The distribution of S-100 protein in articular cartilage from osteoarthritic joints.

The distribution and localization of S-100 protein was compared with safranin-O staining and H and E morphology in relatively unaffected, degenerative, and osteophytic regions of human articular cartilage from 26 joints obtained at the time of total joint replacement for osteoarthritis. S-100 protein immunoreactivity in osteophyte cartilage was more intense than in unaffected regions and comparable to the staining intensity found in metaplastic cartilage tufts in degenerating cartilage. Immunoreactivity for S-100 protein appeared more prominent in the deep layers of osteophyte cartilage adjace…

Ultrasound in osteoarthritis of the hand: a comparison to computed tomography and histology.

OBJECTIVE: To compare structural findings between US, micro-CT (µCT) and histology in people with OA of the hands. METHODS: We analysed DIP and PIP joints of 31 fingers from 15 dissecting-room cadavers with OA of the hands. The occurrence of bone erosions and osteophytes were recorded by US, µCT and histology at 16 regions for each joint and compared for each method. RESULTS: In total, US (n = 558, 56.2% of 992 examined regions) and µCT (n = 493, 49.7%) detected a higher frequency of osteophytes at PIP and DIP joints than histology (n = 161, 23.4% of 689 histological examined regions; P = 0.…

Long-term results after surgical treatment of diffuse idiopathic skeletal hyperostosis (DISH) causing dysphagia.

Diffuse idiopathic skeletal hyperostosis (DISH) is a non-inflammatory, systemic skeletal disease. The associated formation of anterior cervical osteophytes can cause severe dysphagia, so the osteophytes have to be surgically removed. Because the clinical syndrome is rare, long-term outcome after surgical therapy is likewise scarce. In this retrospective, single-center study, five consecutive patients with DISH causing dysphagia and following resection of osteophytes between 2005 and 2015 were included. Patient and surgical reports were evaluated regarding surgery-related and postoperative comp…

Proteinase-activated receptor 2 modulates OA-related pain, cartilage and bone pathology.

OBJECTIVE: Proteinase-activated receptor 2 (PAR2) deficiency protects against cartilage degradation in experimental osteoarthritis (OA). The wider impact of this pathway upon OA-associated pathologies such as osteophyte formation and pain is unknown. Herein, we investigated early temporal bone and cartilage changes in experimental OA in order to further elucidate the role of PAR2 in OA pathogenesis. METHODS: OA was induced in wild-type (WT) and PAR2-deficient (PAR2-/-) mice by destabilisation of the medial meniscus (DMM). Inflammation, cartilage degradation and bone changes were monitored usi…

An unusual presentation of thoracic diffuse idiopathic skeletal hyperostosis (DISH) and video-assisted thoracoscopic surgery (VATS).

INTRODUCTION AND IMPORTANCE: Diffuse idiopathic skeletal hyperostosis (DISH), also known as Forestier's disease and Forestier and Rotes-Querol disease, is a systemic condition characterized by calcification and ossification of ligaments and entheses which often mainly affects the thoracic spine. Anterior osteophyte compression of the esophagus resulting in dysphagia and dyspepsia is extremely rare in symptomatic thoracic DISH. CASE PRESENTATION: A 72-year-old male presented with dyspepsia and dysphagia for 10 months. A large beak-like anterior osteophyte, detected by a radiographic study and …

Diffuse Idiopathic Skeletal Hyperostosis: A Case with Dysphonia, Dysphagia and Myelopathy.

BACKGROUND Diffuse idiopathic skeletal hyperostosis (DISH) is characterized by the ossification of soft tissues, primarily the ligaments and enthesis. Exuberant osteophyte formation of the anterior longitudinal ligament of the spine is usually found. Among the reported complications of cervical osteophyte, dysphagia is the most frequent symptom, and dysphonia is rare. CASE REPORT A 66-year old male was suffering from progressive dysphonia, dysphagia, and myelopathy. Anterior cervical osteophytes and ossification of the posterior longitudinal ligament (OPLL) was shown on x-ray and computed tomo…

Double acetabular wall--a misleading point for hip arthroplasty: an anatomical, radiological, clinical study.

PURPOSE: Despite the great attention focused on cup positioning in primary total hip arthroplasty (PTHA), it is surprising to find so few studies that have dealt with cup placement. A common thwarting problem for correct cup placement during PTHA is the existence of osteophytes, which obscure the anatomical landmarks. In this study we aimed to evaluate the morphology of acetabular osteophyte formation in patients with osteoarthritis. METHOD: We evaluated 276 patients with hip complaints, using plain X-rays and CT scans. RESULTS: Of these patients, 57 underwent surgery. We developed a staging…

[Diagnosis and treatment of esophagustype cervical spondylosis].

Esophagustype cervical spondylosis is easily misdiagnosed or missed diagnosed for its lower incidence. The pathological basis manily concerned with compression of osteophyte for esophageal and tracheal wall and local inflammatory reaction. The diagnosis of disease is generally not difficult. Especially dynamic esophagogram could not only show degree of cervical degeneration and osteophyte shape, but also could clearly observe location and degree of osteophytosis indenting esophageal and tracheal. Recently, the treatment of esophagustype cervical spondylosis has been the focus of attention. It …

Deletion of Hyaluronan-Binding Protein Involved in Hyaluronan Depolymerization (HYBID) Results in Attenuation of Osteoarthritis in Mice.

Hyaluronan (HA)-binding protein involved in HA depolymerization (HYBID) is involved in cartilage destruction via HA depolymerization in human knee osteoarthritis. However, the role of HYBID in the progression of osteoarthritis remain elusive. This study sought to examine whether genetic depletion of Hybid could suppress surgically induced osteoarthritis of mouse knee joints. In osteoarthritis induced by medial collateral ligament transection with meniscus removal, articular cartilage destruction and osteophyte formation at the medial femoral-tibial joint were significantly inhibited in Hybid-d…

Hip subluxation and osteophye morphology are related to coronal contracture of the hip.

Coronal hip contracture induces pelvic obliquity in the presence of hip osteoarthritis (HOA), followed by the functional leg-length discrepa … acture. We investigated HOA, subluxation, anatomical factors, spinal factors, and the morphology of osteophytes at the inferomedial femoral head and compared parameters among groups to clarify the predictors of contracture. Eighteen hips (7.6%) were classified as having adduction contracture and 23 (9.4%) as having abduction contracture. Crowe classification …

Cervical myelopathy caused by pseudoarthrosis between posterior arch of the atlas and lamina of the axis in Klippel-Feil syndrome: a case report.

STUDY DESIGN: Case report. SUMMARY OF BACKGROUND DATA: Cervical myelopathy caused by osteophyte formation appearing at the pseudoarthrosis between the posterior arch of the atlas and the lamina of the axis is extremely rare. To our knowledge, only 2 cases in non-Klippel-Feil syndrome have been reported so far. METHODS: A 61-year-old man, who was diagnosed as Klippel-Feil syndrome with assimilation between C2 and C3 vertebrae, which were fused in hyperlordosis, suffered cervical myelopathy. Complete bipartition of the atlas was also demonstrated. Massive osteophytes, which severely compressed…

Bow Hunter's Syndrome.

Bow hunter's syndrome is due to vertebrobasilar insufficiency caused by rotational compression of the vertebral artery. We report a case in which an osteophyte compressed the left vertebral artery causing cerebellar stroke. The patient underwent successful resection of the osteophyte via anterior surgical approach, and his symptoms of headache and dizziness dissipated postoperatively. This unique syndrome has been treated with multiple modalities and must remain in the clinician's differential as a treatable cause of stroke.

A tricky trigger finger: a patient with intermittent intrinsic locking of a metacarpophalangeal joint.

A trigger finger is not always caused by a stenosing tenosynovitis, but may rarely be due to an internal derangement of the metacarpophalangeal (MCP) joint. Several different mechanisms may be responsible for this intrinsic joint locking. A patient is described with intermittent locking of an index MCP joint due to an osteophyte tethering the radial accessory collateral ligament. Removal of the osteophyte solved the problem. Awareness of this condition may prevent unnecessary exploration of the proximal pulley and tendon sheath.

Size-invariant descriptors for detecting regions of abnormal growth in cervical vertebrae.

Digitized spinal X-ray images exhibiting specific pathological conditions such as osteophytes can be retrieved from large databases using Content Based Image Retrieval (CBIR) techniques. For efficient image retrieval, it is important that the pathological features of interest be detected with high accuracy. In this study, new size-invariant features were investigated for the detection of anterior osteophytes, including claw and traction in cervical vertebrae. Using a K-means clustering and nearest neighbor classification approach, average correct classification rates of 85.80%, 86.04% and 84.4…

Transpharyngeal approach for the treatment of dysphagia due to Forestier's disease.

Forestier's disease (diffuse idiopathic skeletal hyperostosis) is characterized by extensive spinal osteophyte formation and endo-chondral ossification of paravertebral ligaments and muscles. Dysphagia in the setting of Forestier's disease is a rare and hence often unrecognized entity. The dysphagia is due to mechanical obstruction in the initial stages and later due to inflammation and fibrosis. Most of these patients are treated conservatively in the initial stages and later by excision of osteophytes through a lateral cervical approach. We present a case of dysphagia due to cervical osteoph…

[Bony contact in arthritic and healthy elbow joints : Studies in cadaveric specimens].

BACKGROUND: Arthroscopic and open debridement arthroplasty for elbow arthrosis produce excellent results. Resection of the tip of the olecranon and coronoid, as well as debridement of the associated fossae, is frequently described as sufficient for restoring range of motion and reducing pain. OBJECTIVES: The purpose of this study was to demonstrate that medial and lateral osteophytes of the olecranon and the coronoid usually mark the first bony contact and may, therefore, restrict range of motion. MATERIAL AND METHODS: 11 fresh-frozen elbows were dissected. The fossae were coated with Optosi…
