Brain and spinal cord MRI in motor neuron disease.

Motor neuron disease causes widespread degeneration of motor neurons within both the brain and spinal cord. Brain and spinal cord MRI were performed in 11 patients with motor neuron disease, and in 17 controls. Symmetric areas of high signal within the corticospinal tracts were found in nine patients on T2 weighted spin echo (SE) or fast spin echo (FSE) images of the brain and in eight on T2 or T2* weighed images of the spinal cord. High signal within the posterior limbs of the internal capsules was also found in four controls; this finding in isolation is therefore not pathological. No contro…

F-response multiplicity in lower motor neuron disorders.

Multiple F-response following a single supramaximal distal nerve stimulation was observed in about 4% of the patients suffering from disorders of lower motor neuron. The phenomenon was observed in old poliomyelitis, entrapment neuropathies, axonopathies and demyelinating neuropathies. Thirty healthy subjects and 30 patients of upper motor neuron disorder were also studied for this purpose but none showed F-multiplicity. On repeated stimulation at sufficient intervals, some or all the components of the multiple F-response could be reproduced individually or in different combinations with simila

Formation of the cranial motor neurons in the absence of the floor plate.

The inductive signals for the differentiation of motor neurons in the spinal cord have been experimentally shown to arise from cells in the midventral region of the neural tube, often referred to as the floor plate, and from the notochord. Although the prevailing view is that a similar mechanism accounts for the differentiation of motor neurons in the brain stem, supporting experimental evidence is lacking. Here, using the formation of the trochlear nucleus in the midbrain of duck embryos as a model system, we report that the floor plate and the notochord are not necessary for the development …

Involvement of the amygdala, dentate and hippocampus in motor neuron disease.

A pathological hallmark of motor neuron disease (MND) is the presence of ubiquitinated inclusions in lower motor neurons. Recently, ubiquitin-immunoreactive inclusions have been found in neocortical and hippocampal neurons, indicating that in some cases there may be extensive extra-motor involvement. We have observed that some cases of MND with dementia (MND-D), had ubiquitinated inclusions in the amygdala. In order to study this systematically, we examined 18 cases of 'typical' MND, 4 cases of MND-D and 12 neurologically normal and abnormal controls, using antibodies against ubiquitin and a r…

[Failure of empirical treatment with ceftriaxone in motor neuron disease].

We describe two patients with lateral amyotrophic sclerosis who, after informed consent, received empirical treatment with intravenous cephtriaxone at a dose of 2 g/24 hours for three weeks, with no positive results. The pharmacokinetics of this cephalosporin is analyzed, along with the relationship between motor neuron disease, neuroborreliosis and immunoreactivity to Borrelia burgdorferi.

The first case of equine motor neuron disease in Japan.

A 9-year-old male horse showed emaciation, weakness and trembling and was euthanatized. Histopathological examinations revealed loss, swelling and chromatolysis of motor neurons throughout the spinal ventral horns, axonal degeneration of the ventral spinal roots. Eosinophilic cytoplasmic inclusions were distributed in degenerated spinal ventral neurons. Ultrastructurally, the inclusions consisted of aggregations of granular dense material and a few vesicles. They reacted positively with polyclonal antibody against ubiquitin. The present case was diagnosed as equine motor neuron disease, which …

Ethics and the clinician: the daily experience with motor neurone disease.

Ethical issues in health care are typically perceived as arising from extreme situations which do not usually confront the average clinician. However, knowingly or otherwise, clinicians working with motor neurone disease deal daily with ethical issues in the form of value judgements, the application of choice limiting principles and the language of clinician-patient interaction.

A review of dysphagia in four cases of motor neurone disease.

Dysphagia is a common and distressing problem in motor neurone disease. This paper examines some of the strategies available for managing patients with dysphagia, and illustrates these with four contrasting case histories. The factors which influence the decision making process in patients with dysphagia are also discussed.

Palliative care and taboos within motor neurone disease.

Taboos, whether held by professional carers, patients or families have the capacity to influence a whole range of choices that must be made during the course of any illness. In the case of motor neurone disease, decisions regarding if, when and how to break bad news, the place of care (home, hospital or hospice), the introduction of aids and devices, and, ultimately, choices regarding the place of death, will all be influenced by a range of taboos. If professional carers have major unresolved issues concerning their own mortality, it is unlikely that they will be able to truly stand alongside

Rapidly progressive aphasic dementia and motor neuron disease.

Articulatory and language impairment heralded rapidly progressive motor neuron disease in 7 patients aged 54 to 77 years. One patient had a family history of a similar disorder. Severe nonfluent aphasia developed in all 7 patients and 4 were anarthric within a year. Other cognitive domains were impaired, yet 2 patients lived alone until 1 month before their deaths. Four died within 2 years. Abnormalities were found on electromyography, computed tomography, magnetic resonance imaging, single-photon emission computed tomography, and electroencephalography. Neuropathological examination in 3 pati

Rising mortality from motor neurone disease: an explanation.

There is considerable debate about the increasing mortality from motor neurone disease (MND). However, examination of the relationship between increased life expectancy (through decreased general mortality) and increased mortality in both England and Wales and the United States indicates a close association between the two variables. Using a statistical model, defined sub-populations susceptible to MND can be identified in both countries. The size of such a sub-population has been estimated from the 1989 mortality data to be approximately 160,000 people in England and Wales. The proportion of

Persistent and transient "conduction block" in motor neuron diseases.

Although conduction block indicates dysfunction of peripheral nerve, it may occur in patients with clinically typical motor neuron disease. There are no universally accepted criteria to identify conduction block, so diagnosis may be difficult. In some peripheral neuropathies, conduction block persists over long periods of time. If conduction block persists in motor neuron disease, then a more reproducible means for identification would be available. We repeatedly studied 9 patients with different forms of motor neuron diseases; conduction block was suspected because of excessive loss of the am

Motor neuron diseases and viruses: poliovirus, retroviruses, and lymphomas.

The viral theory of motor neuron disease (MND) has been rejuvenated in the last 5 years for several reasons. First, it is now recognized that enteroviruses and picornaviruses similar to poliovirus can persist and induce immune-mediated diseases. In some picornavirus animal models, the immune-mediated disease can occur and continue long after the infectious virus has been cleared, and in some cases of human MND an immune-mediated disease may occur. Second, the human retroviruses human immunodeficiency virus (HIV) and human T-cell lymphotrophic virus (HTLV) have caused isolated MND syndromes. Ne
