[Eye movement abnormalities as a sign for the diagnosis in Niemann-Pick disease type C].

BACKGROUND: Eye movement abnormalities in familial mental retardation syndrome should lead to the suspicion of a storage disorder, including Niemann Pick disease type C, Gaucher's disease, abetalipoproteinemia and Wilson's disease. The eye movement abnormalities in our two patients were suggestive of Niemann Pick disease type C, characterized by initial loss of voluntary vertical eye movements and subsequent loss of horizontal eye movements, with preservation of the vestibulo-ocular response. The characteristics of eye movements in storage disorders are different. In Gaucher's disease a progre

Electrophysiological observations in idiopathic opsoclonus-myoclonus syndrome.

To supplement existing knowledge regarding the pathophysiology of the opsoclonus-myoclonus syndrome, electrophysiological findings are reported in three patients with idiopathic opsoclonus-myoclonus. Surface electromyography (EMG) revealed < 100-ms synchronous discharges correlating with the clinical myoclonus. Short duration EMG discharges, with no back-averaged cortical correlate, normal gross electroencephalogram, and no exaggerated responses with either evoked potential testing or long latency EMG responses were observed. The clinical and electrophysiological findings we describe are consi

Opsoclonus-myoclonus syndrome with abnormal single photon emission computed tomography imaging.

The single photon emission computed tomography (SPECT) findings in 2 patients with opsoclonus-myoclonus syndrome (OMS) who had similar symptoms in the acute stage of the disease are described. In 1 patient with encephalitis, SPECT showed increased blood flow in most of the cerebellum; the highest accumulation of the radiotracer was found in the cerebellar vermis and neighboring structures. These changes disappeared with clinical improvement. In the other patient with chronic OMS, SPECT demonstrated hypoperfusion in most of the cerebellum. Our results suggest that the cerebellum, particularly i

Eye movement abnormalities in a case of Tourette syndrome.

Tourette syndrome (TS) is a neuropsychiatric disorder that is characterised by the presence of multiple vocal, facial, and motor tics which change with time, and a number of other behavioural phenomena. Previous studies have not revealed any ocular-motor abnormalities. We report the eye movement studies of a patient with TS, using electrooculography and simultaneous video recording. Intrusive saccades occurred during smooth pursuit and optokinetic nystagmus. Reflexive and voluntary saccades were dysmetric and there was a complete failure of antisaccades. These abnormalities are characteristic

Presentation of Bilateral Midbrain Infarction as Wall-eyed Bilateral Internuclear Ophthalmoplegia: A Case Report.

This article describes the case of a 68-year-old woman presenting with acute dizziness and double vision, diagnosed with wall-eyed bilateral internuclear ophthalmoplegia (WEBINO). She experienced unprovoked, nonpositional dizziness, and binocular double vision that improved with eye closure. Neurological examination showed right eye exotropia, left eye hypertropia, mild right eyelid ptosis, and impaired extraocular movements, especially in elevation and adduction. Diagnostic tests including blood work and cerebrospinal fluid analysis were largely normal, but brain magnetic resonance imagingind

Advances in AI-Driven EEG Analysis for Neurological and Oculomotor Disorders: A Systematic Review.

Electroencephalography (EEG) has emerged as a powerful, non-invasive modality for investigating neurological and oculomotor disorders, particularly when combined with advances in artificial intelligence (AI). This systematic review examines recent progress in machine learning (ML) and deep learning (DL) techniques applied to EEG-based analysis for the diagnosis, classification, and monitoring of neurological conditions, including oculomotor-related disorders. Following the PRISMA guidelines, a structured literature search was conducted across major scientific databases, resulting in the inclus

A virtual approach: Systematic review and meta-analysis of virtual reality-based therapies for convergence insufficiency.

Convergence insufficiency (CI) is a common binocular vision disorder that significantly impacts visual comfort and quality of life. It has been hypothesized that CI therapy can also be provided effectively with Virtual reality (VR). Recently, a few studies were conducted on virtual reality-based CI therapy. However, to the best of the authors' knowledge, no systematic review and meta-analysis has been performed on the effectiveness of VR-based CI therapy. Therefore, this study aims to fill this critical gap and investigate the effectiveness of VR-based CI therapy. Consequently, it may be helpf

Comparing myopic error in patients with basic and convergence insufficiency intermittent exotropia in China.

PURPOSE: To compare the degree of myopia between the dominant and non-dominant eyes in teenagers with intermittent exotropia (IXT) in China. METHODS: A total of 199 IXT patients with myopia were included in this retrospective study and were divided into two groups according to the difference between near and distance exodeviation: basic IXT and convergence insufficiency (CI) IXT. Refractive errors were analyzed by spherical equivalent (SE) values. Patients were further stratified into anisometropia group and non-anisometropia group based on binocular SE values difference greater than 1.0D or

Sensitivity of Magnetic Resonance Imaging of the Medial Longitudinal Fasciculus in Internuclear Ophthalmoplegia.

BACKGROUND: Internuclear ophthalmoplegia (INO) is a result of insult to the medial longitudinal fasciculus (MLF). Clinicoradiological correlation in patients with INO has been reported to be poor; however, prior studies have used low resolution MRI imaging techniques and included patients with subclinical INO. We aimed to determine the sensitivity of modern MRI interpreted by a specialist neuroradiologist to detect clinically evident INO. METHODS: A retrospective chart review of patients in 2 tertiary University-affiliated neuro-ophthalmology practices with the diagnosis of INO. MRI scans of

Acquired Diplopia in Adults: Heavy Eye Syndrome.

Heavy eye syndrome is a condition that results in an acquired, progressive strabismus that is due to extreme myopia and long axial length. The underlying pathogenesis of the condition is due to prolapse of the supero-temporal aspect of the myopic globe causing displacement of the lateral and superior rectus muscles inferiorly and nasally, respectively. Treatments for heavy eye syndrome often target this anatomical defect and seek to re-place the globe within the extraocular muscle cone. This review will discuss current theories of pathogenesis and treatment of heavy eye syndrome.

COVID-19 provoked internuclear ophthalmoplegia in a child with arrested hydrocephalus.

Internuclear ophthalmoplegia (INO) is a neuro-ophthalmic disorder caused by damage in the medial longitudinal fasciculus between the third and sixth cranial nerve nuclei. We present a 4-year-old female diagnosed with INO triggered by coronavirus disease 2019 (COVID-19) infection. The patient had history of neonatal meningitis with hydrocephalus without history of surgical intervention. To the best of our knowledge, this is the first case with combined COVID-19 and chronic hydrocephalus as an etiology for INO in a child. COVID-19 may trigger neurological manifestations as INO in susceptible cas
