Prenatal Diagnosis of Skeletal Dysplasias: What Can CT Do for You?

Skeletal dysplasias (SDs) are a heterogeneous group of heritable disorders that affect development of bone and cartilage. Because each SD is … , maternal-fetal medicine specialists, and fetal radiologists, and sometimes orthopedic surgeons or neonatologists. The purpose of this study was to review the technique of and indications for fetal CT, as well as discuss fetal radiation risk. Illustrative cases will demonstrate when and how CT may be helpful in the diagnosis of SDs.

A Novel DNAH9 Gene Mutation Causing Primary Ciliary Dyskinesia With an Unusual Association of Jejunal Atresia in a Bahraini Child.

Primary ciliary dyskinesia (PCD) is a rare autosomal recessive genetic disorder. It is caused by a defect in the action of the cilia lining … nfirming the diagnosis of this condition. A high degree of suspicion about PCD among pediatricians, neonatologists, otorhinolaryngologists, and pulmonologists is essential to make early referrals of patients before they develop irreversible lung damage. Hence, early diagnosis and appropriate treatment are very important. Multicenter collaborations might improve …

To Feed or Not to Feed: A Critical Overview of Enteral Feeding Management and Gastrointestinal Complications in Preterm Neonates with a Patent Ductus Arteriosus.

The management of enteral feeds in preterm infants with a hemodynamically significant patent ductus arteriosus (hs-PDA) is a major challenge for neonatologists due to the fear of gastrointestinal (GI) complications. This review aims to analyze the available evidence on the complex relation between the presence and management of PDA, enteral feeding practices, and GI outcomes in the preterm population. There is limited evidence, based on small and heterogeneous trials, that hs-PDA may affect the splanchnic hemodynamic response to enteral feeds. While the presence of PDA seems a risk factor for …

Bifid uvula and familial Stickler syndrome diagnosed prenatally before the sonographic "equals sign" landmark.

PURPOSE: The prenatal ultrasound visualization of the soft palate and especially the uvula may be technically difficult due to its anatomy and presence of surrounding structures. A cleft involving the soft palate and the uvula is one of the clinical features of Stickler syndrome, a rare connective tissue disorder. MATERIALS AND METHODS: Third trimester scan performed at 30 weeks' gestation in a pregnant woman with a familial history of Stickler syndrome using conventional 2D ultrasound. RESULTS: Transabdominal scan performed with the fetal head in oblique plane and following fetal swallowing

[Effect of birth timing on perinatal mortality].

Several reports show increased risk of perinatal/neonatal mortality for night time, weekend and holiday births. Intrapartum-related deaths were attributed to physical and mental fatigue, reduced availability of skilled and experienced staff and inadequate supervision by senior staff. The incidence of intrapartum and neonatal deaths related to intrapartum asphyxia is regarded to be a sensitive indicator of the quality of care during labor. Continuity of care is regarded as a major element in providing quality medical care. Several reports indicate the occurrences of potentially preventable adve

[Complicated anesthetic management for esophageal atresia repair in a baby with total anomalous pulmonary venous connection diagnosed after esophageal surgery].

A male newborn infant born at 38 weeks of gestation and weighing 2,660 g, was diagnosed as esophageal atresia. Although there was mild cyanosis at birth, his initial ultrasonographic examination performed by neonatologist showed normal anatomy of the heart. He underwent a surgical repair of esophageal atresia under general anesthesia. Anesthesia was induced with fentanyl and pancuronium, and maintained with sevoflurane and 60% oxygen. Frequent desaturation occurred during the procedure, requiring manual hyperventilation with 100% oxygen. Since blood pressure during the operation was unstable,

[Retinopathy of prematurity: clinical findings and current opinions on diagnosis and treatment].

Retinopathy of prematurity (ROP) is an ophthalmologic condition which may lead to blindness if compounded by other risk factors. Since many years, ROP is among the main causes of impaired visual acuity and blindness in Polish children. Therefore, the understanding of this disease, progress in diagnostics, and improved methods of treatment will contribute to a better prognosis in ROP. Nowadays, diode laser photocoagulation and cryotherapy are modalities used in ophthalmology centers at specific clinical stages of ROP. When performed at the right time, these procedures offer promising results in

The newborn examination: part I. Emergencies and common abnormalities involving the skin, head, neck, chest, and respiratory and cardiovascular systems.

The routine newborn assessment should include an examination for size, macrocephaly or microcephaly, changes in skin color, signs of birth trauma, malformations, evidence of respiratory distress, level of arousal, posture, tone, presence of spontaneous movements, and symmetry of movements. A newborn with one anatomic malformation should be evaluated for associated anomalies. Total and direct bilirubin levels should be measured in newborns with jaundice, and a complete blood count should be obtained in those with pallor or a ruddy complexion. Neurosurgical consultation is necessary in infants w

Systemic lupus erythematosus and pregnancy.

This article critically analyses the data in the literature on pregnancy in women with systemic lupus erythematosus. Based on the results of recent controlled prospective studies, it is apparent that the long-standing opinion that pregnancy induces exacerbation of the disease should be revised. The presence of active disease and/or a significant loss of renal function at conception are not only associated with a high risk of maternal complications, but also with high frequencies of loss of the fetus, as well as pre- and dysmaturity. Recently, the presence of antiphospholipid antibodies, notabl

Perinatal and neonatal aspects of maternal idiopathic thrombocytopenic purpura.

The pregnant woman with idiopathic thrombocytopenic purpura (ITP) and her fetus-neonate present a number of challenging issues for both the perinatologist and neonatologist. A review of these issues within a single format is helpful because of the widely scattered nature of the pertinent information in the medical literature. Maternal ITP is described herein as chronic, idiopathic, immune thrombocytopenic purpura, and the multiple interactions between ITP and pregnancy are detailed. The chief effect of maternal ITP on the fetus-neonate is thrombocytopenia, which occurs in approximately 37% of

Development of a Serious Game to Simulate Neonatal Intensive Care Unit Experiences: Collaborative Quasi-Experimental Study.

BACKGROUND: Opportunities for neonatal intensive care unit (NICU) training are limited for medical and nursing students due to patient safety concerns and the complexities of neonatal care. In addition, the COVID-19 pandemic significantly disrupted clinical training opportunities, further underscoring the need for alternative educational tools that can provide immersive and practical learning experiences. Serious games have garnered attention as potential tools for medical education; however, few are designed to simulate the complete NICU environment and its unique challenges. OBJECTIVE: To a

The wake project: Improving safe sleep practices in a neonatal intensive care unit.

BACKGROUND: Every year, about 50 babies in New York City die from a sleep-related injury. The Bronx County ranked second highest rate of sleep-related infant deaths (SRID) at 0.5 per 1000 among the other boroughs. The highest rate was among blacks and the rate of SRID cases were highest in our population at 0.97 (zip code 10466) among all other Bronx neighborhoods which comprises 77% of non-Hispanic black population. Further, Bronx has the highest preterm birth rate at 9.5%. This quality improvement (QI) project aimed to develop and implement an educational initiative on infant safe sleep (SS)

Intranasal dexmedetomidine for procedural sedation in infants: a retrospective study.

BACKGROUND: Infants undergoing magnetic resonance imaging (MRI) often require pharmacological sedation. Dexmedetomidine is an alpha-2 receptor agonist that is frequently used to sedate children and infants, because the neonatologist can easily adjust sedation depth, the patient maintains spontaneous breathing, and awakens rapidly afterwards. The present study evaluates whether dexmedetomidine could safely be used as the sole sedative for preterm and term-born infants less than 80 weeks' postmenstrual age undergoing diagnostic procedures. METHODS: We performed a retrospective monocentric analy

Early hypovolemic shock and abdominal distention due to neonatal splenic rupture: urgency of diagnosis and management.

UNLABELLED: Splenic rupture in the neonatal period is a rare condition that can be complicated by hemorrhagic shock. The symptoms are not very specific, rendering the diagnosis difficult and often delayed; sometimes only discovered at autopsy. We report five cases diagnosed in the Rhône-Alpes region of France. From these observations and from a review of the literature, the circumstances of the occurrence, the clinical signs, and the therapeutic possibilities are discussed. In the presence of severe anemia with pallor and abdominal distension, particularly in the context of a difficult birth,

Physiologic basis for intermittent hypoxic episodes in preterm infants.

Intermittent hypoxic episodes are typically a consequence of immature respiratory control and remain a troublesome challenge for the neonatologist. Furthermore, their frequency and magnitude are commonly underestimated by clinically employed pulse oximeter settings. In extremely low birth weight infants the incidence of intermittent hypoxia [IH] progressively increases over the first 4 weeks of postnatal life, with a subsequent plateau followed by a slow decline beginning at weeks six to eight. Over this period of unstable respiratory control, increased oxygen-sensitive peripheral chemorecepto
