Systematic and audiological indication criteria for bone conduction devices and active middle ear implants.

Certain patients with conductive or mixed hearing loss can benefit from bone-conduction hearing devices or active middle ear implants. Available devices differ in coupling site, energy transfer from the sound processor to the implant, and the active or passive actuator technology. The audiological benefit of those devices depends on the maximum stable power output and the noise floor of the device, the degree and expected stability of the sensorineural hearing loss and the coupling efficiency with the aim on achieving a minumum of 30-35 dB effective dynamic range. The choice of the device is o

Magnetic Resonance Imaging Compatibility of a New Generation of Active Middle Ear Implant: A Clinically Relevant Temporal Bone Laboratory Study.

OBJECTIVE: Magnetic resonance imaging (MRI) has become an essential tool of modern medical imaging and disease diagnosis. In November 2014, a new MRI-conditional (up to 1.5 T) generation of an active middle ear implant (AMEI) was released to the market. The aim of the study was to test the MRI compatibility of the new implant system in a clinical-anatomical study. DESIGN: Experimental cadaver head model. SETTING: Temporal bone laboratory. PARTICIPANTS: AMEIs were implanted in 28 fixed temporal bones at three different floating mass transducer (FMT)-coupling positions (N = 8 short process of

Microanatomic analysis of the round window membrane by white light interferometry and microcomputed tomography for mechanical amplification.

OBJECTIVE: The round window membrane (RWM) is increasingly becoming a target for amplification using active middle ear implants. However, the current strategy of using available transducer tips may have negative consequences for the RWM. We investigated the microanatomy of the RWM to establish a basis for the design of the transducer tip for the RWM driver. STUDY DESIGN: Using the guinea pig as an animal model, microcomputed tomography (μCT) and white light interferometry were used to study the topography of the RWM and RW niche (RWN). The curvatures of the RWM surface were calculated using t

[Botulinum toxin to treat sweat caused sequelae in patients with hearing aids, active middle ear implants and cochlear implants].

The production of sweat in the temporal skin region may be a serious problem for patients with hearing aids, active middle ear implants or cochlear implants. We report on two patients suffering from a loss of function of their hearing aid and a reduction of the "wear comfort" of an active middle ear implant. The patients underwent intracutaneous botulinum toxin (BTX) treatment of the temporal skin region. In both patients a distinct improvement of their complaints occurred, enabling them to use their hearing aids and active middle ear implants continuously. BTX injections are suited to improve

[Measuring vibrations by laser doppler vibrometry of the human tympanic membrane after stapes fixation].

At times preoperative diagnosis of otosclerosis can prove difficult. Even the objective measurement of the stapes reflex is not very conclusive in patients suffering from more severe hearing loss. Laser-Doppler vibrometry measures the velocity or amplitude of the tympanic membrane (umbo) vibrations. Laser-Doppler vibrometry has a sensitivity that is highly by the factor 10(4) than tympanometry. The herewith presented examinations show, that there is a correlation between the frequency patterns of the displacement amplitudes and experimentally simulated otosclerosis.

[Otorhinolaryngologic aspects of diving sports].

ENT disorders are the most common of all medical problems of diving. This review summarizes the specific conditions and ENT diseases in Scuba diving. During compression failure to equalize the pressure of air-filled cavities surrounded by bone deprives the middle ear or sinuses of aeration. Middle ear barotrauma is the most common barotrauma encountered in divers while sinus barotrauma and especially inner ear barotrauma (with rupture of the round or oval window) are less common. Decompression sickness in primarily the result of inert gas bubbles; deafness and vertigo may result if the inner e

Surgical and audiological outcomes with a new transcutaneous bone conduction device with reduced transducer thickness in children.

PURPOSE: Due to smaller bone thickness, young children with conductive or mixed hearing loss or single-sided deafness were previously most commonly treated with a percutaneous osseointegrated bone-anchored hearing aid (BAHA) or an active middle-ear implant. While the BAHA increases the risk of implant infections, skin infection, overgrowth of the screw or involvement of the implant in head trauma, middle-ear implant surgery involves manipulation of the ossicles with possible risk of surgical trauma. These complications can be omitted with transcutaneous bone conduction implant systems like the

Finite element analysis of round-window stimulation of the cochlea in patients with stapedial otosclerosis.

An active actuator coupled to the round window (RW) can transmit mechanical vibrations into the cochlea and has become a therapeutic option of hearing rehabilitation for patients with stapedial otosclerosis. A finite-element model of the human ear that includes sound transmission effects of the vestibular and cochlear aqueducts of the inner ear is adopted in this study for investigating the cochlear response to RW stimulation under stapes fixation. There are two effects due to otosclerosis of the stapes: the fixation of the stapedial annular ligament (SAL) and the increase of the stapes mass.

Stapes surgery: a National Survey of British Otologists.

To investigate individual stapes surgery practice in the UK, a retrospective study was conducted by postal questionnaire to all 'assumed' stapes-performing otologists. 225 questionnaires were sent out to practicing otologists in the UK. 184 replies (81.8 %) indicated that 134 (72.9 %) otologists perform stapes surgery [stapedectomy (8.2 %), stapedotomy (91.0 %) or other (0.8 %)]. The '6-10 stapes operation per year' category is the most common, with most using general anaesthetic (GA) (78.3 %). Unilateral surgery is advised in 89.6 %, and 96.3 % perform second-side surgery, with all advising t

Sympathetic auricular chondritis in rats: a model of autoimmune disease?

Following the unilateral implantation of metal ear tags in female Crl:CD(SD)BR-rats, chronic inflammatory lesions were observed in both auricles in 7% and 24% of the animals after 30 and 60 weeks, respectively. Involvement of the collateral auricles was identified only after diffuse inflammation of the ear tag-marked pinnae had developed. Histological examination revealed a multifocal granulomatous chondritis, characterized by progressive destruction of the cartilaginous plate and excessive regeneration of cartilaginous tissue. IgG and complement deposits were present in the matrix of the marg
