Overcoming Airway Hurdles: A Case Report of Anesthetic Challenges in Meningomyelocele Complications.

This case report delves into the anesthesia management in a 23-year-old male with complications of meningomyelocele, a catastrophic congenital neural tube defect. The patient, paraplegic since birth with severe scoliosis, presented with a femoral fracture, prompting the need for careful consideration of anesthesia strategies. The challenges included counseling the family on the risks and benefits of surgery under general anesthesia, selecting an appropriate anesthetic plan for an anticipated difficult airway, and addressing ventilation strategies for restrictive lung disease. To tackle the ant

Current Practices in Pediatric Dermatology Laser Therapy: An International Survey.

BACKGROUND AND OBJECTIVES: To date, there are no well-established guidelines regarding laser therapy for the treatment of cutaneous lesions in pediatric patients. We aim to ascertain the types of lasers commonly used, types of lesions treated, and factors that affect the selection of specific laser modalities in pediatric patients. STUDY DESIGN/MATERIALS AND METHODS: An anonymous online survey was distributed to healthcare providers who treat children with lasers through listservs of four major national and international dermatology and laser organizations. RESULTS: Outpatient office-based p

Expediting the reversal of inferior alveolar nerve block anesthesia in children with photobiomodulation therapy.

In pediatric dentistry, complications arising from extended soft tissue anesthesia can negatively impact patient comfort and trust in dental care. This study evaluates the clinical efficacy of diode laser-based photobiomodulation therapy (PBMT) in expediting the resolution of anesthesia in children aged 6-9 receiving inferior alveolar nerve block (IANB) injections. In this split-mouth double-blind randomized clinical trial, 36 pediatric subjects aged 6-9, requiring pulpotomy procedures on both sides of the mandible, received IANBs (single cartridge of 2% lidocaine/1:100,000 epinephrine). PBMT

How Effective are Inferior Alveolar Nerve Block and Supplemental Intraligamentary Injections in Pediatric Patients with Deep Carious Permanent Mandibular Molars?

Purpose: The purpose of this study was to assess, within the deep carious permanent mandibular molars of pediatric patients: (1) pre-operative pulpal anesthesia following an inferior alveolar nerve block (IANB); (2) preoperative pulpal anesthesia following a supplemental intraligamentary injection (SII); and (3) intraoperative pulpal anesthesia. Methods: Vital permanent mandibular molars with deep caries were first anesthetized with IANB. Preoperative pulpal anesthesia was assessed, and success was defined when the tooth had no response to the sensibility tests. In cases with failed preoperati

In Pursuit of an Opioid-Free Pediatric Ambulatory Surgery Center: A Quality Improvement Initiative.

BACKGROUND: Opioids have been a central component of routine adult and pediatric anesthesia for decades. However, the long-term effects of perioperative opioids are concerning. Recent studies show a 4.8%-6.5% incidence of persistent opioid use after surgery in older children and adults. This means that >2 million of the 50 million patients undergoing elective surgeries in the United States each year are likely to develop persistent opioid use. With this in mind, anesthesiologists at Bellevue Clinic and Surgery Center assembled an interdisciplinary quality improvement team focused on 2 goals: (…

Pediatric neuroanesthesia.

PURPOSE OF REVIEW: A series of recent studies have changed the practice of pediatric neuroanesthesia, improving outcomes and making children's quality of life better. RECENT FINDINGS: Potential long-term neurologic effects in infants and young children undergoing surgery and anesthesia have been recognized for over a decade. Several recent, well performed studies suggest that hypotension may also be a major contributor to postoperative neurologic impairment in children. Craniosynostosis surgery has also been the subject of extensive study, both related to decreasing blood loss and to optimizi

Septo-optic dysplasia with bilateral congenital corneal anesthesia.

Septo-optic dysplasia, or de Morsier syndrome, is characterized by optic nerve hypoplasia with an absent septum pellucidum and/or pituitary abnormalities. Congenital corneal anesthesia is a rare disorder that has been associated with many neurological disorders. Here we present a patient with both conditions who was successfully treated with permanent lateral tarsorrhaphy and aggressive lubrication. To our knowledge, congenital corneal anesthesia has not been reported in association with septo-optic dysplasia. The purpose of this report is to make pediatric ophthalmologists aware of a potentia

Tympanostomy Tube Placement and Auditory Brainstem Response Test Under Sedation.

Auditory brainstem response testing is typically done under sedation in the pediatric sedation unit but often requires tympanostomy tube placement, typically done under general anesthesia in the operating room (OR), to be performed first to obtain accurate results. This is the first report on the feasibility of performing tympanostomy tube placement in combination with auditory brainstem response testing under sedation provided by pediatric critical care medicine and hospitalist physicians outside the OR. A retrospective review was performed of cases at two children's hospitals in the United St

Pediatric inguinal hernia: open versus laparoscopic approaches to surgical management.

PURPOSE OF REVIEW: Inguinal hernia repair remains one of the most common pediatric surgical procedures. Advances in minimally invasive techniques have made laparoscopic herniorrhaphy a safe alternative to open surgery. This review summarizes clinical outcomes in open and laparoscopic pediatric inguinal hernia repair, discusses technical considerations including anesthetic choice, and reviews patient and surgical factors relevant to surgical approach and timing. RECENT FINDINGS: Clinical outcomes appear similar in open and laparoscopic pediatric hernia repairs. Open surgery can be performed un

[Progress in the application of drug-induced sleep endoscopy in pediatric OSA].

Drug-induced sleep endoscopy (DISE) is an endoscopic examination performed under conditions similar to human physiological sleep induced by drugs. In recent years, its clinical application has become increasingly widespread. This article reviews the research progress on the indications, anesthesia, and outcome determination of pediatric DISE, providing a basis for the application of pediatric DISE.

A Narrative Review Illustrating the Clinical Utility of Electroencephalogram-Guided Anesthesia Care in Children.

The major therapeutic end points of general anesthesia include hypnosis, amnesia, and immobility. There is a complex relationship between general anesthesia, responsiveness, hemodynamic stability, and reaction to noxious stimuli. This complexity is compounded in pediatric anesthesia, where clinicians manage children from a wide range of ages, developmental stages, and body sizes, with their concomitant differences in physiology and pharmacology. This renders anesthetic requirements difficult to predict based solely on a child's age, body weight, and vital signs. Electroencephalogram (EEG) moni…

Pattern of Usage of Supraglottic Airway Devices in Pediatric Patients among Anesthesiologists.

BACKGROUND: Supraglottic airway devices (SADs) have become integral to pediatric anesthesia, offering a less invasive alternative to endotracheal intubation with advantages in ease of insertion, hemodynamic stability, and reduced airway trauma. Various SADs, such as the classic laryngeal mask airway (LMA), ProSeal LMA, I-gel, and Ambu AuraOnce are available, each differing in design, seal pressure, and clinical indications. Despite their widespread use, practice patterns among anesthesiologists regarding SAD selection, insertion techniques, and perioperative management in pediatric patients va…

Full-mouth Rehabilitation of a Ventriculoperitoneal Shunt-treated Hydrocephalic Pediatric Patient: A Case Report.

Hydrocephalus is characterized by accumulation of cerebrospinal fluid in the lateral ventricles of the brain, which results in progressive ventricular dilatation and an increased intracranial pressure. A ventriculoperitoneal shunt (VPS) is the most common treatment for hydrocephalus. Delayed development, behavioral disturbance, chronological changes in the eruption of their teeth, changes in the occlusion, greater accumulation of plaque, and increased caries prevalence are some of the clinical manifestations in these patients, which are encountered by a pediatric dentist. Behavioral disturbanc

Quadratus Lumborum Blocks for Pediatric Bone Marrow Harvesting: A Case Report.

Bone marrow harvesting is a means to obtain stem cells to treat certain hematologic conditions in related or unrelated individuals. The most cited complication after bone marrow harvesting is surgical site pain. We developed a protocol incorporating regional anesthesia to improve pain control and reduce opioid use. A retrospective chart review was performed on three pediatric patients who underwent bone marrow harvesting for a sibling recipient and were managed via a standardized regional pain protocol. Each patient was treated with bilateral quadratus lumborum blocks in the operating room, pr

Lymphatic Malformation Treated with Lymphatic Malformation-Venous Anastomosis Under Local Anesthesia.

Lymphatic malformation (LM) can occur in the head and neck regions and cause cosmetic problems in adults. Sclerotherapy and surgical resection have been frequently applied; however, both are far from being minimally invasive in terms of aesthetic satisfaction, including the aesthetic downtime. We performed a less-invasive treatment using the venous anastomosis technique, named the lymphatic malformation-venous anastomosis (LMVA), mainly in pediatric patients with intractable microcystic lesions, in whom general anesthesia was required because the pediatric patients could not remain still. Here

Pediatric tracheostomy.

Tracheotomy refers to a surgical incision made into a trachea. Tracheostomy, on the other hand, refers to a surgical procedure whereby the tracheal lumen is positioned in close proximity to the skin surface. Tracheostomy is an uncommon procedure in the pediatric population. When required tracheostomy is typically performed as an open surgical procedure under general anesthesia with the patient intubated. However, it may need to be performed under local anesthesia or over a rigid bronchoscope in the patient with a precarious airway. Over the past half century, the primary indication for pediatr

Utilizing ultrasonography to determine the minimal transverse diameter of the subglottic airway for informed selection of reinforced cuffed endotracheal tube models in children.

OBJECTIVE: We explored the use of ultrasonography in determining the minimal transverse diameter of the subglottic airway (MTDSA) for the purpose of choosing an appropriate model of reinforced cuffed endotracheal tube. METHODS: A total of 110 pediatric patients who received general anesthesia and tracheal intubation for selective surgeries at the hospital from February 2019 to February 2022 were chosen. They were then randomly assigned to three groups: 39 in the MTDSA group, 35 in the age formula group, and 36 in the height formula group. We assessed how accurately the appropriate endotrachea

How do You Select an Anesthesia Method Prior to Tympanostomy Tube Insertion for a Child?

The use of general (face-mask inhalation and intravenous) anesthesia has been the method of choice for tympanostomy tube insertion in children. However, there is no exact guideline for the choice of anesthesia method and there is no evidence to support the use of one anesthesia method over another. Clinically, the anesthesia method used to be decided by old customs and the surgeon's blind faith that children cannot bear tympanostomy tube insertion under local anesthesia. Clinicians should keep in mind that pediatric anesthesia has a potential risk. Despite infrequent serious complications, the…
