Resource utilization for non-operative cervical radiculopathy: Management by surgeons versus non-surgeons.

OBJECTIVES: To compare the estimated resource utilization for non-operative treatment of cervical radiculopathy if managed by surgeons versus non-surgeons. PATIENTS AND METHODS: A Cervical Spine Research Society-sponsored survey was administered at a national spine surgery conference to surgeons and non-surgeons, as classified above. The survey asked questions regarding resource utilization and perceived costs for the "average patient" with cervical radiculopathy managed non-operatively. Resource utilization and perceived costs were compared between surgeon and non-surgeon participants, and b

Assessment of Surgical Treatment Strategies for Moderate to Severe Cervical Spinal Deformity Reveals Marked Variation in Approaches, Osteotomies, and Fusion Levels.

OBJECTIVE: Although previous reports suggest that surgery can improve the pain and disability of cervical spinal deformity (CSD), techniques are not standardized. Our objective was to assess for consensus on recommended surgical plans for CSD treatment. METHODS: Eighteen CSD cases were assembled, including a clinical vignette, cervical imaging (radiography, computed tomography/magnetic resonance imaging), and full-length standing radiography. Fourteen deformity surgeons (10 orthopedic, 4 neurosurgery) were queried regarding recommended surgical plans. RESULTS: There was marked variation in t

Crouzon syndrome: clinico-radiological illustration of a case.

Crouzon syndrome, also called craniofacial dysostosis, is an autosomal dominant disorder with complete penetrance and variable expressivity. Described by a French neurosurgeon in 1912, it is a rare genetic disorder characterized by premature closure of cranial sutures, midfacial hypoplasia, and orbital defects. Here, we report a case of this rare entity. The patient presented with brachycephaly, maxillary hypoplasia, exophthalmos, mandibular prognathism, along with dental and orbital abnormalities.

The role of transcranial Doppler ultrasonography in the diagnosis and management of vasospasm after aneurysmal subarachnoid hemorrhage.

Transcranial Doppler ultrasonography (TCD) is a tool employed by the neurosurgeon and neurointensivist in the management of vasospasm in the intensive care unit after aneurysmal subarachnoid hemorrhage. A review of the current indications, monitoring parameters, indices, and relevance of modern TCD technology is provided, as well as algorithms for the use of TCD ultrasonography in the management of patients with subarachnoid hemorrhage. Other current uses of TCD ultrasonography are also discussed in the setting of neurocritical care.

Pituitary surgery for Cushing's disease.

BACKGROUND: Surgery is currently the first-line treatment of Cushing's disease. Surgery for Cushing's patients requires technical specificity, especially if no adenoma is identified on dedicated preoperative pituitary MRI. METHOD: From 2006 to 2020, 683 patients with Cushing's disease were operated on with a mononostril endoscopic endonasal approach by the same two senior neurosurgeons. Here, we report the particularities of this challenging surgery. CONCLUSION: A rigorous and planned surgical strategy avoids the pitfalls of Cushing's disease surgery and leads to a high rate of endocrine rem…

Imaging methods for surgical revascularization in patients with moyamoya disease: an updated review.

Neuroimaging is crucial in moyamoya disease (MMD) for neurosurgeons, during pre-surgical planning and intraoperative navigation not only to maximize the success rate of surgery, but also to minimize postsurgical neurological deficits in patients. This is a review of recent literatures which updates the clinical use of imaging methods in the morphological and hemodynamic assessment of surgical revascularization in patients with MMD. We aimed to assist surgeons in assessing the status of moyamoya vessels, selecting bypass arteries, and monitoring postoperative cerebral perfusion through the late…

[Pneumocephalus as a cause of a decrease in the bispectral index].

A sharp decrease in the values of the bispectral index (BIS), along with an increase in suppression rate, was observed in a patient after the removal of an epidermoid tumor in the cerebellopontine angle by right retrosigmoid access under general anesthesia. This was probably related to a frontal pneumocephalus. No accompanying neurological signs were observed. The patient was extubated in the Recovery Room with no further incidents, as the BIS increased again. The neurosurgeons chose conservative treatment, relying on the reabsorption and redistribution of the air.

Imaging the pituitary and parasellar region.

Magnetic resonance imaging is the fundamental imaging tool for the evaluation of tumors and other lesions of the pituitary gland and infundibulum. Abnormalities may arise within the pituitary itself, from vestigial embryologic remnants, or from surrounding tissues. Correct diagnosis rests on accurate assessment of lesion location, imaging appearance, and clinical presentation. This article reviews the radiologic evaluation of lesions within the sella and suprasellar cistern, focusing on common masses and pseudomasses of the pituitary and sellar region that neurosurgeons are most likely to enco…

Bilateral origin of superior cerebellar arteries from the posterior cerebral arteries, and clues to its embryologic basis.

The superior cerebellar artery is the most consistent branch of the basilar artery and arises near the bifurcation of the basilar artery. A bilateral origin of the superior cerebellar arteries from the posterior cerebral arteries has been rarely reported in the literature. Reporting variations in brain vessels is important for neurosurgeons to safely and confidently treat pathologies in this region. We report on a specimen with a bilateral origin to the superior cerebellar artery from the posterior cerebral artery and discuss the embryogenesis of this rare variation.

Traumatic false aneurysm and carotid-cavernous fistula: a complication of sphenoidotomy.

Severe head injury with resultant basilar skull fracture is the most common etiology for traumatic false aneurysm of the cavernous carotid artery and for traumatic carotid-cavernous fistula. We present a case of false aneurysm and fistula that occurred as a complication of sphenoidotomy, fortunately a rare etiology of these two serious problems. Because of the current resurgence of trans-sphenoidal surgery, it is of utmost importance that all neurosurgeons possess an accurate and detailed knowledge of the anatomy of the carotid artery and its intracavernous route.

Use of an inflatable balloon on a catheter to prevent subdural fluid collections after intraventricular tumor surgery: technical note.

Ventricular tumor surgeries pose significant challenges to the neurosurgeon. Apart from the operative challenges and typical complications of surgery, subdural fluid collection is common after these surgeries. Various methods have been used to prevent this complication. The authors' technique of implanting an inflatable balloon attached to a catheter after resection of a huge intraventricular tumor was highly effective in preventing the development of a subdural fluid collection.

Improving health care transition and longitudinal care for adolescents and young adults with hydrocephalus: report from the Hydrocephalus Association Transition Summit.

The health care needs of children with hydrocephalus continue beyond childhood and adolescence; however, pediatric hospitals and pediatric neurosurgeons are often unable to provide them care after they become adults. Each year in the US, an estimated 5000-6000 adolescents and young adults (collectively, youth) with hydrocephalus must move to the adult health care system, a process known as health care transition (HCT), for which many are not prepared. Many discover that they cannot find neurosurgeons to care for them. A significant gap in health care services exists for young adults with hydro…

Pattern of cerebral aneurysms in Morocco: review of the concept of their rarity in developing countries: report of 200 cases.

OBJECTIVE: Many neurosurgeons consider cerebral aneurysms to be rare in Africa and the Middle East. In this report, we describe the pattern of cerebral aneurysms in Morocco and call into question the idea of their rarity in developing countries. Our objective is to urge neurosurgeons in these areas to track them and to treat them under better conditions. METHODS: We report a retrospective study of 200 patients with cerebral aneurysms admitted to our department between 1983 and 1999. The results of this study are supported by pertinent epidemiological surveys, anatomic studies on the incidence…

Educational Utility of a Full-Skull 3-Dimensional Model for Skull Base Surgery Training.

BACKGROUND AND OBJECTIVES: Skull base surgery requires precise 3-dimensional (3D) anatomic understanding and advanced drilling techniques within a confined operative space. Although cadaveric dissection remains the gold standard for training, its availability is limited. This study aimed to evaluate the educational utility of a commercially available full-skull 3D model for skull base surgery training. METHODS: A full-skull 3D model reproducing the cranial base and brain was used in a hands-on training session involving 8 neurosurgeons, including 2 residents and 6 board-certified surgeons. Mu…

Advancing access to emergency care for traumatic brain injuries in Indonesia: a national program integrating task-sharing and simulation technologies.

Severe shortages and geographic maldistribution of neurosurgeons continue to limit timely access to life-saving neurosurgical care in low- and middle-income countries, with Indonesia representing a particularly acute example due to its archipelagic geography, prolonged transfer times, and rising burden of traumatic brain injury and stroke. To address delays in emergency care in districts without neurosurgical coverage, Indonesia has launched a nationally endorsed neurosurgical task-sharing initiative in which selected general surgeons are trained to perform a restricted set of emergency crania…
