Physiology of hypokinetic and hyperkinetic movement disorders: model for dyskinesia.

Although the basal ganglia have been implicated in the development of movement disorders since the 1940s, the exact role played by these structures has remained elusive. The development of the 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine-monkey model of parkinsonism, and the recent resurgence of surgical therapy for the treatment of hypokinetic and hyperkinetic movement disorders has, however, led to an improved understanding of the pathophysiological mechanisms that underlie their development. In this article, we review the functional organization and examine the changes in neuronal activity …

Abnormal soft palate posturing in patients with laryngeal movement disorders.

Laryngeal movement disorders (LMDs), including spasmodic dysphonia and essential voice tremor, have been described as focal disorders affecting the muscles of the larynx. Little reference has been made to possible hyperfunction of supralaryngeal structures and/or palatal involvement. Videonasolaryngoscopic examinations of 83 patients with LMDs revealed a significantly high incidence of abnormal soft palate posturing (84%). Further associations and implications are presented.

Therapeutic stereotactic procedures on the thalamus for motor movement disorders.

The value of functional neurosurgery in the treatment of motor movement disorders is emphasized. The two methods of stereotactic procedures, namely a destructive one with small lesions centered on specific targets, and a non-destructive one with chronically inserted electrodes connected with an also implanted programmable neuropacemaker are described in detail. The results in Parkinsonian tremor, essential tremor, tremor of multiple sclerosis, post-traumatic tremor and in other involuntary movement disorders are reported and demonstrate that stereotactic neurosurgical treatment of these condit…

[Spastic movement disorders: non-medicamentous therapies based on neurophysiologic principles].

Lesions of the upper motor neurons cause a spastic syndrome and a central paresis. The consequence of these "plus" and "minus" symptoms on locomotor function is a spastic movement disorder. Strategies of physical therapy for the spastic movement disorders are discussed in relation to research findings in neurophysiology and muscle physiology. The main therapeutic technics are physiotherapy, functional electrical stimulation (FES) and biofeedback training (BFB). Various methodological problems which arise in carrying out and evaluating therapeutic studies are described. Valid prognostic criteri…

The effect of nocturnal physiological sleep on various movement disorders.

Thirty-one subjects affected by different movement disorders underwent polygraphic and videotape monitoring during nocturnal sleep, to assess movement patterns during the night. It was possible to distinguish two categories of disorders according to their pattern of movements. In the largest group (Meige's syndrome, blepharospasm, amyotrophic choreoacanthocytosis, Tourette syndrome, tonic foot, hemiballism) abnormal movements were still present during sleep, but decreased in frequency and amplitude in all stages. The second group presented three syndromes (nocturnal paroxysmal dystonia, noctur…

[Spinal mechanisms of movement disorders in torsion dystonia (electromyographic analysis)].

A clinico-electromyographical study of 25 patients with torsion dystonia was carried out. The author studied evoked responses of the muscles of the lower extremities (H-reflex, M-response, F-wave) during the electric stimulation of the tibial nerve and the general electric muscular activity both at rest and in hyperkinesia. Two types of changes in the electromyographic data were revealed in the examined patients. It is suggested that there are certain differences in the neurophysiological mechanisms of motor disturbances in torsion dystonia. The role of the spinal apparatus in the development

The ever-expanding universe of botulinum neurotoxin in movement disorders.

Botulinum neurotoxin (BoNT) has become a cornerstone in the management of movement disorders since its introduction into neurological practice. Initially established as a first-line therapy for focal dystonias and other hyperkinetic conditions, its efficacy is supported by robust clinical evidence and guideline recommendations. BoNT exerts its therapeutic effects through inhibition of acetylcholine release via cleavage of SNARE proteins, resulting in reversible chemodenervation, with additional evidence supporting modulation of sensory pathways and central motor circuits. This present review i…

Highlighting the value of polymyography in childhood onset movement disorders.

INTRODUCTION: Characterizing hyperkinetic Movement Disorders (MD) in children is challenging, particularly when distinguishing tremor from myoclonus. Polymyography (EMG combined with accelerometry) and EEG jerk-locked back-averaging are well-established diagnostic tools in adults but are rarely applied in paediatric population. This study aimed to assess the feasibility and contribution of individualized neurophysiological investigations to the classification of hyperkinetic MD in children. MATERIALS AND METHODS: We retrospectively reviewed clinical and neurophysiological data from consecutiv…

Presenting and residual movement disorders in all-cause autoimmune encephalitis.

BACKGROUND: Movement disorders are among the most common autoimmune encephalitis (AE) symptoms, yet their detailed phenomenology and impact remain underexplored. OBJECTIVE: We evaluated the prevalence, characteristics, and impact of movement disorders at presentation and latest follow-up in all-cause AE. METHODS: We retrospectively reviewed AE patients from a tertiary neuroimmunology clinic. Movement disorders were characterized at presentation and latest follow-up and compared between antibody-positive and antibody-negative disease. Outcomes were assessed using the Modified Rankin Scale (mR…

Cancer-related movement disorders: A scoping review and diagnostic approach.

Cancer-related movement disorders (CRMDs) comprise a diverse group of neurological complications of cancer. They result from varied etiologies and can be associated with compromised quality of life and poor prognosis. CRMDs can be divided into two broad categories: movement disorders resulting from cancer-related processes, such as direct tumor infiltration and paraneoplastic disease, and those that are a consequence of cancer-directed treatments, including classic therapies, such as chemotherapy and radiation therapy, novel treatments (eg immunotherapies and CAR T-Cell therapies), and various…

Eye movement disorders in genetic dystonia syndromes: A literature overview.

With the growing possibilities in genetic testing, the number of genetic disorders associated with dystonia has constantly increased over the last few years. Accurate phenotyping is crucial to guide and interpret genetic analyses in the search for an etiological diagnosis. Although eye movements examination has proven a valuable tool in the assessment of patients with inherited movement disorders such as ataxia or parkinsonism, less is known about the association between eye movement disorders and genetic dystonia. This study aimed to summarize the most frequent eye movement disorders in monog…

Automating the analysis of eye movement for different neurodegenerative disorders.

The clinical observation and assessment of extra-ocular movements is common practice in assessing neurodegenerative disorders but remains observer-dependent. In the present study, we propose an algorithm that can automatically identify saccades, fixation, smooth pursuit, and blinks using a non-invasive eye tracker. Subsequently, response-to-stimuli-derived interpretable features were elicited that objectively and quantitatively assess patient behaviors. The cohort analysis encompasses persons with mild cognitive impairment (MCI), Alzheimer's disease (AD), Parkinson's disease (PD), Parkinson's
