Compressive optic neuropathy secondary to a lateral rectus muscle dermoid cyst.

A 48-year-old man presented 2 weeks after sudden OS vision loss. On examination, the patient had decreased OS visual acuity, an ipsilateral afferent pupil defect, and proptosis. MRI of the orbit with and without gadolinium showed a large, enhancing heterogeneous mass extending to the orbital apex. Uncomplicated resection was performed and pathology was consistent with a dermoid cyst. Postoperatively, the patient had improved visual acuity and minimal lateral rectus dysfunction. There are only 4 reported cases of dermoid cysts located within the lateral rectus muscle, albeit this is the first c

[Acute retinal necrosis syndrome following chickenpox].

CASE REPORT: A 34-year-old male patient developed acute retinal necrosis in his left eye about three weeks after the onset of chickenpox. Systemic antiviral treatment with intravenous acyclovir (10 mg/kg/8 hours) and systemic corticosteroids (1 mg/kg/day) controlled the retinitis and the patient suffered no loss of visual acuity. DISCUSSION: Acute retinal necrosis is an unusual complication of chickenpox. A mild form of this entity has been described during the course of primary varicella-zoster infection. Adequate and early therapy during the acute phase of the disease with intravenous acycl

Reactivation of ocular toxoplasmosis after LASIK.

PURPOSE: To report a reactivation of ocular toxoplasmosis after LASIK. METHODS: Case report of a 34-year-old man who underwent bilateral LASIK. The posterior segment examination revealed an old toxoplasmosis scar in the retinal periphery of the right eye. RESULTS: Uncorrected visual acuity improved postoperatively, and the patient was satisfied. However, 52 days after the procedure, he complained of loss of visual acuity in his right eye. Examination revealed signs of anterior uveitis, vitreitis, and active chorioretinal lesion satellite of the old toxoplasmosis scar. The patient was treated

Cyclooxygenase-2 inhibitors: a new therapeutic option in the treatment of macular edema after cataract surgery.

Two patients had uneventful phacoemulsification. After initial improvement, vision deteriorated because of cystoid macular edema (CME). In 1 patient, treatment with systemic nonsteroidal antiinflammatory drugs showed significant improvement in visual acuity but had to be discontinued because of side effects and a relapse of the disease. In the other patient, this therapy was not sufficient. Both patients were given valdecoxib, a cyclooxygenase-2 inhibitor. The new therapy was tolerated well and led to significant and stable improvement in visual acuity in both patients. To our knowledge, this

The Patient-Oriented Scoring of Atopic Dermatitis and SCORAD in young children: New data on interpretability and clinical usefulness.

BACKGROUND: There is limited data about the clinical meaningfulness of the Scoring of Atopic Dermatitis (SCORAD) and Patient-Oriented SCORAD (PO-SCORAD), particularly in children with mild-to-moderate AD. Regular use of patient-reported outcomes, may deliver more accurate information about the overall health status of AD patients than routine but sparse physician assessments. OBJECTIVE: To confirm the correlation between SCORAD, PO-SCORAD, Patient-Oriented Eczema Measure (POEM) and Investigator's Global Assessment (IGA). To evaluate the interpretability and clinical usefulness of the SCORAD a

A classification model for the telephonic triage of low acuity abdominal complaints in South Africa.

INTRODUCTION: Emergency medical services (EMS) dispatch emergency resources according to the appropriate patient acuity, ensuring those in need of urgent care receive prompt attention. Abdominal complaints have been highlighted as an incident type that is over-triaged in EMS. The development of a telephonic classification tool could improve emergency medical dispatch efficiencies by reducing the inappropriate prioritisation of responses to these cases. The study aimed to identify variables that classified abdominal complaints as low acuity at dispatch. METHODS: A retrospective cross-sectional…

Visual Acuity Examination Methodology in Keratoconus.

Visual acuity is one of the most important parameters for evaluating the vision of patients with keratoconus. This study reviewed 295 articles related to keratoconus published between 2017 and 2022 in which visual acuity was one of the parameters measured. The methodology of visual acuity testing in studies on keratoconus was thoroughly analyzed. The analysis showed that the most commonly indicated chart for testing visual acuity papers on keratoconus is the Snellen chart. It was shown that in 150 out of 295 articles, the authors do not describe the methodology for testing visual acuity. What

Discordance Between Eye-Specific Clinical Outcomes and Patient-Reported Visual Function After Faricimab Treatment for Neovascular Age-Related Macular Degeneration: A Two-Case Report.

Neovascular age-related macular degeneration is a retinal disease of older adults causing central vision loss associated with macular neovascularization. Treatment response is commonly evaluated using visual acuity and retinal morphology. These measures may not fully reflect patient-perceived benefit. We report two older adults with neovascular age-related macular degeneration treated with faricimab who demonstrated discordance between clinical and patient-reported outcomes. In the first case, best-corrected visual acuity changed from 20/200 to 20/180 and central retinal thickness decreased fr

Electroretinogram and visual-evoked potentials in children with optic nerve coloboma.

The aim was to evaluate alterations in Visual-Evoked Potentials (VEP) and Electroretinogram (ERG) and discover whether these tests are useful for determining residual visual acuity in cases where a patient is unable to collaborate. Flash and, when possible, Transient Pattern Reversal Visual-Evoked Potentials and Maximal Response ERG were recorded in 8 children (under three years old) affected by different aspects of optic nerve coloboma. None of them had visual acuity evaluated because of poor collaboration. All examinations were carried out using skin electrodes. Amplitude of the a and b comp

Meyer zum Gottesberge's head-shaking test for the evaluation of jumbling.

Jumbling consists of loss of vestibular eye movement reflexes and resultant oscillopsia during movement of the head. This results in a failure of clear vision during head movement. The head-shaking test for evaluation of the jumbling phenomenon was initially suggested by Meyer zum Gottesberge in 1952. In this test, binocular visual acuity is measured while the patient shakes his head at a rate of 2 or 3 movements per second, 10 to 20 degrees horizontally or vertically, and compared head still position. In normal subjects, only a slight decrease in visual acuity is noted due to this head-shakin
