Orofacial infections in Iraq.

In Iraq, oral and maxillofacial (OMF) infections are common; these infections may be of odontogenic or nonodontogenic origin. The former, the commoner, gains importance from the catastrophic consequences that may take place in neglected or mismanaged patients and, as a paradox, from a fact that most of these infections are avoidable and preventable if a timely and accurate management took place for the primary dental problem. Nonodontogenic infections are not uncommon and are of life-threatening potential. Most of these infections are associated with an underlying medical condition that, whene

Intradural Extramedullary Metastatic Conjunctival Malignant Melanoma.

BACKGROUND: Intradural extramedullary (IDEM) metastatic spine tumors of nonneurogenic origin, excluding leptomeningeal seeding, are very rare. Most patients with IDEM metastatic spine tumors of nonneurogenic origin have coexisting brain metastasis. We report a case of IDEM metastatic malignant melanoma without antecedent or coexisting brain metastasis. CASE DESCRIPTION: A 52-year-old man presented with back pain, numbness in the lower extremities, paraparesis, and residual urine sensation after voiding. The patient had a history of conjunctival melanoma in the left eye and underwent surgical

Imaging of perineal suppurations: a pictorial essay.

Perineal suppuration encompasses a broad spectrum of pathologies involving the anatomically complex perineal region. While anal fistula, with or without abscess, is the most frequent cause, infections may also originate from the skin, accessory urinary or genital glands, pelvic and perineal implants, or supralevator digestive structures. Clinical presentations range from benign acute episodes to severe life-threatening conditions or chronic relapsing infections. Diagnosis is often challenging due to overlapping symptoms and variable anatomical involvement. Imaging plays a pivotal role in delin

Iatrogenic injury to the subclavian artery during central venous access.

Subclavian artery injury during central venous line placement is a potentially life-threatening complication. Due to historically high mortality rates with attempted manual pressure for haemostasis, the current favoured method of repair is by open or endovascular means. This case describes the management of a central venous catheter placed through the internal jugular vein into the subclavian artery via a supraclavicular puncture. CT angiography revealed the arterial injury located 1 cm distal to the vertebral artery origin. Under fluoroscopic guidance in an endovascular hybrid theatre, a cove

Evidence for defective energy homeostasis in amyotrophic lateral sclerosis: benefit of a high-energy diet in a transgenic mouse model.

Amyotrophic lateral sclerosis (ALS) is an adult-onset neurodegenerative disease characterized by selective loss of motor neurons and progressive muscle wasting. Growing evidence indicates that mitochondrial dysfunction, not only occurring in motor neurons but also in skeletal muscle, may play a crucial role in the pathogenesis. In this regard, the life expectancy of the ALS G93A mouse line is extended by creatine, an intracellular energy shuttle that ameliorates muscle function. Moreover, a population of patients with sporadic ALS exhibits a generalized hypermetabolic state of as yet unknown o

Risk factors for the development of allergic disease: analysis of 2,190 patient records.

Records of 2,190 patients with demonstrable allergic disease, including 1,148 patients with asthma and 557 patients with allergic rhinitis, were analyzed for the impact of certain risk factors on age of onset of allergic disease and on sensitization to certain specific antigens. The risk factors examined included race, sex, maternal age, family structure, life style, feeding history, environmental factors and heredity of allergic disease. Age of onset was significantly earlier (P less than 0.001) in males, blacks, children of younger mothers, bottle-fed babies, children of rural origin, childr

[Severe, thromboembolic pulmonary hypertension with recurrent pulmonary embolism and right heart thrombi in a patient with past myocardial infarction, cerebral ischaemic stroke and small intestine necrosis].

Chronic thromboembolic pulmonary hypertension (CTEPH) is a chronic progressive disease of pulmonary circulation characterised by indistinct ethiopathogenesis. We present a case of a 50 year-old male with thrombophilia of unknown origin leading to the formation of multiple thrombi within venous circulation followed by episodes of acute pulmonary embolism resulting ultimately in acute heart failure in the course of developing CTEPH. Unfortunately, despite the wide range of haemostasis laboratory tests we were not able to define the type of coagulation abnormality. Owing to the efficient cooperat

Hodgkin's disease in an elderly patient with B-cell chronic lymphocytic leukemia.

Chronic lymphocytic leukemia (CLL) is the most common type of leukemia worldwide. It is an indolent disease, almost exclusively of B-cell origin. Some CLLs evolve into a more aggressive lymphoid malignancy. The most common of these is Richter's syndrome. Transformation to acute lymphoblastic leukemia, plasma cell leukemia, multiple myeloma, or Hodgkin's disease (HD) may also occur. CLL patients are also at a significantly increased risk of developing a second malignant neoplasm later in life. One of the most common of these is HD. Herein, we report a case of HD in an elderly man with a history

Cardiac angiosarcoma arising from pulmonary artery: endovascular treatment.

We report a case of unusual origin of cardiac angiosarcoma rising from the pulmonary trunk. The tumor caused severe obstruction of the right ventricular outflow tract with serious symptoms of right ventricular failure and tricuspid insufficiency. The angiosarcoma was surgically unresectable because of infiltration of the pulmonary artery and cardiac structures. To relieve the patient's symptoms we chose an endovascular stent treatment to dilate the right ventricular outflow tract obstruction. Consequently this palliative treatment changed the short-term prognosis of the patient, improving his

An incidental diagnosis of a giant paraovarian cyst in a female teenager: A case report.

RATIONALE: Paraovarian cysts (PCs) are cystic tumors that can be encountered between the ovarian hilum and the ovarian fimbria located within the mesosalpinx and broad ligament, being usually diagnosed within the 3rd and 4th decade of life. PATIENT CONCERNS: We report the case of a 15-year-old female admitted in our clinic for consciousness loss, who was incidentally diagnosed with a giant pelvic cystic at ultrasound. DIAGNOSES: The magnetic resonance image showed a cystic mass of 170/140/85 mm, suggesting an origin from the left ovary, reaching the subhepatic area. INTERVENTIONS AND OUTCOM

[Management of stridor].

Pediatric stridor is usually due to benign laryngomalacia. This pathology of unknown origin usually heals spontaneously during the first 18 months of life. Mild laryngomalacia is characterized by normal weight growth, the absence of sleep apneas and of associated swallowing or neurological disorders. Medical treatment is sufficient to release symptoms. Severe clinical forms are more rare, representing about 10 % of cases. They require an endoscopic evaluation of the upper airway under general anesthesia, as well as an endoscopic suppraglottoplasty. Non-invasive ventilation is an interesting ad

Lichen sclerosus in the oral mucosa: a rare form of presentation.

Lichen sclerosus is a chronic inflammatory disease of unknown origin, which affects mostly women in the fifth and sixth decades of life, but can also occur in men and children. The involvement of the oral mucosa alone or together with other forms of presentation is extremely rare, requiring a differential diagnosis with other diseases of the oral cavity, particularly lichen planus. There are less than 30 cases of lichen sclerosus in the oral mucosa described in the literature and there are no reports on malignant transformation so far. We describe a patient with skin, oral and genital lesions

Anomalous origin of left pulmonary artery from the aorta with partial anomalous pulmonary venous return.

Anomalous origin of left pulmonary artery (AOLPA) from the aorta is a rare congenital cardiac malformation. We report a case of AOLPA from the aorta with partial anomalous pulmonary venous return (PAPVR) in a 3.2 kg infant. Surgical correction was performed with direct anastomosis of AOLPA to the main pulmonary artery with subsequent baffling of the anomalous right pulmonary veins to the left atrium and patch augmentation of the left pulmonary artery performed at 1-month of life. Recurrent stenosis of the proximal left pulmonary artery was treated with stent placement at 17-months. Pulmonary h

[Septic arthritis as an initial manifestation of a bacterial endocarditis].

Septic arthritis due to endocarditis is a rare and life-threatening disease. Endocarditis occurs with an incidence of 30 patients per 1 million citizens/year. Staphylococcus aureus is one of the most common causative pathogens. Methicillin-resistant Staphylococcus aureus (MRSA) can lead to a severe outcome with a high mortality rate, and embolic complications of the kidney, brain, and spleen are seen in one third of all cases. The diagnosis and treatment of endocarditis is a challenge for all health care providers. We report about a patient who was admitted to our hospital with generalized sep

Ameloblastic fibroma of the maxillary sinus.

Ameloblastic fibroma is a rare true mixed tumor of odontogenic origin with both mesenchymal and ectodermal components. It usually arises from the mandibular dentition and presents in the second decade of life. It is a benign slow growing tumor that is less infiltrative than an ameloblastoma but tends to expand bone. It infrequently involves the maxilla and hence the management of these tumors in that region is controversial. The goal of therapy is to avoid cosmetic deformity without compromising on tumor eradication. We present the tenth reported case of this type of tumor in the maxilla, and

Idiopathic sudden sensorineural hearing loss and postnatal viral labyrinthitis: a statistical comparison of temporal bone findings.

Although the cause of idiopathic sudden sensorineural hearing loss remains uncertain, a viral origin has been suggested in many cases on the basis of anamnestic microbiologic and pathologic data. Twenty-two temporal bone specimens from 18 patients who during life suffered a sudden partial or complete sensorineural hearing loss were studied. On the basis of clinical data, these cases were assigned to one of three diagnostic categories, and the temporal bones were studied by light microscopy and serial section analysis. The implications of the histopathologic findings for the pathogenesis of idi

[Diabetic Foot Status (DFS): A IWGDF-based classification for the assessment and management of diabetic foot disease in primary care].

Diabetic foot disease is a complication of diabetes mellitus, impacting morbidity, quality of life, and healthcare costs. Its clinical assessment is fragmented across multiple classification systems (SINBAD, IDSA), hindering multidisciplinary communication. The Diabetic Foot Status (DFS) is a new clinical classification based on the latest guidelines from the International Working Group on Diabetic Foot (IWGDF). It facilitates the classification of diabetic foot into three states: «Green,» «Amber,» and «Red,» also incorporating the degree of infection and the predominant origin of the lesion:

Severe interscapular pain and increased creatine kinase activity: the answer was in the ankles.

Severe thoracic back pain with increased creatine kinase activity is a clinical presentation that suggests a variety of life threatening conditions. If initial examination is unrevealing, multiple diagnostic tests are usually performed attempting to identify the origin of the problem, sometimes neglecting apparently unrelated subtle physical findings. A patient is described in whom this was the initial presentation of a sensory demyelinating neuropathy, resulting in a diagnostic challenge. This case expands the differential diagnosis of severe thoracic back pain and increased creatine kinase a

Homozygous mutation (A228T) in the 5alpha-reductase type 2 gene in a boy with 5alpha-reductase deficiency: genotype-phenotype correlations.

The molecular basis of a patient with 5alpha-reductase deficiency was investigated in this study. This disease is a rare form of male pseudohermaphroditism with virilization during puberty. The child was raised as a girl, but had a male gender identity early in life. The diagnosis was set at the age of 13 years when the virilization process began. Hypospadias repair was performed and he changed to a male gender. DNA sequence analysis disclosed a homozygous mutation in exon 4 of the 5alpha-reductase type 2 gene, alanine 228 for threonine. The heterozygous parents are first cousins of Pakistani

Use of ultrasonography in dysplasia of the immature hip.

Real-time ultrasonography reconstruction is an imaging technique of relatively recent origin that has dramatically affected the diagnosis and treatment of patients with hip dysplasia. Ultrasonography is replacing conventional radiography as the primary method of diagnosing hip dysplasia and evaluating its treatment during the first six to nine months of life. While conventional radiography reveals a familiar two-dimensional image of ossified structures and to a much lesser extent, soft-tissue structures, it has disadvantages that include ionizing radiation, a relative inability to evaluate uno
