[Late ventricular potentials. Clinical applications and relation to severe ventricular arrhythmias].

The recording of late ventricular potentials with high amplification cardiography (HAC) permits to identify patients presenting a risk of sudden death and ventricular tachycardia, especially in the later stage of myocardial infarction. Few authors have studied the prevalence of these potentials in other heart diseases presenting a risk of sudden death. Most series in the literature are too small to specify variations in the prevalence of these potentials according to the severity of the coronary disease. For this purpose, 835 patients including 535 coronary patients were evaluated with HAC com

Twenty-four-hour electrocardiographic study in the active very elderly.

Cardiac rhythm was studied by means of 24-hour ambulatory ECG in 18 active outpatients, mean age 92.3 +/- 2.3 years. The main characteristics observed were the following: a broad dynamic heart rate range; relatively rare atrial fibrillation (5.5%); incidence of simple atrial and ventricular premature beats, complex atrial premature beats or supraventricular tachycardia in all subjects, while ventricular tachycardia and R-on-T phenomenon were present in only one subject. The 'arrhythmic normality' parameters in the very elderly probably need to be revised, and only severe ventricular arrhythmia

Monitoring of anti-inflammatory drugs.

Advances in analytic technology have allowed clinical laboratories to accurately measure plasma levels of NSAIDs. With the exception of salicylate, however, there is no relationship, or only a weak one, between plasma NSAID levels and clinical responses. Therefore, therapeutic drug monitoring of NSAIDs other than salicylate is of little value to clinicians in their management of patients with rheumatoid arthritis. On occasion, however, it may be helpful to monitor specific patients suspected of noncompliance by measuring plasma drug levels. Routine therapeutic drug monitoring of these NSAIDs m

Programming rapid generalization of correct articulation through self-monitoring procedures.

The purpose of this study was to investigate the use of a self-monitoring activity in the clinical and natural environment as a method of promoting rapid generalization of a target speech sound to beyond-treatment conditions. The subjects were 13 children who lateralized /s/ and /z/, substituted /theta/ for /s/ and /delta/ for /z/, and/or substituted /w/ for /r/. The children attended public school speech therapy, individually or in small groups, twice weekly for 15- to 20-min sessions throughout the entire study. The investigation was conducted in the context of a multiple baseline research d

Blood pressure monitoring for quality assessment of antihypertensive therapy.

Despite many new therapeutic approaches to the treatment of hypertension and new diagnostic methods, the measurement of the most basic parameter, the blood pressure itself, has not changed over many years. The most commonly used method records the blood pressure on a single occasion and it is then assumed that this single reading is representative of the blood pressure of that patient at all times. This paper presents evidence, based on published studies, showing that the single "office" blood pressure reading gives little information about the blood pressure during daily activities. A study i

Assessment of patients with office hypertension by 24-hour noninvasive ambulatory blood pressure monitoring.

To assess the discrepancy between casual (office) and home blood pressure readings in patients performing home blood pressure monitoring, we analyzed office, home, and 24-hour ambulatory blood pressure and heart rates in 19 patients in a prospective four-week study. After the month of study, the average difference between mean office and manual home blood pressures in this office hypertensive group was 30 +/- 17/20 +/- 6 mm Hg. The blood pressures taken in the office were substantially greater than the 24-hour average blood pressures and ambulatory blood pressures during work or while at home

Hypovolemic shock.

Hypovolemic shock is the most common form of shock seen clinically and has attracted the greatest laboratory interest. It is caused by a sudden decrease in the intravascular blood volume relative to the vascular capacity, to the extent that effective tissue perfusion cannot be maintained. The authors of this article discuss the pathophysiology of hypovolemic shock, the assessment of the patient in shock, the immunologic consequences of shock, impairment of cardiac function in hypovolemic shock, and the management of hypovolemic shock.

Day-to-day pneumogram variability.

To determine day-to-day variability in respiratory pattern, we obtained 188 comparisons of pneumograms performed during two successive 24-h intervals. The respiratory pattern values calculated were total duration of brief apnea (apnea density), periodic breathing episodes, longest apnea, number of apneas greater than 11 s, and number of apneas greater than 15 s. For day 1 values for apnea density, periodic breathing, and longest apnea which were within the 90th percentile for normal infants at age 1 month, day 2 values fell outside that range in 4.3, 3.5, and 18% of comparisons, respectively.

Gastroesophageal reflux. Simple measures often suffice.

Regurgitation of the gastric contents into the esophagus is common and often unnoticed. When symptoms such as heartburn, a sour or bitter taste in the mouth, or even chest pain mimicking angina pectoris or myocardial ischemia prompt a patient to seek help, the factor or factors responsible for reflux must be sought. The possible underlying causes are numerous, as Dr Bachman points out in this discussion of the pathophysiology, diagnosis, and treatment of gastroesophageal reflux. The desired end point of management was well stated by Seneca over 2,000 years ago as "a good-humored stomach."

The use of mixed venous blood gas determinations in traumatic shock.

A series of 10 seriously injured patients requiring resuscitation and definitive operative control of hemorrhage was studied. Simultaneous arterial and mixed venous blood gases were measured sequentially throughout the resuscitative and operative periods. Improvement of the mixed venous oxygen saturation was associated with survival. In this study, mixed venous oxygen saturations were valuable predictors of survival and were a helpful parameter to monitor during the resuscitative, operative, and immediate postoperative periods.

Pseudodysrhythmias in ambulatory ECG monitoring.

Artifacts mimicking a variety of dysrhythmias occur relatively frequently in ambulatory ECG (Holter) monitoring records. Proper interpretation is imperative if serious therapeutic errors are to be avoided. Two-channel recording systems may facilitate recognition of some, but not all, of these artifacts. Pseudodysrhythmias may mimic paroxysmal supraventricular tachycardia and atrial fibrillation, atrial dissociation, extrasystoles, and sinus pauses. There are several causes of pseudodysrhythmias. Failure to recognize these patterns may result in serious errors in patient management.
