BACKGROUND: Determinants for adequate lymph node yield (LNY) in prophylactic central neck dissection (pCND) for papillary thyroid carcinoma (PTC) remain unclear. We aimed to determine factors affecting LNY in pCND.
METHODS: Of 230 patients, 109 (47.4%) had total thyroidectomy and unilateral pCND. A specimen of ≥ 6 central lymph nodes (CLNs) was considered adequate. Factors such as patient clinicopathologic features, specimen dimensions, and pathologists' experience were compared between those with LNY < 6 (n = 52) and LNY ≥ 6 (n = 57). A multivariate analysis was conducted to identify i…

OBJECTIVES: Investigate if choice of neck management by sentinel lymph node biopsy (SLNB) or elective lymph node dissection (ELND) affects disease specific (DSS) or overall survival (OS) in cT1-T2 N0 oral cancer patients subsequently diagnosed with occult metastasis (pN+).
METHODS: Retrospective study of cT1-T2 N0 OSCC upstaged pathologically (pN+) by either SLNB or ELND at three major UK head and nek surgery centres between 2006 and 2021. Univariate and multivariate Cox proportional hazard regression analysed factors influencing survival. The most influential hazard ratios were selected for

OBJECTIVES: Shoulder dysfunction syndrome is common in patients with oral cancer after neck dissection. Existing rehabilitation programs are mostly based on fixed stage-specific exercises and lack dynamic adjustment mechanisms according to patients' real-time conditions. This study aims to investigate the effects of a structured rehabilitation training program on shoulder dysfunction syndrome in patients after neck dissection for oral cancer and to provide evidence for the development of precise rehabilitation strategies in clinical practice.
METHODS: A total of 98 patients who underwent oral…

BACKGROUND: Sentinel lymph node biopsy (SLNB) has been introduced as a diagnostic staging modality for detection of occult metastases in patients with early stage oral cancer. Comparisons regarding accuracy to the routinely used elective neck dissection (END) are lacking in literature.
METHODS: A retrospective, multicenter cohort study included 390 patients staged by END and 488 by SLNB.
RESULTS: The overall sensitivity (84% vs. 81%, p = 0.612) and negative predictive value (NPV) (93%, p = 1.000) were comparable between END and SLNB patients. The END cohort contained more pT2 tumours (51%) c…

Traditional neck dissection for oral squamous cell carcinoma (OSCC) involves removal of the submandibular salivary gland. Several studies have cited the low incidence of direct gland invasion by tumours and have recommended gland-sparing neck dissection. In this study, a detailed audit of level Ib involvement in OSCC was performed in order to assess the feasibility of submandibular gland-sparing in neck dissection; the rate of direct involvement by the primary tumours, the involvement of periglandular level Ib nodes, and their determinants were investigated. A total of 586 neck dissection spec…

BACKGROUND: Chyle fistula is a relatively rare complication of neck dissection, and there is a lack of consensus regarding its incidence, risk factors, and management.
METHODS: Between 2007 and 2014, a total of 472 cases of neck dissection involving the level IV compartment were included in the study. The incidence, risk factors, and clinical course of chyle fistula were investigated, as well as the outcomes of conventional management and the use of octreotide injection in high-output chyle fistula.
RESULTS: The overall incidence of chyle fistula was 4.7 % (22/472), with an incidence of 3.0 …

BACKGROUND: Sentinel lymph node biopsy (SLNB) plays an important role in the prognostic classification of melanoma and is now a standard staging procedure. However, due to the complex drainage pattern and the risk of site associated morbidity, the potential survival benefit of SLNB is controversial in head and neck (H&N) melanoma.
METHODS: Patients with primary H&N melanoma with a tumor thickness ≥1.00 mm diagnosed in the Department of Dermatology, University of Tuebingen, Germany between 1991 and 2010 were included in this study. Regarding patterns of metastases, disease-free, and overall-su

BACKGROUND: Recently, the role of chemoradiotherapy (CRT) for preserving organs in the treatment of head and neck cancer has been increasing. However, the indication for post-CRT neck dissection (ND) and its surgical extent is still controversial. The purpose of this study was to discuss the indications for post-CRT ND and the proper extent of the surgical procedure.
METHODS: We performed a retrospective analysis on N2-3 oropharyngeal and hypopharyngeal squamous cell carcinoma (OHSCC) patients treated with CRT in our institute from 1995 to 2008, and determined the prognostic impact of post-CR…

OBJECTIVE: To investigate the safety and efficacy of the no-tie (NT) technique using the harmonic scalpel (HS) in terms of the operating time and complications in total thyroidectomy with central neck dissection (CND).
SUMMARY BACKGROUND DATA: Recently, the HS has been used as an alternative to conventional hand-tied ligation for hemostasis in thyroid surgery, which is a time-consuming procedure. Limited data have been published on the evidence of its safety in total thyroidectomy accompanied by CND without supplementary hand-tied ligation.
METHODS: Sixty-five consecutive thyroidectomized pa…

PURPOSE: We assess the incidence and location of positive surgical margins in a consecutive series of patients who underwent laparoscopic radical prostatectomy.
MATERIALS AND METHODS: Between May 1998 and September 2001, 235 men underwent laparoscopic radical prostatectomy at our institution. During this period modifications were made in the technique. We stopped preserving the puboprostatic ligaments during 1999 and stopped preserving the bladder neck during 2000. We compared the incidence of apical and bladder neck margins before and after these changes in surgical technique. Nerve sparing

INTRODUCTION: Head and neck cancer (HNC) affects the mouth, throat, salivary glands, voice box, nose or sinuses. Every year, over 12 000 people in the UK are diagnosed with HNC. Neck dissection is a key, surgical component of patient care. However, many people experience postoperative restriction in shoulder and neck movements, pain, fatigue and low mood, with only half ever returning to work.
METHODS AND ANALYSIS: Getting Recovery Right After Neck Dissection (GRRAND) is a two-arm, multicentre, pragmatic randomised controlled trial. The trial aims to compare clinical and cost-effectiveness of…

PURPOSE: High-grade major salivary gland cancer is associated with poor prognostic outcomes, but the role for elective neck dissection (END) for improving survival in high-grade N0 major salivary gland cancer is under debate, especially in the context of planned adjuvant therapy.
METHODS: We performed a secondary analysis of the SEER cancer database for all patients surgically treated for N0 major salivary gland cancer from 2000 to 2022. Our primary outcome was disease-specific survival (DSS), and our secondary outcome was overall survival. We generated multivariable Cox regression models to …

INTRODUCTION: Submandibular gland (SMG) resection is traditionally included in level IB neck dissection for oral squamous cell carcinoma (OSCC), although isolated SMG metastases are rare. Routine removal may contribute to xerostomia and a reduced quality of life. This systematic review and meta-analysis assessed whether SMG preservation compromises oncologic outcomes or improves functional outcomes.
METHODS: This systematic review and meta-analysis followed PRISMA 2020 and Cochrane guidelines, with prospective registration in PROSPERO (CRD420251027851). The literature search included PubMed, …
