SHORT syndrome in an adult Brazilian patient.

We report an individual from Brazil with SHORT syndrome. The term SHORT stands for its common characteristics: short stature (S), hyperextensibility of joints, and/or inguinal hernia (H), ocular depression (O), Rieger anomaly (R), and teething delay (T). In addition to most of the clinical signs previously described in SHORT syndrome, the patient presented here also shows microcephaly and intellectual disability. Diagnosis was confirmed by exome sequencing revealing a novel heterozygous variant c.1456G>A (p.Ala486Thr) at PIK3R1. Human recombinant growth hormone (r-hGH) therapy was administered

Short syndrome-an expanding phenotype.

The phenotypic description of SHORT syndrome (OMIM- 269880) is expanding since its initial description in 1975. There have been 26 case reports till date but the genetic locus of this syndrome is elusive. Involvement of PITX2 gene initially envisaged is probably is not the only gene involved but has an important role to play in ocular development. Our case did not demonstrate mutation in PITX2 gene. Here, we report a case of SHORT syndrome with two new unreported features deviated nasal septum and cryptorchidism and stress on lipodystrophy, a cardinal feature but not a part of the pneumonic SH

HDR syndrome (hypoparathyroidism, sensorineural deafness and renal disease) accompanied by renal tubular acidosis and endocrine abnormalities.

Type 1 (distal) and type 2 (proximal) renal tubular acidosis (RTA) are uncommon disorders, particularly in adults. HDR syndrome (hypoparathyroidism, sensorineural deafness and renal disease) is an autosomal dominant condition, defined by the triad hypoparathyroidism, renal dysplasia and hearing loss. Here, we describe a 19-year-old man with HDR syndrome accompanied by renal tubular acidosis and endocrinopathic changes.

Nephrolithiasis during pregnancy secondary to primary hyperparathyroidism.

Nephrolithiasis secondary to primary hyperparathyroidism infrequently complicates pregnancy. It can cause severe maternal and fetal complications. We present a case of a pregnant woman with nephrolithiasis and primary hyperparathyroidism. We reviewed the management of nephrolithiasis due to primary hyperparathyroidism during pregnancy. We believe that early recognition and timely intervention can significantly reduce the incidence of complications.

Case Report: Renal sarcoidosis coexisting with membranous and IgA nephropathy, a very uncommon association.

INTRODUCTION: Sarcoidosis is a multisystemic inflammatory disease of unknown etiology with more frequent lung and lymph node involvement. Despite kidney involvement occurring between 25% and 43%, it may be underdiagnosed, with interstitial granulomatous nephritis and nephrocalcinosis being the most common features, and glomerular disease being less frequent. CASE REPORT: A 43-year-old man came to the emergency room (ER) because of macroscopic hematuria, arthralgias, pitting inferior extremities edema, asthenia, and dry cough without dyspnea. Evaluation revealed nephrotic range proteinuria and…

KCNJ16 tubulopathy presenting with recurrent acute flaccid paralysis and sensorineural hearing loss.

A 13-year-old girl, born to third-degree consanguineous parents, presented with acute flaccid paralysis. There was a past history of a similar episode at 5 years of age. Her current laboratory investigations revealed hypokalemia and normal anion gap-metabolic acidosis. Pure tone audiometry showed bilateral sensorineural hearing loss (SNHL). There was no nephrocalcinosis, ataxia, or epilepsy. Whole exome sequencing detected a homozygous deletion (c.269_270del) in exon 4 of KCNJ16 gene that led to frameshift and premature truncation of the protein (p. Ser90CysfsTer15; ENST00000392671.6). She was…

Hypercalcemia and Renal Affection: An Unusual Initial Presentation of Sarcoidosis.

Sarcoidosis is a chronic, multisystem, inflammatory granulomatous disease that affects multiple organs in the body, but mostly the lungs and the lymph glands. Hypercalcemia and renal affection are rarely initial presenting features, and in the absence of pulmonary symptoms, the diagnosis of sarcoidosis in those patients could be a diagnostic challenge. A case-patient with sarcoidosis presented with elevated serum calcium and creatinine levels. Renal biopsy showed nephrocalcinosis with chronic fibrosing interstitial nephritis. The extensive mediastinal, abdominal, axillar, and neck lymphadenopa…

[Erythema nodosum: a case report].

BACKGROUND: Erythema nodosum is the most common clinical-pathological variant of panniculitis, presenting with erythematous nodular lesions. Infections are the most common cause. However, other causes cannot be ruled out: immunological, inflammatory, or metabolic. Therefore, the diagnosis must be multidisciplinary and direct treatment toward the underlying cause, as in most cases, it is usually the tip of the iceberg of a significant pathological process. CASE REPORT: An 18-year-old female patient with a history of hepatoblastoma, nephrocalcinosis, and recurrent infections presented one month…

At 3 years of age, despite satisfactory growth, hypomagnesemia persisted and nephrocalcinosis appeared and progressed rapidly thereafter. Whole-genome analysis then revealed compound heterozygous mutations in the AGXT gene, thus leading to the diagnosis of PH1. CONCLUSION: Given the emergence of new targeted therapies, thorough genetic anal …

Hypertension in a patient with medullary sponge kidney: A case report.

RATIONALE: Medullary sponge kidney (MSK) is a congenital renal disorder characterized by recurrent nephrolithiasis or nephrocalcinosis. Recently, it has been found that MSK can be also combined with other diseases, such as primary aldosteronism and Beckwith-Wiedemann, but whether it is associated with secondary hypertension remains unknown. PATIENT CONCERNS: A 22-year-old hypertensive female presented to our hospital characterized by hypokalemia and hypertension. DIAGNOSIS: The laboratory examination showed secondary aldosteronism. And the common causes for secondary aldosteronism include re…

Medullary sponge kidney and Caroli's disease in a patient with stricture urethra: look for the hidden in presence of the apparent.

Caroli's disease is a rare congenital disorder with incidence rate of approximately 1 in 1 000 000 population. Renal anomalies which may be associated with Caroli's disease include medullary sponge kidney (MSK), cortical cysts, adult recessive polycystic kidney disease and rarely autosomal dominant polycystic kidney disease. Exact incidence of MSK in patients of Caroli's disease is not known. There are only a handful of reported cases of this association in literature. We hereby report a case of Caroli's disease with MSK with nephrocalcinosis. He presented to primary health centre with symptom…

Medullary sponge kidney: state of the art.

Medullary sponge kidney (MSK) is a kidney malformation that generally manifests with nephrocalcinosis and recurrent renal stones; other signs may be renal acidification and concentration defects, and pre-calyceal duct ectasias. MSK is generally considered a sporadic disorder, but an apparently autosomal dominant inheritance has also been observed. As MSK reveals abnormalities in both the lower and the upper nephron and is often associated with urinary tract developmental anomalies, its pathogenesis should probably be sought in one of the numerous steps characterizing renal morphogenesis. Given…
