[Trends in antenatal diagnosis, pregnancy termination and perinatal mortality in infants with congenital heart disease: evaluation in the general population of Paris 1983-2000].

OBJECTIVE: To examine population-based overall and malformation-specific trends in the prenatal diagnosis, pregnancy termination, and perinatal mortality for congenital heart disease (CHD) during a period of rapid progress in prenatal diagnosis and medical management of CHD and to explore the impact of prenatal diagnosis on early neonatal mortality for specific (isolated) cardiac malformations. METHODS: A total of 1982 cases of CHD, which were not associated with a known chromosomal anomaly, were obtained from the Paris Registry of Congenital Malformations. Main outcome measures were trends i…

The paradoxical effect of smoking in preeclamptic pregnancies: smoking reduces the incidence but increases the rates of perinatal mortality, abruptio placentae, and intrauterine growth restriction.

OBJECTIVES: Smoking is associated with a reduced risk of preeclampsia, but what is the outcome of pregnancy when preeclampsia develops in wo … severe preeclampsia, smoking at least 10 cigarettes per day was associated with increased rates of perinatal mortality (from 24 to 36 per 1000), abruptio placentae (from 31 to 67 per 1000), and being small for gestational age (from 28% to 68%), whereas the corresponding smoking-related increases in rates in nonhypertensive pregnancies were considerably less. In-hospital mortality and associated factors among term neonates with documented perinatal asphyxia in eastern Democratic Republic of the Congo: a retrospective cohort study.

BACKGROUND: Perinatal asphyxia remains an important contributor to neonatal mortality and neurodevelopmental impairment, particularly in resource-limited settings, yet contemporary evidence from eastern Democratic Republic of the Congo (DRC) is scarce. We assessed the hospital burden, clinical severity, and factors associated with in-hospital mortality among affected neonates admitted to a tertiary neonatal intensive care unit (NICU). METHODS: We conducted a retrospective cohort study at the Hôpital Provincial Général de Référence de Bukavu, eastern DRC, including neonates with documented per

Temporal trends in fetal mortality at and beyond term and induction of labor in Germany 2005-2012: data from German routine perinatal monitoring.

PURPOSE: While a variety of factors may play a role in fetal and neonatal deaths, postmaturity as a cause of stillbirth remains a topic of debate. It still is unclear, whether induction of labor at a particular gestational age may prevent fetal deaths. METHODS: A multidisciplinary working group was granted access to the most recent set of relevant German routine perinatal data, comprising all 5,291,011 hospital births from 2005 to 2012. We analyzed correlations in rates of induction of labor (IOL), perinatal mortality (in particular stillbirths) at different gestational ages, and fetal morbid…

Mortality in perinatally HIV-infected young people in England following transition to adult care: an HIV Young Persons Network (HYPNet) audit.

OBJECTIVES: Mortality in young people with perinatally acquired HIV infection (PHIV) following transfer to adult care has not been characterized in the UK. We conducted a multicentre audit to establish the number of deaths and associated factors. METHODS: Fourteen adult clinics caring for infected young people reported deaths to 30 September 2011 on a proforma. Deaths were matched to the Collaborative HIV Paediatric Study, a clinical database of HIV-infected children in the UK/Ireland, to describe clinical characteristics in paediatric care of those who died post-transition. RESULTS: Eleven

A prospective observational study of early fetal growth velocity and its association with birth weight, gestational age at delivery, preeclampsia, and perinatal mortality.

OBJECTIVES: We aimed to measure early fetal growth velocity and to correlate this with the birth weight, gestational age at delivery, and with the incidence of adverse pregnancy outcomes specifically preeclampsia and perinatal mortality. METHODS: A data based prospective observational study, wherein sonographic biometry data and specific pregnancy outcome related data were collected from pregnant women's records, starting soon after their first antenatal visit. Early fetal growth velocity was measured using BPD growth between 11 and 14 weeks scan and anomaly scan and standardizing this by Z s…

Perinatal morbidity and mortality in early-onset fetal growth restriction: cohort outcomes of the trial of randomized umbilical and fetal flow in Europe (TRUFFLE).

OBJECTIVES: Few data exist for counseling and perinatal management of women after an antenatal diagnosis of early-onset fetal growth restriction. Yet, the consequences of preterm delivery and its attendant morbidity for both mother and baby are far reaching. The objective of this study was to describe perinatal morbidity and mortality following early-onset fetal growth restriction based on time of antenatal diagnosis and delivery. METHODS: We report cohort outcomes for a prospective multicenter randomized management study of fetal growth restriction (Trial of Randomized Umbilical and Fetal Fl

Decrease in hospitalization and mortality rates among children with perinatally acquired HIV type 1 infection receiving highly active antiretroviral therapy.

BACKGROUND: The impact of highly active antiretroviral therapy (HAART) on human immunodeficiency virus type 1 (HIV-1) disease progression in perinatally infected children is not well documented. This study aims to identify the effect of evolving antiretroviral therapy on the immunologic and virologic status of and hospitalization and mortality rates among perinatally infected children. METHODS: Children receiving outpatient care during 1994-2001 at 3 HIV clinics in southern California were observed longitudinally for CD4+ cell percentage, plasma HIV-1 RNA load, antiretroviral treatment, Pneum

[Epstein-Barr virus infections in pregnancy. The serological profile of active infections in pregnant women. Their possible relationship with perinatal mortality (author's transl)].

A significant number of cases of deaths and of perinatal misfortunes are still of unknown origin. The role of pathological infection is probably important in these cases and particularly those in the Torch group of viruses, and among these, as has been shown by epidemiological and virological tests, the Epstein-Barr virus (E.B.V.). The authors have undertaken a prospective study based on serology, looking for E.B.V. antibodies in pregnancy. 1,164 women were studied and 74 of these had perinatal abnormalities. There is a significant relationship between active E.B.V. infection and mishaps in pr

The Global Network Socioeconomic Status Index as a predictor of stillbirths, perinatal mortality, and neonatal mortality in rural communities in low and lower middle income country sites of the Global Network for Women's and Children's Health Research.

BACKGROUND: Globally, socioeconomic status (SES) is an important health determinant across a range of health conditions and diseases. However, measuring SES within low- and middle-income countries (LMICs) can be particularly challenging given the variation and diversity of LMIC populations. OBJECTIVE: The current study investigates whether maternal SES as assessed by the newly developed Global Network-SES Index is associated with pregnancy outcomes (stillbirths, perinatal mortality, and neonatal mortality) in six LMICs: Democratic Republic of the Congo, Guatemala, India, Kenya, Pakistan, and …

Perinatal mortality associated with induction of labour versus expectant management in nulliparous women aged 35 years or over: An English national cohort study.

BACKGROUND: A recent randomised controlled trial (RCT) demonstrated that induction of labour at 39 weeks of gestational age has no short-term adverse effect on the mother or infant among nulliparous women aged ≥35 years. However, the trial was underpowered to address the effect of routine induction of labour on the risk of perinatal death. We aimed to determine the association between induction of labour at ≥39 weeks and the risk of perinatal mortality among nulliparous women aged ≥35 years. METHODS AND FINDINGS: We used English Hospital Episode Statistics (HES) data collected between April 2…

External causes are leading causes of death in women of reproductive age: a registry study on maternal perinatal health, hypertensive pregnancy disorders and mortality in Finland.

BACKGROUND: There is a known association between pre-eclampsia (PE) and other hypertensive disorders of pregnancy (HDP), and increased risk of cardiovascular diseases (CVD). Reproductive history is associated with maternal mortality. We studied the causes of death in women of reproductive age and how a history of HDP affects mortality. METHODS: We collected and analysed the perinatal data of national registers in a study of 555 345 women born in Finland during 1966-1990. The follow-up started from the woman's first birth and ended on the first CVD, death or at the end of the follow-up of 23 y

Effect of caesarean birth on perinatal mortality for singleton breech presentation in spontaneous preterm labour-A target trial emulation using Scottish health record data.

BACKGROUND: The effects of mode of birth for women in preterm breech labour could not be successfully determined in randomised trials. We aimed to explore the effect of caesarean birth on perinatal mortality for women in spontaneous-onset preterm labour with a singleton baby presenting breech through target trial emulation. METHODS: A target trial emulation of a parallel group randomised controlled trial using routinely collected Scottish electronic health record data was performed. Participants were pregnant women at 24-36 gestational weeks with a singleton breech baby, no prior caesarean bi…

One year experience of neonatal mortality and morbidity in a state level neonatal intensive care unit and its comparison with national neonatal-perinatal database.

Neonatal mortality is still the leading group among the infant mortality in India. Poor infrastructure, home delivery and lack of facility, trained personnel in rural hospital are the major determining factors leading to neonatal mortality and morbidity including perinatal asphyxia. The data, which is gathered from a tertiary neonatal intensive care unit (NICU) of West Bengal and its statistical interpretation, has thrown light upon the current state of neonatal care in our county. Encouragement for institutional delivery, routine use of blood culture and sensitivity and standard antibiotic po

To guide future practice, perinatal trials should be much larger, simpler and less fragile with close to 100% ascertainment of mortality and other key outcomes.

The Australian Placental Transfusion Study (APTS) randomised 1,634 fetuses to delayed (≥60 s) versus immediate (≤10 s) clamping of the umbilical cord. Systematic reviews with meta-analyses, including this and similar trials, show that delaying clamping in preterm infants reduces mortality and need for blood transfusions. Amongst 1,531 infants in APTS followed up at two years, aiming to delay clamping for 60 s or more reduced the relative risk of the primary composite outcome of death or disability by 17% (p = 0.01). However, this result is fragile because nominal statistical significance (p <
