When the Doctor Doesn't Know: Exploring the Liminal Space Occupied by Patients and Caregivers Living with Cyclic Vomiting Syndrome.

Cyclic Vomiting Syndrome (CVS) is a poorly understood and frequently stigmatized chronic condition characterized by recurrent, disabling episodes of nausea and vomiting. This qualitative study analyzes illness narratives from adults living with CVS and their caregivers, drawing on a modified social practice framework to examine how individuals navigate diagnostic uncertainty, clinical dismissal, and systemic neglect. Participants describe a pattern of epistemic rupture-where the failure of healthcare professionals to recognize and validate their suffering undermines not only clinical relations

"To Remember Is to Live Again": Geragogical Approaches to Online Storytelling With Older Immigrants.

Seeing aging not solely as a biological process but as a meaning-making practice, this paper presents findings from an exploratory study conducted within a community-based organization's transition to online educational programming during the COVID-19 pandemic. The study aimed to explore how collaborative storytelling functioned as a geragogical practice in an online setting. The study involved 17 Spanish-speaking older immigrants in co-designing an age- and culturally responsive online storytelling curriculum. Through creative and reflective prompts, the virtual sessions facilitated autoethno

Framing, Narratives, and the Overdose Crisis.

Narratives and frames have shaped the overdose crisis since its early stages. Efforts to control knowledge about the role of opioids in chronic pain have influenced clinical guidelines and prescribing behaviour. Dominant narratives shape policy by influencing how problems are defined, and which solutions are considered appropriate. A more nuanced understanding of how framing shapes interactions among stakeholders, including patients, clinicians, advocacy groups, industry, educators, and regulators, can clarify these dynamics. Engaging multiple perspectives, rather than relying on a single domi

Electroencephalographic Functional Connectivity, Heartrate Synchrony, and Eye Movements Reveal Distinct Components within Narrative Engagement and Immersion.

Storytelling is a fundamental and universal human behavior, representing a vehicle for cultural information exchange throughout human history. In the present day, consumption of narrative audiovisual media is one of the most common recreational activities worldwide. Despite the importance and ubiquity of storytelling, relatively little is known about the neurocognitive mechanisms by which narrative media capture and sustain our attention. In this study, 40 participants watched 10 short clips from television shows of various genres while electroencephalography, eye tracking, heart rate, and sel

Recognizing Humanity, Perceiving Ableism: Supporting Parents of Children with Down Syndrome.

This commentary reflects on twelve narratives authored by parents of children with Down syndrome. While the narratives represent a diverse set of experiences, the commentary identifies some repeated narrative aims, unified under the goal of recognition. For example, multiple authors explicitly or implicitly seek recognition of their children beyond stereotypes while others hope to make ableist values and practices more visible to clinicians and other readers. Through their rich descriptions of parenting, the authors also show themselves to be in need of support both in terms of material and bu

ARCH: Large-scale knowledge graph via aggregated narrative codified health records analysis.

OBJECTIVE: Electronic health record (EHR) systems contain a wealth of clinical data stored as both codified data and free-text narrative notes (NLP). The complexity of EHR presents challenges in feature representation, information extraction, and uncertainty quantification. To address these challenges, we proposed an efficient Aggregated naRrative Codified Health (ARCH) records analysis to generate a large-scale knowledge graph (KG) for a comprehensive set of EHR codified and narrative features. METHODS: Using data from 12.5 million Veterans Affairs patients, ARCH first derives embedding vect

Recovering the Lost Soul: Older Women's Reflections on Past Intrafamilial Child Sexual Abuse.

Intrafamilial child sexual abuse (IFCSA) is a social problem with longstanding effects on victims' lives. While scholarly literature has focused on the adverse aftermath of sexual abuse, only a few studies have acknowledged older women's perspectives on their experience of IFCSA and their journey of healing and recovery. The aim of the present study was to explore how older survivors of IFCSA construct and shape their experience of healing in later life and the meaning they assign to this process. Narrative inquiry was selected to explore the narratives of 11 older women survivors of IFCSA. Pa

An Examination of the Synergy of Age and PTSD on Narrative Coherence in Child Sexual Abuse Testimony.

In a criminal proceeding, a witness is considered as reliable if he/she can recall in narrative form the events, chronologically ordered, with salient contextual (place and time) details, and essential evaluations for the definition of meanings. This study aimed to confirm the effects of age and PTSD on narrative coherence and to investigate the moderating role of age on the association between PTSD and narrative coherence. Narrative coherence was analyzed in 92 allegations of children (M = 10.3; range: 4-17), who were victims of sexual abuse. Thirty-five children presented the symptoms for a

Youth participatory research evidence to inform health policy: a systematic review protocol.

INTRODUCTION: Young people's participation in health research produces knowledge that is indispensable for creating appropriate and effective policies. However, how best to disseminate youth participatory research evidence to impact health policy is not known. Therefore, the objectives of this systematic review are to describe the evidence produced through youth participatory research, including the strategies used to disseminate youth participatory research evidence to health policymakers. These are necessary to improve policymakers' use of youth participatory research evidence and, thereby,

Predictors of Dropout in Cognitive Processing Therapy for PTSD: An Examination of Trauma Narrative Content.

Dropout rates in trauma-focused treatments for adult posttraumatic stress disorder (PTSD) are high. Most research has focused on demographic and pretreatment predictors of dropout, but findings have been inconsistent. We examined predictors of dropout in cognitive processing therapy (CPT) by coding the content of trauma narratives written in early sessions of CPT. Data are from a randomized controlled noninferiority trial of CPT and written exposure therapy (WET) in which CPT showed significantly higher dropout rates than WET (39.7% CPT vs. 6.4% WET). Participants were 51 adults with a primary

Which Emergency Medicine Milestone Sub-competencies are Identified Through Narrative Assessments?

INTRODUCTION: Evaluators use assessment data to make judgments on resident performance within the Accreditation Council for Graduate Medical Education (ACGME) milestones framework. While workplace-based narrative assessments (WBNA) offer advantages to rating scales, validity evidence for their use in assessing the milestone sub-competencies is lacking. This study aimed to determine the frequency of sub-competencies assessed through WBNAs in an emergency medicine (EM) residency program. METHODS: We performed a retrospective analysis of WBNAs of postgraduate year (PGY) 2-4 residents. A shared m

Assessing sociodemographic differences in human papillomavirus vaccine impact studies in the United States: a systematic review using narrative synthesis.

OBJECTIVES: Sociodemographic disparities in the incidence and mortality of human papillomavirus (HPV)-associated conditions have been well documented in the pre-HPV vaccine era. It is still unknown if the introduction of routine vaccination has been effective in reducing these prevaccine era inequalities. The purpose of this review was to determine the utilization of sociodemographic variables to assess for disparities in population-level HPV vaccine impact research and to evaluate the current evidence for disparities in the reduction of HPV-associated conditions after vaccine introduction in

Evaluating the Impact of a Multistrategy Inference Intervention for Middle-Grade Struggling Readers.

PURPOSE: We examined the effectiveness of a multistrategy inference intervention designed to increase inference making and reading comprehension for middle-grade struggling readers. METHOD: A total of 66 middle-grade struggling readers were randomized to treatment (n = 33) and comparison (n = 33) conditions. Students in the treatment group received explicit instruction in 4 inference strategies (i.e., clarification using text clues; activating and using prior knowledge; understanding character perspectives and author's purpose; answering inferential questions). In addition, narrative and info

On Becoming an Involuntary Member in the Antepartum Unit.

In this essay, I articulate the ways in which my scholarship and personal life collided when I became an involuntary member in the antepartum unit of a major university hospital. I draw on research examples taken from my dissertation work in prison and my time in the hospital to illustrate the interconnectedness of these involuntary experiences. After I share these stories, I offer a brief interlude to reflect on the meaningfulness of approaching membership from a continuum-based perspective and the relative implications for health communication scholars, before ending with an articulation how

Historical trauma as public narrative: a conceptual review of how history impacts present-day health.

Theories of historical trauma increasingly appear in the literature on individual and community health, especially in relation to racial and ethnic minority populations and groups that experience significant health disparities. As a consequence of this rapid growth, the literature on historical trauma comprises disparate terminology and research approaches. This critical review integrates this literature in order to specify theoretical mechanisms that explain how historical trauma influences the health of individuals and communities. We argue that historical trauma functions as a public narrat

Gradually losing one's foothold--a fragmented existence when living alone with dementia.

The number of persons with dementia who lives at home for a longer period of time after diagnosis is increasing. Even if the literature in the dementia field is growing, there is a need for more knowledge about everyday life of persons with a dementia disease; particularly the lived perspective of persons who live alone. The aim of this study was to elucidate the phenomenon of living alone with dementia and having a manifest care need. This phenomenological study was carried out from a reflective lifeworld approach. The data material in the study consisted of field notes from 32 visits and tra

Coexisting addiction and pain in people receiving methadone for addiction.

The aim of this qualitative study was to examine the narratives of people who experience chronic pain (lasting 6 months or more) and were receiving methadone for the treatment of their opiate addiction through a major methadone clinic. This paper featured the pathway of how the participants developed chronic pain and addiction, and their beliefs of how prescription opioids would impact their addiction in the future. Thirty-four participants who experienced chronic pain and received methadone for treatment of opiate addiction were willing to tell the story of their experiences. The findings in

Illness, everyday life and narrative montage: the visual aesthetics of cancer in Sara Bro's Diary.

This article presents a study of Sara Bro's Diary (2004), a book montage of images and texts recording the experiences of a Danish breast cancer survivor, Sara Bro. It examines two montages of photography and text, drawing on Roland Barthes' concept of 'the third meaning' to explain and discuss the effect of the layered meanings in the montage alongside their multi-medium and self-referential expression. The discussion is centred on the aesthetic practices that are invited by Bro's book montage. The article considers how the juxtaposition of images and texts are experienced and co-created by t

Becoming a vampire without being bitten: the narrative collective-assimilation hypothesis.

We propose the narrative collective-assimilation hypothesis--that experiencing a narrative leads one to psychologically become a part of the collective described within the narrative. In a test of this hypothesis, participants read passages from either a book about wizards (from the Harry Potter series) or a book about vampires (from the Twilight series). Both implicit and explicit measures revealed that participants who read about wizards psychologically became wizards, whereas those who read about vampires psychologically became vampires. The results also suggested that narrative collective
