Clinical Review of Hypertensive Acute Heart Failure.

Although acute heart failure (AHF) is a common disease associated with significant symptoms, morbidity and mortality, the diagnosis, risk stratification and treatment of patients with hypertensive acute heart failure (H-AHF) still remain a challenge in modern medicine. Despite great progress in diagnostic and therapeutic modalities, this disease is still accompanied by a high rate of both in-hospital (from 3.8% to 11%) and one-year (from 20% to 36%) mortality. Considering the high rate of rehospitalization (22% to 30% in the first three months), the treatment of this disease represents a major

Digital Health Interventions and Patient Safety in Abdominal Surgery: A Systematic Review and Meta-Analysis.

IMPORTANCE: Over the past 2 decades, several digital technology applications have been used to improve clinical outcomes after abdominal surgery. The extent to which these telemedicine interventions are associated with improved patient safety outcomes has not been assessed in systematic and meta-analytic reviews. OBJECTIVE: To estimate the implications of telemedicine interventions for complication and readmission rates in a population of patients with abdominal surgery. DATA SOURCES: PubMed, Cochrane Library, and Web of Science databases were queried to identify relevant randomized clinical

Polypharmacy and Clinical Outcomes in Hospitalized Patients With Acute Decompensated Heart Failure.

BACKGROUND: Polypharmacy is a common problem among patients with acute decompensated heart failure (ADHF) who often have multiple comorbidities. OBJECTIVE: The aim of this study was to define the number of medications at hospital discharge and whether it is associated with clinical outcomes at 1 year. METHODS: We evaluated the number of medications in 2578 patients with ADHF who were ambulatory at hospital discharge in the Kyoto Congestive Heart Failure Registry and compared 1-year outcomes in 4 groups categorized by quartiles of the number of medications (quartile 1, ≤ 5; quartile 2, 6-8; q

The LACE+ Index as a Predictor of 90-Day Supratentorial Tumor Surgery Outcomes.

BACKGROUND: The LACE+ (Length of stay, Acuity of admission, Charlson Comorbidity Index [CCI] score, and Emergency department [ED] visits in the past 6 mo) index risk-prediction tool has never been successfully tested in a neurosurgery population. OBJECTIVE: To assess the ability of LACE+ to predict adverse outcomes after supratentorial brain tumor surgery. METHODS: LACE+ scores were retrospectively calculated for all patients (n = 624) who underwent surgery for supratentorial tumors at the University of Pennsylvania Health System (2017-2019). Confounding variables were controlled with coarse

Implementation of Institutional Discharge Protocols and Transition of Care Following Acute Coronary Syndrome.

Despite improvements in acute care and survival following acute coronary syndrome (ACS) hospitalization, readmission remains common. In response, individual institutions have begun to develop their own protocols to reduce variability of care and readmission rates. This review provides approaches for developing and implementing institutional discharge protocols for continuity of care for ACS patients and describes key components of the discharge protocol. Furthermore, specific objectives of the protocol, including medication adherence, patient education, enabling access to cardiac rehabilitatio

Readmission and overstay after day case nasal surgery.

BACKGROUND: A readmission is classified as a patient necessitating readmission to hospital due to a post-operative complication following discharge. An overstay however, is classified as a patient having to stay longer than the planned duration in hospital (not having been discharged in the interim) due to a post-operative complication. This study aims to investigate patient-related factors that predispose to readmission or overstay and thus make recommendations to decrease the likelihood of readmission or overstay. METHOD: In this retrospective study 312 'day-case nasal procedures', were sele

Unplanned 30-day hospital readmission as a quality measure in gynecologic oncology.

OBJECTIVES: Thirty-day readmission is used as a quality measure for patient care and Medicare-based hospital reimbursement. The primary study objective was to describe the 30-day readmission rate to an academic gynecologic oncology service. Secondary objectives were to identify risk factors and costs related to readmission. METHODS: This was a retrospective, concurrent cohort study of all surgical admissions to an academic, high volume gynecologic oncology service during a two-year period (2013-2014). Data were collected on patient demographics, medical comorbidities, psychosocial risk factor

Impact of Discharge Medication Reconciliation Across a Five-Hospital Health System.

PURPOSE: Hospital readmission of a primary diagnosis can have a substantial financial impact via reductions in reimbursement rates up to 3%, and have a negative impact on the lives of patients. Discharge medication reconciliation (DMR) can reduce medication errors that play a role in readmission. The objective of this study is to evaluate the impact of expanding pharmacist-led DMR across a four hospital health system on 30-day readmission rate for high risk of readmission disease groups. METHODS: During the 3-month period, DMR was performed for patients with at least one of the disease states

Organisational determinants and consequences of diagnostic discrepancy in two large patient groups in the emergency departments: a national study of consecutive episodes between 2008 and 2016.

BACKGROUND: Diagnostic discrepancy (DD) is a common phenomenon in healthcare, but little is known about its organisational determinants and consequences. Thus, the aim of the study was to evaluate this among selected emergency department (ED) patients. METHOD: We conducted an observational study including all consecutive ED patients (hip fracture or erysipelas) in the Danish healthcare sector admitted between 2008 and 2016. DD was defined as a discrepancy between discharge and admission diagnoses. Episode and department statistics were retrieved from Danish registers. We conducted a survey am

Factors Associated With 30-Day Readmission After Primary Total Hip Arthroplasty: Analysis of 514 455 Procedures in the UK National Health Service.

IMPORTANCE: Thirty-day readmission to hospital after total hip arthroplasty (THA) has significant direct costs and is used as a marker of hospital performance. All-cause readmission is the only metric in current use, and risk factors for surgical readmission and those resulting in return to theater (RTT) are poorly understood. OBJECTIVE: To determine whether patient-related predictors of all-cause, surgical, and RTT readmission after THA differ and which predictors are most significant. DESIGN, SETTING, AND PARTICIPANTS: Analysis of all primary THAs recorded in the National Health Service (N

Comparison of readmission data between different categories of antipsychotic drugs at a state psychiatric hospital in Oregon.

INTRODUCTION: This chart review utilizes readmission rates and mean time to readmission as markers of drug efficacy to compare different categories of long-acting injectable antipsychotics (LAIAs), antipsychotic polypharmacy, and clozapine to oral antipsychotic monotherapy (OM) at a state psychiatric hospital in Oregon (Oregon State Hospital). METHODS: Charts were reviewed for patients discharged between October 20, 2011, and September 23, 2015, with a diagnosis of schizophrenia spectrum or mood disorder. Admission dates, discharge dates, and discharge antipsychotics were reviewed for each pa

Hospital variation of 30-day readmission rate following transcatheter aortic valve implantation.

OBJECTIVES: Thirty-day readmission rate is one of the hospital quality metrics. Outcomes of transcatheter aortic valve implantation (TAVI) have improved significantly, but it remains unclear whether hospital-level variance in 30-day readmission rate exists in the contemporary TAVI era. METHODS: From the 2017 US Nationwide Readmission Database, endovascular TAVI were identified. The unadjusted 30-day readmission rate and 30-day risk-standardised readmission rate (RSRR) were calculated and we then conducted model testing to determine the relative contribution of hospital characteristics, patien

Effectiveness of digital health technologies for post-discharge follow-up and management in older adults: a systematic review.

Older adults (≥65 years) are a rapidly growing population that are experiencing a higher number of hospitalisation admissions, longer hospital stays, and greater hospitalisation-related costs than younger adults. There is an important gap in post-discharge care for older adults, and digital technologies, such as video visits, mobile health apps, and remote patient monitoring, may support follow-up and management after hospital discharge. This systematic review examined the effectiveness, feasibility, acceptability, and impact (ie, effects on rehospitalisation, quality of life, mental health, a

Predictors and implications of 30-day readmissions following craniotomy for brain tumors: A systematic review and meta-analysis.

Thirty-day readmission rates are commonly used as quality indicators (QIs) due to their feasibility and financial impact on healthcare systems. However, their validity in neurosurgical oncology remains uncertain. This study evaluates 30-day readmission rates following craniotomies for brain tumor resection, focusing on causes, predictors, and their impact on overall survival (OS). A systematic review and meta-analysis were conducted following PRISMA guidelines. PubMed, Cochrane, Scopus, and Web of Science databases were queried for studies reporting 30-day readmission rates in craniotomies for
