Ofatumumab-mediated CD20+ B-cell depletion promotes neuromuscular junction functional recovery and sustains long-term efficacy in refractory generalized myasthenia gravis.

OBJECTIVE: This study aimed to evaluate the long-term efficacy and safety of the fully human anti-CD20 monoclonal antibody ofatumumab in refractory generalized myasthenia gravis, with focus on its potential to promote recovery of neuromuscular junction transmission function. METHODS: Fourteen refractory patients treated at the Eighth Medical Center of the Chinese PLA General Hospital from March 2023 to June 2024 were included. All had relapsed after maintaining Minimal Manifestation Status for over 1 year and showed inadequate response to immunosuppressants. Ofatumumab 20 mg was administered

Curve Sprint Ability as an Indicator of Neuromuscular Function and Physical Fitness in Youth Soccer Players: Comparative Insights from U14 and U16 Groups.

Background and Objectives: In modern soccer, players frequently perform curved sprinting (CS) actions that combine speed, agility, and neuromuscular control, highlighting the importance of assessing CS performance in youth athletes. This study aimed to investigate age-related differences in CS performance and its relationship with key components of physical fitness-including aerobic capacity, vertical jump height, and agility-based field tests-in U14 and U16 male soccer players. Materials and Methods: A total of 76 registered players participated, evenly divided between the two age groups. Sta

Risk Factors for Postoperative Ileus after Scoliosis Surgery.

INTRODUCTION: One complication after scoliosis surgery is ileus; however, few reports have described the frequency of and risk factors for this complication. We conducted a retrospective clinical study with logistic regression analysis to confirm the frequency of and risk factors for ileus after scoliosis surgery. METHODS: After a retrospective review of data from patients who underwent surgical correction of spinal deformity from 2009 to 2014, 110 cases (age range, 4-73 yr; median, 14 yr) were included in the study. We defined postoperative ileus (POI) as a surgical complication characterize

Optimizing Drug Therapy in ECMO-Supported Critically Ill Adults: A Narrative Review and Clinical Guide.

Extracorporeal membrane oxygenation (ECMO) is increasingly used to support critically ill adults with severe cardiac or respiratory failure, but ECMO circuits and the physiological disturbances of critical illness significantly alter drug pharmacokinetics (PK) and pharmacodynamics (PD), complicating dosing and monitoring. This narrative review synthesizes current clinical evidence on ECMO-related PK/PD alterations and provides practical guidance for optimizing pharmacotherapy in adult intensive care. A structured literature search (January-May 2025) was conducted across PubMed/MEDLINE, EMBASE,

Visual classification of nocturnal transcutaneous CO2 and oximetry patterns in slowly progressive neuromuscular diseases: a derivation and feasibility study.

BACKGROUND: Sleep-disordered breathing is highly prevalent in subjects with neuromuscular diseases (NMD). We hypothesized that visual analysis of nocturnal transcutaneous CO2 (TcCO2) monitoring could help physicians identify distinct respiratory phenotypes. This study aimed to derive a visual classification of nocturnal transcutaneous CO2 and oximetry patterns in patients with slowly progressive neuromuscular diseases and to assess its feasibility and inter-rater agreement. METHODS: Retrospective study of 149 NMD subjects, naive to noninvasive ventilation, who underwent full-night TcCO2 monit

Effects of rocuronium dosage on intraoperative neurophysiological monitoring in patients undergoing spinal surgery.

WHAT IS KNOWN AND OBJECTIVE: Intraoperative neurophysiological monitoring (IONM) has been widely used in clinical practice. Therefore, the influence of neuromuscular blockers essential for spinal anaesthesia on IONM is worthy of our attention, but no randomized study has evaluated the dose-response effect. This study investigated the effects of different doses of rocuronium bromide on the intraoperative monitoring of motor evoked potentials (MEPs). METHODS: We conducted a randomized, double-blind trial to assess the effects of three rocuronium bromide doses (6.0, 9.0, 12 μg·kg-1 ·min-1 ) comb

Clinical relevance of chronic neuropathic pain phenotypes in mice: A comprehensive behavioral analysis.

Despite a large number of preclinical studies performed each year, the safe and effective therapeutic interventions for chronic pain are scant. Therefore, it appears that pre-clinical modeling requires a systematically organized behavioral test paradigm to quantify the response of animals for a specific pain state. The present study, therefore, conceptualized a test battery to evaluate the behavioral changes in mice following neuropathic pain. We employed sciatic nerve chronic constriction injury (CCI) in C57BL/6 J mice to model chronic pain state. Mice were monitored for thermal hyperalgesia

Post-ECT agitation and plasma lactate concentrations.

This prospective study evaluated the hypothesis that emergence agitation after electroconvulsive therapy (ECT) could be caused by lactate-induced panic secondary to insufficient neuromuscular blockade. Plasma lactate levels were measured before and after 245 consecutive ECT sessions in 37 patients monitored for evidence of post-ECT agitation. ECT was administered using a brief-pulse, rectangular, constant-current device through bilaterally placed electrodes under general anesthesia and neuromuscular blockade. Agitation was observed in 7% of all ECT sessions. No significant difference could be

Modern Dual Mobility Implants in Total Hip Arthroplasty: Stability, Survival, and Ion Safety Evaluation: The Comprehensive Review.

» Dual mobility cup (DMC) implants reduce dislocation rates in both primary and revision total hip arthroplasty (THA), particularly in high-risk patients with neuromuscular disorders, cognitive impairment, or obesity. » The dual articulation mechanism enhances joint stability by increasing jump distance and the head-to-neck ratio, without compromising range of motion. » Incorporation of highly cross-linked polyethylene and vitamin E-infused liners improves wear resistance, supporting broader use in younger, more active patients. » Long-term survivorship of modern DMCs is comparable or superior

Mycobacterium abscessus Pulmonary Disease in a Myasthenia Gravis Patient: A Case Report.

There is limited guidance on managing Mycobacterium abscessus pulmonary disease in Myasthenia Gravis patients. Macrolides and aminoglycosides form the backbone of M. abscessus pulmonary disease treatment but are avoided in Myasthenia Gravis patients due to the risk of precipitating myasthenic crisis. A 29-year-old female, with a history of anti-acetylcholine receptor antibody-positive Myasthenia Gravis and bronchiectasis, was diagnosed with M. abscessus pulmonary disease. She was commenced on a macrolide-based regime safely with close monitoring, pretreatment with intravenous immunoglobulin (1

Cardiac Troponin I and T in Checkpoint Inhibitor-associated Myositis and Myocarditis.

Checkpoint inhibitor-associated myocarditis (ir-myocarditis) and myositis (ir-myositis) may occur concurrently among patients on checkpoint inhibitor immunotherapy. While cardiac-specific troponin I (cTnI) and troponin T (cTnT) are regarded to have similar sensitivities and specificities in conditions such as acute coronary syndrome, the cardiac specificity of cTnT has been challenged following observation that patients with neuromuscular diseases, including myositis, may have elevated cTnT without apparent clinical evidence of myocardial injury. Consequently, in the context of concurrent ir-m

Effects of presynaptic muscarinic cholinoreceptor blockade on neuromuscular transmission as assessed by the train-of-four and the tetanic fade response to rocuronium.

This study investigated the effect of muscarinic M1 and M2 receptor antagonists on the rocuronium-induced train of four (TOF) fade and tetanic fade, respectively. Ex-vivo phrenic nerves and diaphragms were obtained from adult Sprague-Dawley rats and stabilized in Krebs buffer; the nerve-stimulated muscle TOF fade was observed at 20 s intervals. For the TOF study, phrenic nerves and diaphragms were incubated with pirenzepine (an M1 blocker) at concentrations of 0 nmol L-1 (control), 10 nmol L-1 (PZP10), or 100 nmol L-1 (PZP100). Rocuronium was then administered incrementally until the first twi

Use of BIS monitor during anaesthesia of a narcoleptic patient for avoiding possible delayed emergence.

Anaesthesia for a narcoleptic patient is not a common practice of anaesthesioligists' daily working life. Therefore special problems related to narcolepsy should be considered pre-, peroperatively and during emergence. The aim of presenting this case report is to emphasize the importance of BIS monitor use in a narcoleptic patient undergoing surgery under general anaesthesia to avoid possible prolonged emergence.

Combined recurrent laryngeal nerve monitoring and one-lung ventilation using the EZ-Blocker and an electromyographic endotracheal tube.

BACKGROUND: Intraoperative neuromuscular monitoring (IONM) is a widespread procedure to identify and protect the recurrent laryngeal nerve (RLN) during thyroid surgery. However, for left thoracic surgery with high risk of RLN injury, both reliable recurrent laryngeal nerve monitoring and one-lung ventilation could interfere. METHODS: In this prospective study, a new method for IONM during one-lung ventilation combining RLN monitoring with an electromyographic (EMG) endotracheal tube (ETT) and lung separation using the EZ-Blocker (EZB) is described and its clinical feasibility and effectivenes…

[Preoperative risk and perioperative management of obese patients].

The obese patient is at an increased risk of perioperative complications. Most importantly, these include difficult access to the airways (intubation, difficult or impossible ventilation), and post-extubation respiratory distress secondary to the development of atelectasis or obstruction of the airways, sometimes associated with the use of morphine derivatives. The association of obstructive sleep apnea syndrome (OSA) with obesity is very common, and induces a high risk of peri- and postoperative complications. Preoperative OSA screening is crucial in the obese patient, as well as its specific
