OBJECTIVES: Hybrid operating rooms (HORs) incorporating robotic C-arm systems facilitate concurrent localization and resection of small pulmonary nodules, albeit with significant cost barriers. Contemporary mobile 3 D C-arm systems may provide superior soft tissue visualization with enhanced economic viability and accessibility. This prospective single-arm clinical pilot trial was designed to assess the technical feasibility, clinical efficacy, and procedural safety of employing mobile 3 D C-arm technology for single-stage localization and excision of small pulmonary nodules.

BACKGROUND: DEP KP-80 is a real-world prospective study evaluating the effectiveness of organized lung cancer screening using low-dose chest CT (LDCT) across a French administrative department. Final results were published in 2024. The study, conducted from May 2016 to February 2020, included individuals aged 55-74 who were current or former smokers (within the past 15 years) with a smoking history of at least 30 pack-years. This ancillary analysis aimed to assess and compare the CT characteristics and demographic profiles of participants presenting with benign or malignant nodules within the

This article details a modified method of tactile localization for deep pulmonary nodules of less than 20 mm size and at a distance of more than 15 mm from the visceral pleura. Sixteen patients with deep nodules were successfully located and underwent subsequent thoracoscopic wedge resection. This technique is a continuance of the traditional finger-tough method, which can be an effective complementary technique.

While performing thoracoscopic wedge resection of the lung, the location of the lesion is generally identified by visual inspection or palpation. When difficulty in identification of the lesion by thoracoscopy is anticipated, preoperative marking is performed. However, complications and technical difficulties plague current marking techniques. To overcome this problem, we designed a new, safe and easy marking technique that avoids pleural puncture, called the intrathoracic stamping method.

BACKGROUND: Sjogren's syndrome (SS), which affect salivary gland function, is an autoimmune disease. SS may involve extraglandular organs. Approximately 10 to 20 percent of SS patients have clinically significant lung disease, but presentation of pulmonary amylodosis is extremly rare. The incidence of benign monoclonal gammopathy in SS patients is high, but multiple myeloma is rare. No case involving the simultaneous occurrence of two rare diseases, pulmonary amyloidosis and multiple myeloma, in the same patient with SS has been reported so far.

Pulmonary involvement in multiple myeloma (MM) is exceedingly rare and poses a diagnostic challenge due to its overlap with infectious and neoplastic pulmonary diseases. We describe the case of a 61-year-old man with known MM who presented with progressive respiratory symptoms. High-resolution CT (HRCT) revealed randomly distributed pulmonary nodules with interstitial thickening, mimicking granulomatous or metastatic disease. CT-guided biopsy demonstrated monoclonal plasma cell infiltration positive for CD138 and kappa light chain restriction, confirming pulmonary involvement by MM. This case

A 3-year-old Himalayan cat was presented with respiratory distress. Radiography showed multiple gas opacity foci, with the locations dependent on patient positioning, and severe pleural effusion with a contralateral mediastinal shift. A large volume of fluid and air was aspirated, and the fluid components were consistent with a nonseptic exudate. A pulmonary mass, pleural nodules, and an air-fluid interface with air bubbles trapped in fibrous septations were identified using thoracic ultrasonography and CT. The cat died 2 days after imaging studies. Necropsy revealed tension pyopneumothorax ca

Mixed dust pneumoconiosis secondary to domestic wood smoke exposure is a cause of pneumoconiosis in women from developing countries, but is rarely seen in the USA. An elderly female non-smoker, who immigrated to the USA from Pakistan 10 years previously, presented with a worsening non-productive cough and dyspnoea on exertion. She did not have any occupational or environmental exposures other than utilising firewood for cooking while living in Pakistan. Radiographs revealed multiple bilateral pulmonary nodules associated with hilar and mediastinal adenopathy. A video-assisted thoracoscopic bio

Descending necrotizing mediastinitis is a severe infection of the mediastinum. Lemierre's syndrome manifests as thrombophlebitis in the cervical veins, which is caused by a neck infection. A 50-year-old woman had a fever with a 13-day history of neck pain. CT showed a deep neck abscess with vertebral vein thrombosis and mediastinal abscesses. Multiple small pulmonary nodules were found, suggesting septic pulmonary embolism. Emergency surgery for bilateral cervical and mediastinal drainage was performed, and antibiotics and anticoagulation agent was administered. Edoxaban was required to clear

Pulmonary cryptococcoma is an unusual disease in immunocompetent patients. The typical radiologic findings are single or multiple small nodules. We describe a case of a large pulmonary cryptococcoma in a 69-year-old patient, who presented with generalized weakness and poor appetite. Computed tomography of the chest showed a large mass that was strongly suggestive of lung cancer. Pulmonary cryptococcal infection was diagnosed by transthoracic needle biopsy. Human immunodeficiency virus testing was negative. The mass resolved with 200 mg daily of oral fluconazole over the course of 6 months.

Skin and lung findings, including the rare findings of leukocytoclastic vasculitis (LCV) and necrobiotic pulmonary nodules, are known extraintestinal manifestations (EIMs) of inflammatory bowel disease. However, these rare EIMs have typically been reported as being a sign of active intestinal disease. We report an ulcerative colitis patient with multiple skin and pulmonary findings while in endoscopic remission. Clinicians should remain alert to the possibility of EIMs, even in the absence of intestinal symptoms.

We present a case of atypical presentation of secondary syphilis with extensive lymph node involvement and pulmonary lesions, initially suspected as lymphoma. The patient presented with weight loss, dry cough, chest pain, palpable lymph nodes in several peripheral chains, and multiple pulmonary nodules and masses on chest imaging. The key features for secondary syphilis diagnosis were a lymph node biopsy suggestive of reactive lymphadenopathy, positive serologic tests for syphilis, and complete recovery after antisyphilitic treatment.

Tuberous sclerosis (TS) is an autosomal-dominant disorder characterized by a variety of hamartomatous lesions in various organs. Various organ involvements can be detected with radiology: neurologic involvement (cortical tubers, subependymal nodules and white matter abnormalities), renal involvement (angiomyolipoma) and pulmonary involvement (pulmonary lymphangioleiomyomatosis, multifocal micronodular pneumocyte hyperplasia). Here, we are reporting a case of tuberous sclerosis with multiple organ involvement and massive bilateral AMLs in a 49-year-old female patient.

Wegener's granulomatosis is a multisystem disorder characterized by necrotizing granulomatous inflammation and vasculitis of small vessels and can affect any organ system. The most common sites of involvement are upper and lower respiratory tracts, and kidneys. Breast involvement is unusual and very rare. We report a case of breast Wegener's granulomatosis in a 32-year-old woman who presented with pulmonary lesions and palpable masses in the left breast. Mammography showed multiple, sharply delineated nodules without microcalcifications. Ultrasonography revealed multiple hypoechoic solid lesio

Lymphomatoid granulomatosis is defined as an Epstein-Barr virus-associated B-cell lymphoma which possibly progresses to malignant lymphoma. The pulmonary lesions associated with this disease typically reveal multiple poorly defined nodules. A 71-year-old man with acute respiratory failure consulted a local doctor. A chest CT scan demonstrated a bilateral lower ground glass appearance. The shadow improved after pulse steroid therapy but it showed multiple nodules after the corticosteroid treatment was tapered. We finally diagnosed lymphomatoid granulomatosis (grade 2) based on the findings of a

BACKGROUND: Preoperative computed tomography-guided marking can help identify small non-palpable pulmonary nodules during surgery. However, this technique is associated with the risk of air embolism. We retrospectively evaluated whether small pulmonary nodules could be intraoperatively localized using cone-beam computed tomography (CBCT).
METHODS: A hybrid operating room permitting stable lateral positioning and scanning from the pulmonary apex to the base was used in all patients. CBCT images were obtained using a 10-s protocol with 180º rotation of the C-arm flat panel detector around the p

PURPOSE: Optimization of the clinical management of screen-detected lung nodules is needed to avoid unnecessary diagnostic interventions. Herein we demonstrate the potential value of a novel radiomics-based approach for the classification of screen-detected indeterminate nodules.
MATERIAL AND METHODS: Independent quantitative variables assessing various radiologic nodule features such as sphericity, flatness, elongation, spiculation, lobulation and curvature were developed from the NLST dataset using 726 indeterminate nodules (all ≥ 7 mm, benign, n = 318 and malignant, n = 408). Diffuse cystic and nodular lung diseases have characteristic imaging findings. The most common causes of cystic lung disease are lymphangioleiomyomatosis and Langerhans cell histiocytosis. Other less common cystic lung diseases include Birt-Hogg-Dube syndrome, lymphocytic interstitial pneumonitis, and light chain deposition disease. Computed tomography is used to differentiate cystic lung disease from emphysema, honeycombing, cavities, and bronchiectasis, which mimic cystic lung disease. Diffuse nodular lung disease are categorized as centrilobular, perilymphatic, and random types. In diffuse
