Nerve structures at risk in the plantar side of the foot during anterior tibial tendon transfer: a cadaver study.

BACKGROUND: Anterior tibial tendon transfer is a common procedure for treatment of clubfoot recurrence. Fixation of the tendon usually includes passing the tendon through the lateral cuneiform. Drilling the bone and passing sutures through the plantar aspect of the foot may cause neurovascular damage. METHODS: Anterior tibial tendon transfer was performed through the lateral cuneiform in twelve cadaveric limbs. Drill holes were made perpendicular to the lateral cuneiform surface (group A), made perpendicular to the weight-bearing surface (group B), inclined 15° in the frontal and sagittal pla

Phrenic nerve transfer in the restoration of elbow flexion in brachial plexus avulsion injuries: how effective and safe is it?

OBJECTIVE: Phrenic nerve transfer has been used for treating lesions of the brachial plexus since 1970. Although, today, surgeons are more experienced with the technique, there are still widespread concerns about its effects on pulmonary function. This study was undertaken to evaluate the effectiveness and safety of this procedure. METHODS: Fourteen patients with complete palsy of the upper limb were submitted to phrenic nerve transfer as part of a strategy for surgical reconstruction of their plexuses. Two patients were lost to follow-up, and 2 patients were followed for less than 2 years. O…

Nerve transfers to the biceps and brachialis branches to improve elbow flexion strength after brachial plexus injuries.

OBJECT: In this study the authors evaluated the outcome in patients with brachial plexus injuries who underwent nerve transfers to the biceps and the brachialis branches of the musculocutaneous nerve. METHODS: The charts of eight patients who underwent an ulnar nerve fascicle transfer to the biceps branch of the musculocutaneous nerve and a separate transfer to the brachialis branch were retrospectively reviewed. Outcome was assessed using the Medical Research Council (MRC) grade to classify elbow flexion strength in conjunction with electromyography (EMG). The mean patient age was 26.4 years…

Results of nerve transfer techniques for restoration of shoulder and elbow function in the context of a meta-analysis of the English literature.

We report the results of 15 patients who underwent nerve transfer for restoration of shoulder and elbow function at our institution for traumatic brachial plexus palsy. We present these results in the context of a meta-analysis of the English literature, designed to quantitatively assess the efficacy of individual nerve transfers for restoration of elbow and shoulder function in a large number of patients. One thousand eighty-eight nerve transfers from 27 studies met the inclusion criteria of the analysis. Seventy-two percent of direct intercostal to musculocutaneous transfers (without interpo…

The Rise of Nerve Transfers: Tracing a National Evolution Toward the Postgrafting Era in Brachial Plexus Injury Management.

BACKGROUND: Nerve transfers have been heralded as a "quantum leap" in peripheral nerve surgery, fundamentally altering the reconstructive paradigm over the past 2 decades. This study analyzed national trends in adult traumatic brachial plexus injury (BPI) management, tracking specialty-specific adoption of nerve transfers and comparing 30-day postoperative outcomes with traditional reconstruction. METHODS: Data were extracted from the National Surgical Quality Improvement Program (2006 to 2022). Adult patients (18 years or older) with a primary BPI diagnosis, who underwent nerve reconstructio…

Exploring the Role of Fascia Lata Suspension in Triple Nerve Transfer for Facial Reanimation: A Comparative Outcomes Study.

BACKGROUND: Triple nerve transfer (TNT) combines cross-face nerve grafts, masseteric-to-facial nerve transfer, and hypoglossal-to-facial nerve transfer to restore facial function in long-standing facial paralysis. However, its efficacy in patients with severe soft tissue laxity or atrophy may be limited. This study evaluates whether adding autologous fascia lata suspension (TNTF) to TNT improves functional and psychosocial outcomes. METHODS: A retrospective study compared 22 patients with chronic unilateral facial paralysis, treated with either TNT alone (n=14) or TNT plus fascia lata suspens…

Comparative Efficacy of Masseteric-to-Facial Nerve Transfer with and without Fascia Lata Grafts in Reanimating Facial Paralysis.

BACKGROUND: Masseteric-to-facial nerve transfer excels in dynamic reanimation but is lacking in restoring baseline resting tone. Combining it with fascia lata grafts is expected to more fully enhance facial symmetry. Comprehensive comparison of the results of masseteric-to-facial nerve transfer versus combined static suspension may provide valuable insights into the most appropriate treatment for patients with subacute facial paralysis. METHODS: In this retrospective study, 104 patients with facial paralysis were categorized by surgical procedure: 39 received masseteric-to-facial nerve transf…

Transfer of the Motor Branch of the Abductor Digiti Quinti for Thenar Muscle Reinnervation in High Median Nerve Injuries.

PURPOSE: In high median nerve repairs, thenar muscle reinnervation is impossible because of the long distances over which axons must regenerate. To overcome this obstacle, we propose transferring the abductor digiti quinti motor branch (ADQMB) to the thenar branch of the median nerve (TBMN). METHODS: We used 10 embalmed hands for anatomical and histological studies. Thereafter, 5 patients with a high median nerve injury underwent surgical reconstruction within 8 months of their accident and were followed for at least 10 months after surgery (mean, 13.2 months). We transferred the ADQMB to the
