Selective peripheral denervation in patients with spasmodic torticollis.

The results of selective peripheral denervation of the involved muscles in 35 patients with spasmodic torticollis are reported. We modified the operation first described by Bertrand. Follow-up was 3 months to 2.5 years in 34/35 patients. 73% noticed a significant improvement or disappearance of dystonia and pain following surgery and physiotherapy for 3 months postoperatively. Selective peripheral denervation is recommended for patients with spasmodic torticollis of at least 1-2 years duration which is resistant to conservative treatment. It may also be used in patients who do not respond to i

How Do Skeletal Muscles Die? An Overview.

Clarifying the confusion regarding the term "muscle death" is of great importance, especially for clinicians. In response to various stimuli, skeletal muscle may undergo pathological changes, leading to muscle atrophy and consequently resulting in the loss of muscle strength and function. Depending on the stimulus, skeletal muscles can be induced to die through different mechanisms mainly via apoptosis, autophagy and necrosis. Muscle death may occur secondary to various physiological and pathological conditions such as aging, starvation, immobilization, denervation, inflammation, muscle diseas

A family with severe X-linked arthrogryposis.

Five males are reported with severe X-linked arthrogryposis. Main findings are marked respiratory insufficiency and feeding problems, multiple contractures, deformities of chest and vertebral column, and typical facies. Most of these findings can be explained by a pronounced prenatal and postnatal muscle weakness. The sole living child has severe psychomotor retardation. Several female carriers show mild features (clubfeet, contractures, hyperkyphosis, and slight muscle weakness). One manifesting carrier is affected more severely (multiple contractures, mental retardation, and various dysmorph

Neurotrophic factors improve motoneuron survival and function of muscle reinnervated by embryonic neurons.

Motoneuron death can occur over several spinal levels with disease or trauma, resulting in muscle denervation. We tested whether cotransplantation of embryonic neurons with 1 or more neurotrophic factors into peripheral nerve improved axon regeneration, muscle fiber area, reinnervation, and function to a greater degree than cell transplantation alone. Sciatic nerves of adult Fischer rats were cut to denervate muscles; 1 week later, embryonic ventral spinal cord cells (days 14-15) were transplanted into the tibial nerve stump as the only source of neurons for muscle reinnervation. Factors that …

Manual stimulation of forearm muscles does not improve recovery of motor function after injury to a mixed peripheral nerve.

Transection and re-anastomosis of the purely motor facial nerve leads to poor functional recovery. However, we have recently shown in rat that manual stimulation (MS) of denervated vibrissal muscles reduces the number of polyinnervated motor endplates and promotes full recovery of whisking. Here, we examined whether MS of denervated rat forearm muscles would also improve recovery following transection and suture of the mixed (sensory and motor) median nerve (median-median anastomosis, MMA). Following MMA of the right median nerve, animals received no postoperative treatment, daily MS of the fo

The features registered here suggest that T. cruzi-infected mice developed a bimodal muscle denervation with an early acute period at any time before month 4, followed by reinnervation with a subsequent new acute denervation period by month 6, followed in turn by a slow later reinnervation.

Muscle reinnervation in one or two stages?: experimental study in rats with end-to-side nerve graft.

PURPOSE: To compare muscle reinnervation in one and two surgical stages using end-to-side neurorrhaphy (ESN) without donor nerve injury. METHODS: The experiment was performed on four groups of 20 rats. Group 1 (G1), one stage, received the graft which was sutured to the tibial nerve, with ESN, and its free stump was sutured end-to-end to the distal stump of the sectioned peroneal nerve (PN), all in the same operation. In Group 2 (G2), two stages, the nerve graft was sutured to the tibial nerve, with ESN. Two months later the PN was sectioned and its distal stump connected to the distal stump

Electromyographic abnormalities in Paget disease of the thoracolumbar vertebral column.

This report describes a patient with Paget disease of the lower thoracic and lumbosacral vertebrae who developed slowly progressive paraparesis. The electromyographic findings were abnormal in the thoracolumbar paraspinal muscles and the right medial gastrocnemius muscle. In the presence of a normal myelogram, we concluded that the denervation found by electromyography was due to neural ischemia rather than mechanical compression of neural tissue, as is sometimes the case.

The significantly reduced number of interstitial cells of Cajal in chagasic megacolon (CM) patients might contribute to the pathophysiology of CM.

In addition to neurons, interstitial cells of Cajal (ICC) play an important role in coordinating intestinal motility with a pacemaker function. This study aimed to quantitatively analyze ICC, neurons, and muscular area, the latter to correct for quantitation errors resulting from dilation in case of a megacolon and from the dispersion of ICC that can be attributed to muscular hypertrophy. We analyzed 30 colon samples: ten chagasic megacolon (CM), ten chagasic colons without megacolon (CXM), and ten nonchagasic control patients (NC). We measured the area of muscularis propria and counted the nu

Prevention of Axonal Degeneration by Perineurium Injection of Mitochondria in a Sciatic Nerve Crush Injury Model.

BACKGROUND: Axon degeneration leads to cytoskeletal disassembly, metabolism imbalance, and mitochondrial dysfunction during neurodegeneration or nerve injury. OBJECTIVE: In this study, we assess the possibility of mitigating axon degeneration by local injection of mitochondria in a crushed sciatic nerve. METHODS: Sciatic nerve explants cocultured with mitochondria were assessed for the optimal dosage in local injection and nerve regeneration potential. The left sciatic nerve was crushed in Sprague-Dawley rats and then local injection of mitochondria into the distal end of the injured nerve w

[Electroneuromyographic criteria of amyotrophic lateral sclerosis].

Electrophysiological study plays a key role in the diagnosis of amyotrophic lateral sclerosis (ALS). ENMG demonstrates lower motor neuron involvement in muscles clinically involved but also in non clinically involved territories. Conventional EMG shows mixed signs of active denervation, partial chronic denervation and fasciculation, in a variable pattern from one muscle to another. The choice of the muscles to be recorded will privilege muscles innervated by motor neurons from bulbar region, cervical, thoracic and lumbar medulla in order to define the extension of the pathological process. A Therapeutic Strategy for Lower Motor Neuron Disease and Injury Integrating Neural Stem Cell Transplantation and Functional Electrical Stimulation in a Rat Model.

Promising treatments for upper motor neuron disease are emerging in which motor function is restored by brain-computer interfaces and functional electrical stimulation. At present, such technologies and procedures are not applicable to lower motor neuron disease. We propose a novel therapeutic strategy for lower motor neuron disease and injury integrating neural stem cell transplantation with our new functional electrical stimulation control system. In a rat sciatic nerve transection model, we transplanted embryonic spinal neural stem cells into the distal stump of the peripheral nerve to rein

X-linked bulbospinomuscular atrophy (Kennedy's disease) masquerading as lead neuropathy.

A 43-year-old male was referred by a veterinarian who evaluated his dog for a seizure and suspected a toxic lead exposure for both. He refurbished houses, removing old paint, and complained of decreased cognition, fatigue, and muscle cramps. He had a depressed affect, postural tremor, right arm weakness with partial denervation on EMG, and borderline-low sensory nerve action potential (SNAP) amplitudes. A mild anemia and elevated serum and urine lead levels supported a diagnosis of lead neuropathy. Chelation therapy increased urine lead excretion without symptomatic improvement. His brother wo

Botulinum toxin therapy for cervical dystonia.

Cervical dystonia (CD) is the most common form of focal dystonia treated with botulinum toxin (BoNT) injections. BoNT has been shown in numerous clinical trials to correct the abnormal posture and movement and to markedly reduce pain associated with CD. In addition, BoNT has favorably modified the natural history of the disease by preventing contractures and other complications of CD, such as secondary degenerative changes of the cervical spine and associated radiculopathy. In a long-term follow-up of patients treated for up to 20 years, the duration of response appears to be sustained and the

Reorganization of motor units in reinnervated muscles of the rat.

Changes in motor unit organisation following nerve injury in adult and neonatal rats were compared. Motor units were studied in extensor digitorum longus muscles reinnervated after nerve injury in either neonatal or adult rats. The force developed by individual motor units was measured by stimulating ventral root filaments. After nerve section in adult rats the distribution of motor unit force was restored to normal but this did not occur following nerve crush in neonatal animals. Thus following nerve injury during the neonatal period the muscles were not only permanently weaker, but the distr

Loss of supersensitivity of the cat eye to carbachol at prolonged periods after ciliary ganglionectomy.

Adult female cats (2.4-2.8 kg) underwent surgical removal of the left ciliary ganglion under pentobarbital anesthesia. Twenty-one, 560 and 616 days later, pupil size of both left and right (control) eyes was measured in response to progressively increasing doses of carbachol applied topically. By 21 days after denervation, ganglionectomized eyes showed marked supersensitivity to the miotic effects of pilocarpine and carbachol. By 560 days, however, responsiveness of the denervated eyes to lower and intermediate doses of carbachol was the same as that of contralateral control eyes, while respon

[Neurogenic muscle hypertrophy].

Enlargement of the calf usually is associated with venous thrombosis, haemorrhage, focal myositis or a tumour in bone or muscle. Occasionally a calf enlargement is due to isolated muscle hypertrophy as a sign of radicular or peripheral nerve injury. Most neurogenic muscle hypertrophy is confined to the calf muscle, but the phenomenon has also been seen in other muscles. Three patients with S-I radiculopathy leading to ipsilateral neurogenic calf hypertrophy following hernia nuclei pulposi are described, two men of 79 and 78 years old and a woman of 46 years. The symptoms gradually subsided wit

Reversal of paralysis in nerve-muscle preparations isolated from animals with hereditary motor endplate disease.

Motor endplate disease (med) in the mouse is an hereditary disorder of the skeletal neuromuscular system. Affected animals suffer a 'functional denervation' of skeletal muscle (Duchen & Stefani, 1971). Muscle fibres do not respond to indirect excitation, but motor nerve terminals release transmitter spontaneously. Spontaneous transmitter release can be enhanced by raising [K+]o or by exposing muscles to red-back spider venom and functional transmission following indirect stimulation may be restored by 4-aminopyridine.
