Fresh tympanic membrane perforations heal without significant loss of strength.

HYPOTHESIS: The mechanical and structural properties of the tympanic membrane change after a perforation has healed. BACKGROUND: In previous studies, efforts have been made to enhance the healing process of tympanic membrane perforations. The strength of the healed perforation has been tested with moiré interferometry in gerbils, but in no other species. METHODS: A laser myringotomy was made on 10 Sprague-Dawley rats and 10 CBA mice, and assessments were made after 2 or 4 weeks with moiré interferometry and light and electron microscopy. RESULTS: The mean peak displacement at pressure loads

Pars flaccida displacement pattern in purulent otitis media in the gerbil.

HYPOTHESIS: Our hypothesis is that purulent otitis media and otitis media with effusion cause stiffness loss of the tympanic membrane. This loss of stiffness may be persistent and precede the development of retraction pockets and cholesteatoma. BACKGROUND: Postinflammatory changes such as retraction pockets and cholesteatoma develop in the pars flaccida and in the pars tensa of the tympanic membrane. In our previous experimental studies, stiffness changes were shown to develop early in the pars tensa in response to purulent otitis media and otitis media with effusion. These changes are sugges

Balloon Dilation With Tympanostomy Enhances Outcomes in Adult Chronic Otitis Media With Effusion.

OBJECTIVES: This study aimed to comparatively evaluate the therapeutic efficacy of balloon dilation eustachian tuboplasty (BDET) versus tymp … interventions: (1) TTI alone, (2) BDET + TTI, or (3) BDET alone. Preoperative evaluations included otoscopy, pure-tone audiometry (PTA), tympanometry, nasopharyngeal endoscopy, eustachian Tube Dysfunction Questionnaire (ETDQ-7), and temporal bone CT scans. Postoperative evaluation incorporated PTA, tympanometry, and ETDQ-7 assessment. Statistical analysis …

Diagnosis of tympanic effusions using hyperspectral imaging to prevent unnecessary paracentesis procedures.

BACKGROUND: Otitis Media with Effusion (OME) is a prevalent childhood condition, affecting up to 90% of children with 10% progressing to a chronic state. It is commonly treated by tympanostomy tube placement. Diagnosis relies on clinical history, otoscopy, audiometry, and tympanometry, but these methods have a high false-positive rate of 15-28%. This highlights the need for an objective, reliable, and non-invasive diagnostic tool to reduce unnecessary surgeries. METHODS: To evaluate the rate of false-positive OME diagnoses in our tertiary hospital, a retrospective database analysis was conduc…

Otogenic meningitis in children.

PURPOSE: Otogenic meningitis is a rare but potentially life-threatening intracranial complication of otitis media (OM). Our aim was to study the incidence of childhood otogenic meningitis and to compare clinical presentation, causative pathogens, diagnostics, treatment, and outcome of otogenic versus non-otogenic meningitis. METHODS: Charts were reviewed for 47 children admitted to our tertiary center with bacterial meningitis (BM) between 2010 and 2020. Otoscopy and/or imaging were used to determine the otogenic meningitis ratio and the mean annual incidence was calculated. RESULTS: Eight (…

Bilateral Hearing Loss in Primary Ciliary Dyskinesia: A Study of Conductive and Sensorineural Mechanisms from Pediatric and Adult Cases.

BACKGROUND: Primary ciliary dyskinesia (PCD) is a rare genetic disorder that affects the respiratory and auditory systems. This study aims to assess the prevalence, type, and severity of bilateral hearing loss (HL) in PCD and Kartagener syndrome (KS) patients, examining age-related differences and chronic impacts of otologic pathologies. METHODS: A total of 19 patients (38 ears), including 6 children and 13 adults, were evaluated from June to September 2021. Comprehensive clinical examinations included otoscopy, tympanometry, and pure tone audiometry (PTA) for air and bone conduction. Tympano…

Ear disease and hearing loss: a descriptive study of Aboriginal children living in metropolitan South Australia.

OBJECTIVE: This article aims to describe the rates of middle ear disease in Aboriginal children living in metropolitan Adelaide. METHODS: Data from the Under 8s Ear Health Program (population-based outreach screening) were analysed to identify rates of ear disease and the referral outcomes for children identified with ear conditions during screening. RESULTS: In total, 1598 children participated in at least one screening between May 2013 and May 2017. Males and females were equally represented; 73.2% had one or more abnormal findings on otoscopy at the first screening visit, 42% had abnormal…

Ear health and hearing in urban Aboriginal children.

OBJECTIVE: Evaluate ear health and hearing among urban Aboriginal children and quantify relationships with child, family and social factors. METHODS: Baseline questionnaire and ear health examinations from 1430 children with diagnoses (0.5-18 years) attending Aboriginal Health Services enrolled in SEARCH. Ear health outcomes were Otitis Media (OM), and hearing loss (three-frequency average hearing loss >20dB) diagnosed using pneumatic otoscopy, tympanometry, and audiometry. RESULTS: Half the children 0.5-3 years had OM (51.5%, 136/264). One third 0.5-18 years (30.4%; 435/1430) had OM, includ…

Second Sets of Tympanostomy Tubes Extrude Sooner.

INTRODUCTION: Some clinicians believe second sets of tympanostomy tubes extrude more quickly than first sets. STUDY DESIGN: Retrospective case-control series. METHODS: We identified children who were examined 12 months after placement of their second set of tympanostomy tubes and compared them to a similar number of children who were examined 12 months after their first set of tympanostomy tubes. Extrusion was determined by otoscopy, otomicroscopy, and/or tympanometry. RESULTS: One hundred eighteen children had 12-month follow-up data available after their first set of tubes, 54 had 12-mont…

Iatrogenic Third Window After Retrosigmoid Approach to a Vestibular Schwannoma Managed with Cochlear Implantation.

OBJECTIVE: To present a case of an iatrogenic inner ear third window after vestibular schwannoma microsurgery. PATIENTS: A 42-year-old male presented 9-months after left-sided retrosigmoid approach for an intracanalicular vestibular schwannoma with hearing-preservation attempt performed elsewhere. [Long-term Follow up and patient satisfaction after tympanoplasty with Titanium Clip-Prothesis].

Aim of the present study is to evaluate the long term results of ossiculoplasty using titanium clip prosthesis. A retrospective analysis with prospective follow-up has been done including for all patients who had a Clip-PORP implanted between the years 2002-2013. Following this, audiometric hearing results were measured with clinical examination including otoscopy and quality of life evaluation using Glasgow benefit inventory. 48 patients (51 ears, 29 men, 19 women, average age 44.1 years) received followed up after an average duration of 6.3 years.
