Empiric use of ampicillin and cefotaxime, compared with ampicillin and gentamicin, for neonates at risk for sepsis is associated with an increased risk of neonatal death.

BACKGROUND: We reported previously that the use of cephalosporin among premature neonates increased the risk of subsequent fungal sepsis. As a result, we recommended that ampicillin and gentamicin be used as empiric coverage for early-onset neonatal sepsis while culture results are awaited. OBJECTIVES: To describe antibiotic use during the first 3 days after birth for neonates admitted to the NICU and to evaluate the outcomes for neonates treated with 2 different antibiotic regimens. METHODS: We assembled a cohort of inborn neonates, from our deidentified administrative database, who had doc…

Efficacy of fresh frozen plasma immunotherapy as an adjuvant treatment of neonatal sepsis in puppies.

The aim of this study was to evaluate the effects of fresh frozen plasma (passive immunotherapy) on clinical and laboratorial findings in puppies treated for neonatal sepsis. A total of thirty-one puppies were included in this study, 16 were diagnosed with sepsis and treated either with plasma + antibiotic (plasma group, n = 7; PG) or antibiotic only (control group, n = 9; CG); whereas 15 were healthy neonates (HG). The groups were evaluated at the first appointment (M0), after 24 (M24), 48 (M48), 72 h (M72) and at the end of treatment (ME). At M0, neonates with sepsis presented a reduction (p…

Sphingomonas paucimobilis Bacteremia in a Preterm Neonate: A Rare Cause of Early-Onset Sepsis.

Sphingomonas paucimobilis (S. paucimobilis), an environmental Gram-negative bacillus, is a rare cause of clinical infections in neonates. This case report describes a preterm neonate with early-onset sepsis (EOS) caused by S. paucimobilis. Identification was achieved using the VITEK 2 system, and targeted antimicrobial therapy with meropenem, vancomycin, and metronidazole resulted in clinical improvement. Early recognition and microbiological diagnosis are essential for managing unusual pathogens in neonatal intensive care settings.

Clinical Utility of the Syndromic Meningitis/Encephalitis Molecular Panel in Neonates with Suspected Sepsis.

BACKGROUND: Newborns are highly vulnerable to meningitis due to an immature blood-brain barrier. In cases of suspected clinical sepsis, empirical antibiotics are typically initiated after obtaining appropriate cultures when clinically indicated. This study aimed to evaluate the clinical utility of syndromic meningitis/encephalitis (sME) molecular panel testing in neonates with suspected sepsis, with a focus on its impact on clinical management and outcomes under selective versus routine testing strategies. METHODS: Data of 58 neonates hospitalized between January 2019 and August 2023 who unde

IL-27 neutralization with and without antibiotics as an approach to prevent and treat neonatal sepsis.

Neonatal sepsis is a predominant cause of neonatal mortality and long-term morbidity that severely affects preterm and low-birth-weight newborns. Antibiotic resistance and long-term developmental issues associated with neonatal sepsis necessitate finding new and improved treatment options. IL-27 has diverse influences on the immune response, is elevated during the neonatal period compared with adulthood, and continues to rise further during infection. Elevated levels of IL-27 early in life predispose the host to impaired control of the pathogen burden and increased mortality. This study explor…

Epidemiology and Outcomes of Late-Onset Neonatal Sepsis in Preterm Infants in a Tertiary Hospital.

Background/Objective: Late-onset sepsis (LOS), a systemic infection occurring after 72 h of life, is a significant issue of morbidity and mortality in preterm neonates. Nevertheless, in this population, cultures frequently remain negative, even in the presence of typical clinical signs of sepsis. Materials and Methods: This single-center, retrospective study included preterm infants with a birth weight (BW) < 1500 g and/or a gestational age (GA) ≤ 32 weeks, diagnosed with culture-negative LOS (CNLOS) and culture-proven LOS (CPLOS). The study aimed to determine the incidence of these conditions

Presepsin as a diagnostic biomarker for sepsis across neonates, children, and adults: A meta-analysis.

Sepsis remains a leading global health challenge, with delayed recognition and limited diagnostic accuracy of current tools contributing to high morbidity and mortality. Conventional clinical scores (SOFA/qSOFA), standard biomarkers (CRP, PCT), and blood cultures suffer from delayed responsiveness, insufficient specificity, or slow turnaround, underscoring the urgent need for more reliable early diagnostic strategies. Presepsin, a soluble CD14 subtype generated during pathogen recognition by innate immune cells, has emerged as a promising biomarker with potential to reflect infection status ea

Antibiotic use rate in neonates: A comparison of categorical risk assessment and the sepsis calculator.

BACKGROUND: Early-onset neonatal sepsis (EOS) is defined as an infection with a positive blood culture within the first 7 days of life. The categorical risk assessment guideline from the Centers for Disease Control and Prevention is used to manage high-risk infants, but it often results in a high rate of antibiotic use. A neonatal early-onset sepsis calculator (EOS calculator) was developed to reduce the overuse of antibiotics. METHODS: A retrospective study was conducted among infants born ≥35 weeks' gestation admitted to Vajira Hospital between October 1, 2018, and September 30, 2019. Infan…

Factors Associated with Prolonged Antibiotic Therapy in Neonates with Suspected Early-Onset Sepsis.

Early-onset sepsis (EOS) is a rare but profoundly serious bacterial infection. Neonates at risk of EOS are often treated with antibiotics. The start of empiric antibiotic therapy can successfully be reduced by the implementation of the EOS calculator. However, once started, antibiotic therapy is often continued despite a negative blood culture. To decrease the burden of antibiotic therapy, it is necessary to know whether the clinician's reasons are based on objective factors. Therefore, we performed a retrospective single-centre cohort study to identify the factors associated with prolongation

Role of neutrophil-to-lymphocyte ratio and platelet-to-lymphocyte ratio in diagnosing neonatal sepsis.

Clinical manifestations of neonatal sepsis are often unspecified. Therefore, sepsis biomarkers could be used to support diagnosis while waiting for blood culture results, such as the neutrophil-to-lymphocyte ratio (NLR) and platelet-to-lymphocyte ratio (PLR). The aim of this study was to evaluate the role of NLR and PLR as diagnostic markers in neonatal sepsis. A cross-sectional study was conducted at Haji Adam Malik General Hospital, Medan, Indonesia, from April to October 2019. This study included neonates aged less than 28 days, diagnosed with suspected sepsis, and had no previous history o…

Designing of a unique bioreceptor and fabrication of an efficient genosensing platform for neonatal sepsis detection.

We report the results of studies related to the fabrication of a nanostructured graphene oxide (GO)-based electrochemical genosensor for neonatal sepsis detection. Initially, we selected the fimA gene of E. coli for nenonatal sepsis detection and further designed a 20-mer long amine-terminated oligonucleotide. This designed oligonucleotide will work as a bioreceptor for the detection of the virulent fimA gene. An electrochemical genosensor was further developed where GO was used as an immobilization matrix. For the formation of a thin film of GO on an indium tin oxide (ITO)-coated glass electr…

Cut-off values of serum interleukin-6 for culture-confirmed sepsis in neonates.

INTRODUCTION: Neonatal sepsis accounts for 0.97% of all disability-adjusted life years worldwide. Interleukin-6 has been used in sepsis diagnosis, but cut-off values are missing. METHODS: Neonates admitted to the neonatal wards with measurements of serum interleukin-6 born between September 2015 and September 2019 were retrospectively analysed. Mean serum interleukin-6 values of patients who never had increased laboratory parameters of infection nor died during their stay and mean interleukin-6 values on the day of blood sampling for a later positive culture in patients with culture-confirmed…

Risk Factors for 30-Day Mortality in Neonates With Carbapenem-resistant A. baumannii Sepsis.

BACKGROUND: Carbapenem-resistant Acinetobacter baumannii (CRAB) sepsis is becoming an extreme threat caused by high-case fatality rates and poor prevention and control in ICUs worldwide. However, the risk of mortality among neonatal CRAB sepsis is still unclear. METHODS: A retrospective medical records review study, which aimed to identify the risk factors of mortality in neonates with CRAB sepsis (including both bacteremia and/or meningitis) in Thailand from 1996 to 2019. All cases featuring positive blood and cerebrospinal fluid cultures for CRAB were reviewed. A multivariable logistic regr

Comparison of Early-Onset Sepsis Risk-Stratification Algorithms in Neonates in a Kenyan Nursery.

BACKGROUND AND OBJECTIVES: Risk stratification algorithms (RSAs) can reduce antibiotic duration (AD) and length of stay (LOS) for early-onset sepsis (EOS). Because of higher EOS and antibiotic resistance rates and limited laboratory capacity, RSA implementation may benefit low- and middle-income countries (LMIC). Our objective was to compare the impact of 4 RSAs on AD and LOS in an LMIC nursery. METHODS: Neonates <5 days of age admitted for presumed sepsis to a Kenyan referral hospital in 2019 (n = 262) were evaluated by using 4 RSAs, including the current local sepsis protocol ("local RSA"),

Risk Factors Associated With 30-Day Mortality Among Neonates With A. baumannii Sepsis.

BACKGROUND: Acinetobacter baumannii sepsis constitutes an extreme threat with a poor prognosis and is a difficult infection to control, especially in Asia. Moreover, a knowledge gap in the risk of mortality in neonatal A. baumannii sepsis still exists. METHODS: This study aimed to identify the risk factors of mortality in neonates with A. baumannii sepsis in Thailand from 1996 to 2019. A multivariable logistic regression model was analyzed for nonsurvivors and survivors of neonatal A. baumannii sepsis. RESULTS: In a 24-year period, 91 neonates with A. baumannii sepsis were reviewed. The medi
