Isolated leukemic infiltration of peripheral nervous system.

INTRODUCTION: Isolated leukemic infiltration of peripheral nerves without central nervous system involvement, or isolated neuroleukemiosis, is an extremely rare complication of leukemia. METHODS: We report the case of a patient with isolated neuroleukemiosis and review the pertinent literature. RESULTS: A man with a 12-year history of acute monoblastic leukemia presented with median mononeuropathy. Magnetic resonance imaging revealed a thickened median nerve. Nerve biopsy confirmed the diagnosis of leukemic infiltration. Clinical resolution was achieved through local radiation. Only 10 cases

Clinical, electrophysiologic, and imaging features of zoster-associated limb paresis.

INTRODUCTION: Paresis is a long-recognized complication of herpes zoster, but there has been comparatively little study of zoster-associated limb paresis (ZALP). METHODS: In this study we reviewed 49 Mayo Clinic patients with ZALP. RESULTS: The mean age of onset was 71 years, 67% were men, and the lower limb was affected in 55%. The mean weakness score was 2.0 (0 = normal strength, 4 = plegia). Most patients developed postherpetic neuralgia (PHN, 92% at 1 month and 65% at 3 months), and the average minimum duration of weakness was 193 days. ZALP was caused by radiculopathy (37%), plexopathy

Subacute proximal leg weakness after a minor traffic accident in a patient treated with anticoagulants.

One day after a minor accident, an 85-year-old man developed headache and proximal left leg weakness. He was on warfarin due to atrial fibrillation. On hospital admission, a right posterior parasagittal subdural haematoma that was visualised on CT was considered as cause of all symptoms. Although no surgical intervention was performed he stayed for 2 weeks in a neurosurgical ward. Clinical deterioration occurred within the first days as he was not able to stand on his left leg or rise up from the sitting position. Headache had ceased, but the patient reported low abdominal pain. This condition

Lyme disease: a multisystem infection that affects the nervous system.

PURPOSE OF REVIEW: This article will enable the reader to diagnose and treat nervous system Lyme disease appropriately. RECENT FINDINGS: Appropriately applied serologic testing has high positive and negative predictive values in nervous system Lyme disease. Oral antibiotics can be curative in most cases. SUMMARY: Infection with the tick-transmitted spirochete Borrelia burgdorferi causes Lyme disease, a disorder that involves the nervous system in about 15% of patients. Common manifestations include lymphocytic meningitis, cranial neuritis (particularly cranial nerve VII), painful radiculitis

Nerve conduction studies in paucibacillary and multibacillary leprosy: a comparative evaluation.

Involvement of peripheral nerves in patients with leprosy results in sensory, motor and autonomic dysfunctions along with deformities and disability. Pattern of nerve involvement is different for different forms of leprosy. In this study, we evaluated and compared the nerve conduction parameters of paucibacillary leprosy with that of multibacillary leprosy. In this study, 40 consecutive patients of leprosy (19 cases of paucibacillary and 21 cases of multibacillary leprosy) were included. Nerve conduction studies were performed according to the standard procedure described in the manual of the

[Vasculitis associated with Churg-Strauss syndrome].

A 52-year-old man was admitted to hospital with abdominal pain, erythema and mild eosinophilia. Medical history revealed fatigue and dyspnoea on exertion after returning from the Gambia a year previously, and allergic rhinitis with nasal polyps that were surgically excised. Before a cause of the abdominal symptoms was determined he developed mononeuropathy, severe eosinophilia and arthralgia in combination with positive anti-neutrophil cytoplasmic antibodies (ANCA) against myeloperoxidase, which lead to the diagnosis of 'Churg-Strauss syndrome'. Despite treatment with high-dose corticosteroids

Antihyperalgesic activity of verbascoside in two models of neuropathic pain.

OBJECTIVES: This study reports on the rapid isolation of verbascoside from Lippia citriodora H.B.K. (Verbenaceae), an inexpensive and widespread source, and the evaluation of its antihyperalgesic activity. METHODS: Isolation of verbascoside was achieved by size exclusion chromatography with Sephadex LH-20 eluting with 50% EtOH, which is proposed as a fast and efficient method of separation. KEY FINDINGS: The antihyperalgesic activity of verbascoside was tested by in-vivo assay using the paw-pressure test in two animal models of neuropathic pain: a peripheral mononeuropathy produced either by

[Cardiac lesion in the Churg-Strauss syndrome].

The Churg-Strauss syndrome is a systemic vasculitis, the manifestations of which are asthma, eosinophilia, pulmonary infiltrates, poly- and mononeuropathy, polyserositis. Along with nodular polyarteritis and nonspecific aortoarteritis, the Churg-Strauss syndrome belongs to a group of systemic vasculitis, in the clinical picture of which cardiac lesion is recognized as one of the leading visceral manifestations and may be a common cause of fatal outcomes. In the Churg-Strauss syndrome, cardiac pathology may be associated with the involvement of the endocardium, myocardium, and pericardium. The

Iatrogenic axillary neuropathy after intramuscular injection of the deltoid muscle.

A previously healthy 26-yr-old male presented for an electrodiagnostic evaluation with complaints of significant right deltoid muscle atrophy and shoulder abduction weakness after receiving an intramuscular (IM) deltoid injection of an antiemetic 4 wk earlier. Electrodiagnostic evaluation confirmed an acute axillary neuropathy. We hypothesize that direct mechanical trauma to the anterior branch of the axillary nerve resulted in axillary mononeuropathy with axonal loss, although chemically induced nerve injury cannot be excluded. Injections in and about the shoulder complex are performed routin

Lupus-like disease with anticardiolipin antibodies and lupus anticoagulant as a cause for atypical Eaton-Lambert syndrome and peripheral nerve disease: a case report.

In the practice of neurology, the type of clinical involvement suggests the site of the lesion and the mechanism beneath. Sometimes, the symptoms can be delusive, turning the diagnostic approach to a wrong path and raising the necessity of an algorithm considering the less probable entities. Dysimmunity as mechanism of neurological disease involving both the neuromuscular junction and peripheral nerves is frequently insidious and difficult to suspect on clinical basis alone. We report the case of a 67-year-old male with atypical Eaton-Lambert syndrome and mononeuropathy probably in relation wi

Ataxia and peripheral neuropathy: rare manifestations in Henoch-Schönlein purpura.

Henoch-Schönlein purpura (HSP) is a multisystemic vasculitis. Nervous system involvement is usually underestimated. Headaches, mental status changes and seizures are the most frequent neurologic symptoms. Ataxia and mononeuropathy are both very rare. We present an 11-year-old boy with HSP who suffered from ataxia during the initial presentation and peripheral neuropathy at the time of a relapse. Brainstem vasculitic involvement was shown by magnetic resonance imaging, while cranial tomography was normal. All the neurologic symptoms and signs resolved following bolus methylprednisolone administ

The sensitivity and specificity of tests for carpal tunnel syndrome vary with the comparison subjects.

The performance of a variety of common office-based clinical tests for detection of carpal tunnel syndrome (CTS) was assessed in 119 subjects with and without electrophysiological evidence of CTS. Symptoms compatible with CTS and electrophysiological tests positive for median mononeuropathy at the wrist were observed in 57 hands, symptoms compatible with CTS and normal electrophysiological test results were observed in 58 hands, and no symptoms compatible with CTS and normal electrophysiological test results were observed in 123 hands. For all the diagnostic tests studied, the proportion of su

Lyme neuroborreliosis in the rhesus monkey.

Although there are several animal models of Lyme disease, only the rhesus monkey model exhibits all of the key manifestations of the disease. After infection with Borrelia burgdorferi, rhesus monkeys develop signs of early localized, early disseminated, and chronic Lyme disease. Specific features include erythema migrans, uveitis, myocarditis, arthritis, and disease of the peripheral and central nervous system. One of the unique features of the rhesus monkey model is the development of Lyme neuroborreliosis. Peripheral nervous system (PNS) involvement is usually in the form of a mononeuropathy

Peripheral neuropathy associated with primary Sjögren's syndrome.

Clinical and electrophysiological signs of peripheral neuropathy were found in 10 of 46 patients (21.7%) with primary Sjögren's syndrome, symmetric polyneuropathy in seven (mainly sensory in five, mainly autonomic in two), sensory neuronopathy in two patients, and mononeuropathy multiplex in one patient. Peripheral neuropathy was the presenting manifestation in five patients (10.9%). Onset of the disease after 50 years was significantly more common in the polyneuropathy group (six of seven) than in non-neuropathic patients with primary Sjögren's syndrome (14 of 36; p = 0.034). No other differe

Benign recurrent VI nerve palsy in childhood.

The case of a child with six documented episodes of benign recurrent unilateral VI nerve palsy between the ages of 2 1/2 months and 3 years is presented. Despite the recognized self-limiting course of this disorder, its possible evolution into a comitant esotropia makes close follow-up mandatory. The practical aspects of management including maintenance occlusion therapy are stressed as well as the need for prompt surgical intervention once the acquired stabismus has become stabilized. The etiology of benign VI nerve palsy of childhood may have the same immunological basis as other cases of pa

An atypical presentation of microscopic polyangiitis without renal involvement in an older patient with falls, bilateral lower limb swelling and neuropathy.

Microscopic polyangiitis (MPA) is a primary systemic small-vessel vasculitis associated with antineutrophil cytoplasmic antibodies (ANCA). The condition is uncommon among the UK population, although it increases with age. MPA manifests heterogeneously and may affect any organ, although glomerulonephritis is almost always present. This case report describes a rare presentation of MPA with isolated mononeuritis multiplex, without glomerulonephritis or other hallmark features, and reflects the varied manifestations of MPA and challenges in recognising the condition. Awareness of MPA is essential

Red flags in asymmetric axonal neuropathy revealing MPO-ANCA-associated microscopic polyangiitis: a case report.

BACKGROUND: Mononeuritis multiplex is a recognized, though sometimes overlooked, clinical feature of myeloperoxidase antineutrophil cytoplasmic antibody (MPO-ANCA)-associated vasculitis. When renal or pulmonary involvement is initially mild, the condition may mimic common neurological disorders and diagnosis can be delayed. CASE PRESENTATION: A 51-year-old woman presented with a five-month history of progressive bilateral lower-limb pain and edema, followed by three weeks of rapidly progressive asymmetric weakness affecting all four limbs. Nerve conduction studies showed severe multifocal axo
