Clinical and radiological outcomes of bimalleolar ankle fractures treated with open reduction and internal fixation: a prospective observational study.

PURPOSE: Bimalleolar ankle fractures are common injuries that typically result from trauma such as falls or road traffic accidents. Open reduction and internal fixation (ORIF) is the standard treatment and aims to restore proper alignment, stability, and ankle function. Despite the overall effectiveness of ORIF, patient outcomes may vary depending on age, sex, fracture type, and the timing of surgery. This study evaluates the clinical, radiological, and functional outcomes of bimalleolar ankle fractures treated with ORIF and identifies factors influencing recovery and complications. METHODS:

Impact of surgical timing on perioperative outcomes after open reduction and internal fixation of periprosthetic proximal femoral fractures.

BACKGROUND: Periprosthetic proximal femoral fractures (PPFF) are expected to increase substantially due to demographic changes and the rising number of total hip arthroplasties. While early surgery is clearly recommended for native hip fractures, the optimal timing of surgery for PPFF remains controversial. The present study aimed to evaluate the effect of early versus delayed surgery on perioperative outcomes in patients with PPFF treated by open reduction and internal fixation (ORIF). METHODS: We conducted a retrospective analysis of 110 patients with PPFF who underwent ORIF at a Level 1 tr

Impact of Preoperative Heel Ulceration on Surgical Outcomes Following Open Reduction and Internal Fixation of Rotational Ankle Fractures.

BACKGROUND: As surgical treatment with open reduction and internal fixation (ORIF) becomes more common, attention to preoperative risk stratification has grown. Although multiple modifiable risk factors and comorbidities have been associated with adverse outcomes, the impact of preexisting decubitus heel ulcers in this context remains yet to be defined. METHODS: Using a nationally representative multi-institutional database, a retrospective analysis was conducted on adults undergoing ORIF for rotational ankle fractures between 2010 and 2022. Patients with a documented decubitus heel ulcer wit

Outcomes of Metacarpal Shaft Fractures Treated by Open Reduction Internal Fixation With Plates and Screws Versus Intramedullary Screws.

BACKGROUND: Metacarpal fractures are among the most common orthopedic injuries. When metacarpal shaft fractures require surgical fixation, common options include closed reduction percutaneous pinning, open reduction internal fixation (ORIF) using plates and screws (PS), and intramedullary screw fixation (IMS). Certain metacarpal shaft fractures are amenable to ORIF with either PS or IMS. The purpose of this study is to compare the functional outcomes and complication rates between these techniques. METHODS: A retrospective review was performed of all patients that underwent ORIF with PS or IM

Comparative efficacy of tunnel minimally invasive technique versus traditional open reduction and internal fixation for rib fractures.

BACKGROUND: Rib fractures are often treated conservatively, but for cases with significant displacement or instability, surgical intervention may be necessary. Traditional open reduction and internal fixation (ORIF) has been the standard surgical approach; however, the tunnel minimally invasive technique is emerging as a less invasive alternative. This study compares the efficacy of these two surgical methods in terms of incision length, postoperative pain, surgical time, and hospital stay. METHODS: A retrospective evaluation was conducted from June 2022 to December 2024 at our hospital, incl

Anesthetic management for open reduction and internal fixation of the mandibular fracture in a patient with congenital pulmonary hypoplasia: a case report.

BACKGROUND: Congenital pulmonary hypoplasia is an exceptionally rare developmental disorder that is usually diagnosed in neonates and seldom encountered in adult.Consequently, evidence-based perioperative strategies for adult CPH patients undergoing non-thoracic surgery remain virtually absent. We present a case of an adult with CPH who required general anesthesia for open reduction and internal fixation of the mandibular fracture, and we describe a comprehensive perioperative management plan centered on lung-protective ventilation strategy and multimodal analgesia. Does local fat thickness correlate with post-operative infection in open reduction and internal fixation of acetabulum fractures?

PURPOSE: Obesity is an epidemic which increases risk of many surgical procedures. Previous studies in spine and hip arthroplasty have shown that fat thickness measured on preoperative imaging may be as or more reliable in assessment of risk of post-operative infection and/or wound complications than body mass index (BMI). We hypothesized that, similarly, increased local fat thickness at the surgical site is a predictor of wound complication in acetabulum fracture surgery. METHODS: Patients who underwent open reduction and internal fixation (ORIF) of an acetabulum fracture through a Kocher-Lan

Does a Short Period of Maxillomandibular Fixation Decrease Complications in Open Reduction Internal Fixation of Mandibular Angle Fractures?

PURPOSE: A short duration of postoperative maxillomandibular fixation (MMF) has the potential to reduce complications following open reduction and internal fixation (ORIF) of mandibular angle fractures. The purpose of this study was to determine if a short duration of MMF is associated with a reduced rate of postoperative inflammatory complications (POICs) in patients with mandibular angle fractures undergoing ORIF. METHODS: The authors conducted a retrospective cohort study consisting of patients treated with ORIF for mandibular angle fractures from August 1, 2015 to May 31, 2020 at an urban

Preoperative Anemia Is Associated With Worse Postoperative Outcomes After Open Reduction Internal Fixation of Distal Radius Fractures.

BACKGROUND: Preoperative anemia has been shown to be associated with complications after numerous orthopedic procedures. No studies to our knowledge have examined its impact on outcomes after open reduction internal fixation (ORIF) of distal radius fracture (DRF). We hypothesized that patients with preoperative anemia would have increased likelihood of adverse outcomes, and likelihood would increase with severity of anemia. METHODS: A total of 14 136 patients underwent ORIF for DRF over 2012-2021, identified in the National Surgical Quality Improvement Program database. Patients were classifi

The radiographic soft tissue thickness is associated with wound complications after open reduction and internal fixation of patella fractures.

BACKGROUND: Displaced patella fracture is commonly treated with open reduction and internal fixation (ORIF). Wound complications after surgery often lead to prolonged hospitalization and dissatisfaction of patients. Periarticular adiposity and swelling may be associated with wound complications. The purpose of this study is to determine the relationship between periarticular soft tissue thickness and wound complications after ORIF of patella fractures. METHODS: We retrospectively studied 237 patients undergoing ORIF for patella fractures from June 2017 to February 2021 at our institution. We

Which Surgical Approach Provides Maximum Visualization and Access for Open Reduction and Internal Fixation of Femoral Head Fractures?

OBJECTIVES: Femoral head fracture open reduction and internal fixation (ORIF) requires adequate surgical access and visualization. The purpose of this study was to objectively characterize femoral head access associated with commonly used surgical approaches. Our hypothesis was that a surgical hip dislocation (SHD) provides the greatest visualization and access to the femoral head. METHODS: Ten fresh-frozen cadaveric whole-pelvis specimens (n = 20 hips) were used to compare 4 surgical approaches to the femoral head (n = 5 hips per approach): SHD, Smith-Petersen (with and without rectus releas
