative, Mini-Sentinel, is being conducted in compliance with applicable federal and state laws. The article also sets forth the attributes of Mini-Sentinel that enhance privacy and public trust, including the use of a distributed data system (where identifiable information remains at the data partners) and the adoption by participants of additional mandatory policies and procedures implementing

Combat damage control surgery.

BACKGROUND: Although the use of damage control surgery for blunt and penetrating injury has been widely reported and defined, the use of damage control surgery on the battlefield (combat damage control surgery) has not been well detailed. DISCUSSION: Damage control surgery is now well established as the standard of care for severely injured civilian patients requiring emergent laparotomy in the United States. The civilian damage control paradigm is based on a "damage control trilogy." This trilogy comprises an abbreviated operation, intensive care unit resuscitation, and a return to the opera

A three-dimensional multiscale finite element model of bone coupling mineralized collagen fibril networks and lamellae.

Despite evidence of contribution of mineralized collagen fibrils (MCF) to both the microscale elastic and fracture response of bone, the extent of influence of MCF orientation and material property variation on the lamellar scale mechanical properties is still not well quantified. To this end, in this study, we developed a three-dimensional multiscale finite element model that linked submicroscale models of MCF networks to microscale models of several lamellae. The developed models evaluated the individual and relative influence of MCF orientation as well as material property variation due to

Graphical modeling for gene set analysis: A critical appraisal.

Current demand for understanding the behavior of groups of related genes, combined with the greater availability of data, has led to an increased focus on statistical methods in gene set analysis. In this paper, we aim to perform a critical appraisal of the methodology based on graphical models developed in Massa et al. (2010) that uses pathway signaling networks as a starting point to develop statistically sound procedures for gene set analysis. We pay attention to the potential of the methodology with respect to the organizational aspects of dealing with such complex but highly informative s
