The IL-1β/NETs/AIM2 axis participates in the formation of trauma-induced heterotopic ossification by orchestrating crosstalk between neutrophils and macrophages.

INTRODUCTION: The limitations of clinical treatment for trauma-induced heterotopic ossification (tHO) make it of great significance to fully study its pathogenesis for the treatment of this disease. The infiltration of inflammatory cells, as an important indicator of disease progression, should be further studied. OBJECTIVES: In our study, we found that in the early stages of acute tendon injury, increased IL-1β can induce NETosis in neutrophils. Neutrophils and macrophages are early infiltrating cells in acute tendon injury, and their relationship has aroused our curiosity. METHODS: In our

The effect of extracorporeal shock wave therapy on large neurogenic heterotopic ossification in a patient with pontine hemorrhage: A case report and literature review.

RATIONALE: Heterotopic ossification (HO), an ectopic bone formation in soft tissue around the joint, is a complication observed in stroke patients. HO around the hip joint causes a reduction in the functional ability of patients by generating pain and limiting range of motion (ROM). In addition, it results in impaired mobility, ultimately affecting quality of life and increasing the mortality of patients. Extracorporeal shock wave therapy (ESWT) has demonstrated efficacy in treating soft tissue inflammation and has been used to reduce patients' pain in HO. However, almost none of the studies r

Treatment of Temporomandibular Joint Heterotopic Ossification: A Novel Protocol With Multimodal Therapy Based on Literature Review and Presentation of a Unique Case Report.

PURPOSE: The purpose of this study was to create a treatment protocol for cases of heterotopic ossification (HO) of the temporomandibular joint (TMJ), particularly those refractory to current TMJ HO protocols. In addition, we demonstrate the success of this protocol on a unique case of recurrent HO that failed multiple TMJ HO protocols in the setting of an improvised explosive device (IED) blast in a wounded warrior. METHODS: An electronic literature review was conducted via PubMed and Web of Science. Twenty-five studies were identified to provide supporting evidence for a proposed, up-to-dat

Incidence of Fusion Across Total Disc Replacement With Heterotopic Ossification: Are Ball and Socket Disk Replacements Fusing With and Without Radiographic Evidence.

STUDY DESIGN: This is a level III retrospective study. OBJECTIVE: The authors aim to review the outcomes and complications of ball and socket total disk replacements (TDRs). SUMMARY OF BACKGROUND DATA: TDR is a motion-preserving technique that closely reproduces physiologic kinematics of the cervical spine. However, heterotopic ossification and spontaneous fusion after implantation of the total cervical disk have been reported in several studies to decrease the range of motion postulated by in vitro and in vivo biomechanical studies. METHODS: The medical records of 117 consecutive patients

Association between muscle trauma and heterotopic ossification in spinal cord injured patients: reflections on their causal relationship and the diagnostic value of ultrasonography.

Paraplegic patients presenting with a subacute limitation of hip joint mobility were subjected to serial sonographic examinations. In four patients the initial sonographic study disclosed discontinuity with fluid collection in the psoas muscle, which was diagnostic of a traumatic muscle rupture. All four patients subsequently developed sonographic and radiographic evidence of heterotopic ossification. Our findings confirm that ultrasonography is an easy and inexpensive screening method for the early diagnosis of heterotopic ossification. The sonographic results obtained in these four paraplegi

Incidence, risk factors, characteristics and shoulder joint function status of heterotopic ossification following surgical treatment of clavicle fracture or acromioclavicular joint dislocation.

BACKGROUND: Heterotopic ossification (HO) around the shoulder is common in patients with clavicle fracture or acromioclavicular joint (ACJ) dislocation. Periarticular HO sometimes leads to poor joint function. HO of the elbow and hip requires active prevention. However, studies on the incidence, risk factors, characteristics, and shoulder joint function of HO around the shoulder after surgical treatment of clavicle fracture or ACJ dislocation are still lacking. STUDY HYPOTHESIS: ACJ dislocation, clavicular hook plate fixation and excessive screw length are independent high-risk factors for po

A new BMP type 1 receptor kinase inhibitor for safe and efficient oral treatment to prevent genetically induced heterotopic ossification in mice.

Fibrodysplasia ossificans progressiva (FOP) is a rare genetic heterotopic ossification (HO) disorder that currently lacks a practical and definitive preventative approach. FOP is driven by gain-of-function variants in ACVR1, increasing dysregulated BMP signaling pathway, thus resulting in progressive and permanent replacement of skeletal muscle and connective tissues with heterotopic bone, ultimately leading to severe debilitation and premature death. Here, we describe the discovery of RK783, a small-molecule that inhibit BMP type 1 receptor kinase developed for treating FOP. This compound, th

Unintended fusion in cervical artificial disk replacement: a prospective study on heterotopic ossification, progression, and clinical outcome, with 5-year follow-up.

PURPOSE: Heterotopic ossification (HO) may cause unintended fusion in artificial disk replacement (ADR), failing to preserve motion. The reported incidence of HO varies hugely. This study aimed to determine prevalence of HO, progression, predisposing factors to occurrence and to progression, and potential effect on clinical outcomes. METHODS: Eighty-three patients treated with ADRs for cervical radiculopathy at one or two segments were extracted from a previously published RCT. Of the 83 patients, 59 had remaining ADR (79 implants) and sufficiently high-quality X-rays at 5 years of follow-up

Extremely severe scoliosis, heterotopic ossification, and osteoarthritis in a three-generation family with Crouzon syndrome carrying a mutant c.799T>C FGFR2.

BACKGROUND: Crouzon syndrome is a rare and complex autosomal dominant craniosynostosis syndrome with a prevalence of approximately 1 in 60,000 births. The typical features are craniosynostosis, proptosis, midfacial hypoplasia, and noncranial manifestations, including deformities in the cervical spine, elbow, and fingers. Crouzon syndrome is usually caused by pathogenic variants in the fibroblast growth factor receptor 2 (FGFR2) gene. METHODS: We reported a three-generation family with Crouzon syndrome; the proband showed extremely severe limb abnormalities. The clinical features were obtained

Sustained delivery of rhBMP-2 by means of poly(lactic-co-glycolic acid) microspheres: cranial bone regeneration without heterotopic ossification or craniosynostosis.

BACKGROUND: Commercially available recombinant human bone morphogenetic protein 2 (rhBMP2) has demonstrated efficacy in bone regeneration, but not without significant side effects. The authors used rhBMP2 encapsulated in poly(lactic-co-glycolic acid) (PLGA) microspheres placed in a rabbit cranial defect model to test whether low-dose, sustained delivery can effectively induce bone regeneration. METHODS: The rhBMP2 was encapsulated in 15% PLGA using a double-emulsion, solvent extraction/evaporation technique, and its release kinetics and bioactivity were tested. Two critical-size defects (10 m

Recombinant human bone morphogenetic protein 2-induced heterotopic ossification of the retroperitoneum, psoas muscle, pelvis and abdominal wall following lumbar spinal fusion.

A 45-year-old man presented with vertebral collapse at L5 as an initial manifestation of multiple myeloma and underwent spinal fusion surgery using recombinant human bone morphogenetic protein-2 (rhBMP-2). Subsequent computed tomography (CT) scans and X-rays revealed heterotopic ossification of the left psoas muscle, pelvis, and anterior abdominal wall. While the occurrence of heterotopic ossification has previously been reported when rhBMP-2 has been used for spinal fusion surgery, this case demonstrates that it can occur to a much greater degree than previously seen.

The bone turnover marker procollagen type I N-terminal propeptide is associated with 12-week neurogenic heterotopic ossification after traumatic brain injury.

Neurogenic heterotopic ossification (NHO) after traumatic brain injury may involve osteogenic synthesis of a type I collagen-rich matrix. Procollagen type I N-terminal propeptide (P1NP) reflects type I collagen formation. This single-center retrospective cohort study examined whether baseline P1NP was associated with radiographic NHO within 12 weeks after traumatic brain injury. We reviewed 124 adults transferred to physical and rehabilitation medicine within 21 days after injury who completed 26-34 days of inpatient rehabilitation and had 12-week follow-up. The primary Firth penalized logisti

Integrin-linked kinase-mediated promotion of osteogenic differentiation in bone marrow mesenchymal stem cells: A driver of heterotopic ossification in ankylosing spondylitis.

Excessive osteogenesis in bone marrow mesenchymal stem cells (BMSCs) contributes to the ectopic ossification associated with ankylosing spondylitis (AS), yet the underlying mechanisms are not fully understood. Integrin-linked kinase (ILK) plays an important role in the inflammatory process of AS, but its expression and effects on osteophytogenesis require further evaluation. Hence, we aimed to explore the role and mechanisms of ILK in the syndesmophyte formation of AS. After establishing the BMSC lines, the mineralization potential of BMSCs from AS patients (AS-BMSCs) was found to be greater t

Cartilage-derived retinoic acid-sensitive protein (CD-RAP): A stage-specific biomarker of heterotopic endochondral ossification (HEO) in fibrodysplasia ossificans progressiva (FOP).

BACKGROUND: Genesis of a cartilaginous scaffold is an obligate precursor to bone formation in heterotopic endochondral ossification (HEO). We tested the hypothesis that cartilage-derived retinoic acid-sensitive protein (CD-RAP) can serve as a plasma biomarker for the pre-osseous cartilaginous stage of HEO. Palovarotene, a retinoic acid receptor-gamma (RARγ) agonist, has been proposed as a possible treatment for fibrodysplasia ossificans progressiva (FOP) and is a potent inhibitor of HEO in mouse models. Current drug development for FOP mandates the identification of stage-specific biomarkers t

Paraneoplastic stiff-person syndrome, heterotopic soft tissue ossification and gonarthritis in a HLA B27-positive woman preceding the diagnosis of Hodgkin's lymphoma.

Paraneoplastic neurologic syndromes associated with Hodgkin's lymphoma include the stiff-person syndrome. A case of stiff-person syndrome is reported who first presented with muscular hyperactivity and acute respiratory failure followed by heterotopic soft tissue ossification and acute seronegative gonarthitis. Initial improvement of a tetanus-like clinical picture was achieved with benzodiazepam given by continuous infusion for analgo-sedation to mechanically ventilate the patient followed by baclofen after successful weaning. The patient was HLA B27 positive and on conventional testing no au

Prevention of heterotopic ossification after total hip replacement. A prospective, randomised study using acetylsalicylic acid, indomethacin and fractional or single-dose irradiation.

We have carried out a prospective, randomised study of prophylaxis for heterotopic ossification (HO) comparing indomethacin for 7 and 14 days, acetylsalicylic acid, and fractional (4 x 3 Gy) or single exposure of 5 or 7 Gy irradiation after operation. We initially had 723 patients (733 hip replacements), but after withdrawals there were 685 hips of which 18.4% developed HO; 14% were grade I, 2.9% grade II and 1.5% grade III of the Brooker classification. We compared the results between these groups with those of a matched control series and found that indomethacin, 2 x 50 mg for 7 and 14 days,

Head and neck squamous cell carcinoma with heterotopic ossification, lymphovascular invasion, and nodal and pulmonary metastases in a 23-year-old Morgan gelding.

Primary squamous cell carcinoma of the head and neck occurs in the skin or squamous epithelial lining tissues of the oral cavity, pharynx, larynx, and sinonasal tract. Although it is a common tumor in horses, distant metastatic spread to the lung is rare. This report describes a case of metastatic pulmonary squamous cell carcinoma in a 23-year-old Morgan gelding. The clinical signs displayed by this gelding in some ways mimicked the typical presentation of equine multinodular pulmonary fibrosis or thoracic lymphoma. The postmortem diagnosis in this case was head and neck squamous cell carcinom

Bony Cubital Tunnel Syndrome: A Case Report of Heterotopic Ossification Causing Circumferential Ulnar Nerve Encasement at the Elbow and Review of Current Management.

BACKGROUND: Heterotopic ossification (HO) is a rare but known complication of brain and nerve trauma, orthopedic trauma, and burns. Nerve compression due to HO is extremely rare; "bony cubital tunnel syndrome," or compression of the ulnar nerve at the elbow due to HO, is an unusual presentation that requires special considerations for treatment. CASE PRESENTATION: We present a 50-year-old man who presented to our hospital after vehicular polytrauma with associated car fire and prolonged extrication. He experienced extensive trauma, with all classically described risks for HO. He developed bon
