Non-operative management of acute upper cervical injuries.

The results of a closed, non-operative type of treatment in acute upper cervical injury have been reviewed, especially as they relate to the use of the halo cast or vest. The morbidity with the use of this device has been minimal and there has been no mortality. Early patient mobilization has been readily achieved and in the case of the Hangman's, Jefferson's, and odontoid fractures, stable solid bony healing has been achieved in nearly all cases. Similar results have been noted in the C-1 arch fracture, as well as the acute post-infectious subluxations of C-1/C-2. In a small number of cases i

The Challenges of Monoaxial Bone Transport in Orthopedics and Traumatology.

BACKGROUND: Bone defects represent the main challenging problem for the orthopedic surgeon and, consequently, they increase the duration of hospitalization, risk of complications and health expenditures. The aim of our observational, descriptive and retrospective study is to evaluate the outcomes of patients treated with a mo-nolateral external fixator for bone defects greater than 3 cm. MATERIAL AND METHODS: Between January 2003 and January 2013, 21 patients were treated at our center by bone transfer with a monolateral external fixator. The main etiologies were trauma in 17 cases (80.9%) an

A minimally invasive hybrid procedure to correct pectus arcuatum.

BACKGROUND: Pectus arcuatum, also known as horns of steer anomaly or Currarino-Silverman Syndrome, is a distinct chest wall anomaly characterized by severe manubriosternal angulation, a shortened sternum, and mild pectus excavatum. The anomaly is typically repaired using open techniques, employing orthopedic fixation devices. Here, we report the results of a minimally invasive hybrid procedure to repair pectus arcuatum. METHODS: The procedure combines a standard Nuss procedure to correct the depressed sternum with a short upper chest (in boys) or inter-mammary (in girls) incision for bilatera…

Plasma-Induced Amine-Modified PEEK Promotes Osteointegration via Multiple Osteogenic Pathways and Macrophage Modulation.

Polyetheretherketone (PEEK) is a semicrystalline synthetic polymer commonly used in orthopedic devices because of its chemical stability, thermal elasticity, radiopacity, and mechanical moduli similar to that of bone. However, its inherent biological inertness results in poor initial fixation and insufficient bone bonding, which can compromise implant stability and long-term success. To overcome this limitation, various surface modification strategies, such as physical treatments (for example, sandblasting or sulfonation) and coating deposition of titanium or hydroxyapatite, have been explored

[Study on Intraoperative Motion-Following Technology for a Novel Robot-Assisted Total Knee Arthroplasty System].

At present, joint surgical robots for total knee arthroplasty (TKA) still face certain technical limitations during the bone resection process. The conventional approach of using lower-limb fixators to immobilize the extremity suffers from two major drawbacks: increased risk of osteotomy error and prolonged operative time. To address these shortcomings, this study introduces an innovative real-time tracking motion-following technology for orthopedic surgical robots. This approach enables the robotic arm to dynamically monitor and precisely follow subtle movements of osseous structures intraope

Revision surgery of spinal dynamic implants: a literature review and algorithm proposal.

INTRODUCTION: Dynamic stabilization of the spine has been performed since the 1990s with the double purpose of restoring spinal segmental stability and allowing residual movement at the operated level. When we take into account the different motion-preserving devices and the spinal areas where they are applied, we can identify three categories of spinal implants: anterior cervical, anterior lumbar, and posterior lumbar. However, as in all prosthetic procedures performed in orthopedic surgery, the life span of a joint replacement device is a central topic of discussion, and this is true also fo

Bacteriophage therapy for bone and joint infections.

Antibiotic resistance represents a threat to human health. It has been suggested that by 2050, antibiotic-resistant infections could cause ten million deaths each year. In orthopaedics, many patients undergoing surgery suffer from complications resulting from implant-associated infection. In these circumstances secondary surgery is usually required and chronic and/or relapsing disease may ensue. The development of effective treatments for antibiotic-resistant infections is needed. Recent evidence shows that bacteriophage (phages; viruses that infect bacteria) therapy may represent a viable and

The effect of sacral decortication on lumbosacral fixation in a calf spine model.

STUDY DESIGN: Animal cadaveric biomechanical study. OBJECTIVE: We hypothesized that increasing bony destabilization of a bovine cadaveric sacrum by anterior pedicle screw penetration and bilateral alar decortication would decrease the amount of load necessary for failure of the construct and result in failure through the decortication sites. SUMMARY OF BACKGROUND DATA: Fusion to the sacrum has become commonly used for treatment of neuromuscular scoliosis and fusions in osteoporotic patients. Fixation failure after attempted fusion to the sacrum may be attributed to iatrogenic causes such as

Improved pulmonary function after concave rib resection and posterior instrumentation for idiopathic scoliosis.

The authors conducted a prospective study in order to evaluate the effect'of concave rib osteotomy on pulmonary function, in the context of posterior instrumentation for the treatment of idiopathic scoliosis. Concave rib osteotomy improves the flexibility of the spine, especially in very rigid curves above 70. Seventy-eight consecutive children with a Cobb angle above 70 degrees were included in the study. Pulmonary function was measured preoperatively and at 12 months postoperatively. After surgery, all patients took part at an intense pulmonary rehabilitation program. The mean preoperative C

Radiographic evaluation of selective anterior thoracolumbar or lumbar fusion for adolescent idiopathic scoliosis.

According to Lenke classification of adolescent idiopathic scoliosis (AIS), patients with type 5 curve in which the structural major curve is thoracolumbar or lumbar curve with nonstructural proximal thoracic and main thoracic curves, could be surgically treated with selective anterior thoracolumbar or lumbar (TL/L) fusion. This study retrospectively analyzed the radiographies of selective anterior TL/L fusion in 35 cases of AIS with Lenke type 5 curve. Segmental fixation with a single rigid rod through anterior thoracoabdominal approach was applied in all patients. Measurements of scoliosis c

Device for zone-II flexor tendon repair. A multicenter, randomized, blinded, clinical trial.

BACKGROUND: The stainless-steel Teno Fix tendon-repair device has improved biomechanical characteristics compared with those of suture repair, and it was well tolerated in a canine model. The purpose of this study was to compare the Teno Fix with suture repair in a clinical setting. METHODS: Sixty-seven patients with isolated zone-II flexor tendon injury were randomized to be treated with a Teno Fix or a four-stranded cruciate suture repair. There were eighty-five injured digits: thirty-four were treated with the Teno Fix, and fifty-one served as controls. A modified Kleinert rehabilitation t

[Comparative evaluation of the methods of treatment of dislocations of the clavicle].

An analysis of treatment was made in 93 patients divided into three groups: conservative treatment (11 patients), operations by traditional methods (28 patients), surgery using constructions of titanium nickelide (54 operations). It was found that the conservative treatment of complete acromioclavicular end dislocation was followed by a repeated displacement, development of deforming arthrosis and deformity of the area of injury. Transcapticular fixation of acromioclavicular end dislocation resulted in the development of deforming arthrosis (68%), the incidence of this complication depending o

Simultaneous maxillary and mandibular distraction osteogenesis with a semiburied device.

Distraction osteogenesis is a technique utilizing natural healing mechanisms to generate new bone; it is commonly used to lengthen the hypoplastic mandible. Distraction of the maxilla and mandible as a unit is an obvious extension of the technique. We describe the application of a semiburied distractor to simultaneously lengthen the mandible and maxilla and level a canted occlusal plane in three cases. The indications for bimaxillary distraction are reviewed, including its advantages, disadvantages and limitations.

Surgical stabilisation of the lower limb in osteogenesis imperfecta using the Sheffield Telescopic Intramedullary Rod System.

The Sheffield Expanding Intramedullary Rod System was developed after experiencing problems with existing rod systems in the management of osteogenesis imperfecta. Between 1986 and 1996 we treated 74 bones in the lower limb in 28 children at a median follow-up of 5.25 years. We have reviewed 24 children with a total of 60 rods. Before surgery, all children had had multiple fractures of the lower limb. At review eight patients had experienced no further fractures, but three had suffered five or more subsequently. Before initial stabilisation, 15 children had never walked, and only three (13%) u

Superior dislocation of the fibular head associated with a tibial shaft fracture.

Superior dislocation of the proximal tibiofibular joint occurred in a 34-year-old woman who sustained a Grade 3B open left tibial fracture. An external fixator was applied to the tibia, and the tibiofibular joint was treated by closed reduction. Two bone graftings were performed, and the patient ultimately required a fibular osteotomy to allow tibial impaction. At 10 months posttrauma, the patient is fully weight-bearing and has no pain either at the tibia or the tibiofibular joint.

Clubfoot: a cord retainer.

Using a cord to retain the surgically corrected clubfoot replaces the need for repeated plaster casts and prolonged orthotics. Cords of braided polyester or nylon suture material 0.5 mm in diameter (a pair or more) were stretched between the fifth metatarsal and the lower metaphysis of the fibula. The required strength of the cord was judged from the initial range of motion (ROM) of the ankle and subtalar joints, and a factor derived from the body weight. The mean follow-up in five patients was 26.8 months (range 18-33 months). The peroneal muscles recovered an average of 9 months (range 7-11

Aneurysmal bone cyst of the thoracic spine: treatment by excision and segmental stabilization with Luque rods.

Preoperative radiological evaluation with magnetic resonance imaging and computed tomography was valuable in planning the surgical management of a destructive lesion of the posterior elements of the thoracic spine that was causing spinal cord compression in an 18-year-old woman. Preoperative recognition of bilateral involvement of the pedicles in addition to the laminae and spinous process led to use of prophylactic segmental stabilization of the spine with Luque rods after successful excision of an aneurysmal bone cyst. This case provides an example of the usefulness of computed tomographic s

Total posterior vertebrectomy of the thoracic or lumbar spine.

A total vertebrectomy in the thoracic or lumbar spine via a posterior approach can provide optimal neural decompression in certain severely compromised terminal patients affected with a malignant metastatic spinal lesion. The Arbeitsgemeinschaft für Osteosynthesefragen (AO) internal fixator, accompanied by methyl methacrylate reconstruction of the anterior vertebral column, provides sufficient stabilization so that immediate ambulation with minimal external support is possible. This single operative procedure allows reduction of the iatrogenic trauma by avoiding an additional anterior exposure
