Analysis of the pathophysiology of hypokalemic paralysis, as it occurs in barium poisoning, chronic potassium deficiency, and thyrotoxicosis, suggests that these disorders may have a similar mechanism. An increased ratio of muscle sodium permeability to potassium permeability reduces the ionic diffusion potential, while the resting membrane potential is sustained by an increase of Na-K pump electrogenesis. The result is that potassium entry (the sum of active and passive influx) exceeds potassium efflux; this causes a large shift of extracellular potassium into muscle until the Na-K pump turns

Electrocardiographic conduction disturbances and nonspecific interval changes are not uncommon during hypokalemic periodic paralysis. Although premature ventricular contractions may occur during the paretic episodes associated with hyperkalemic, normokalemic, and hypokalemic periodic paralysis, ventricular tachycardia is rare; to my knowledge, ventricular fibrillation has not been previously reported. In 1977 a 40-year-old man of Puerto Rican descent was resuscitated from ventricular fibrillation and subsequently received a diagnosis of thyrotoxic hypokalemic periodic paralysis. Following thyr

The cases of a 14-year-old farm labourer is presented as a further example of nonacetazolamide responsive hypokalaemic periodic paralysis. The family history was negative, the clinical picture classical, with prompt reversal of symptoms after the use of potassium salts. However, the administration of acetazolamide led to precipitation of an attack within a few days. The use of triamterene 100mg daily resulted in complete cessation of the attacks for the last 12 months with the maintenance of normal serum potassium levels without the use of supplementary potassium.

Although acetazolamide usually prevents paralytic attacks in hypokalemic periodic paralysis, not all patients benefit from this treatment. We studied a father and two sons in whom attack frequency and severity increased on acetazolamide. Administration of triamterene virtually abolished attacks in three separate single-blind trials totaling more than 12 months. Spontaneous and glucose-insulin provoked occurred with only slight hypokalemia. Acetazolamide produced slight hypokalemia and provoked attacks of weakness whereas triamterene increased potassium levels significantly. Certain patients wi

In an effort to determine whether the refractory period in exercise-induced asthma derived from mediator consumption we had seven asthmatic subjects repeatedly perform both exercise and eucapnic hyperventilation at matched minute ventilations under precisely controlled inspired air conditions. We reasoned that, if airway cooling were causing an agent to be released whose depletion resulted in less responsiveness, we should be able to observe this phenomenon irrespective of how cooling was produced. Repetitive exercise at short intervals produced a diminution in the obstructive response that di

A post-mature female newborn with a history of birth trauma developed frequent apneic spells coincident with left posterior-quadrant alpha d … the anticonvulsant at eight months of age. The authors relate her favorable course to the etiology, periodicity and therapeutic response of her clinical and electrical seizures.

Experience during the last three years with nasotracheal intubation in children with acute epiglottitis indicates that a period of intubation of 8-12 hours after the institution of appropriate antibiotic therapy is satisfactory in most patients. Shortening the period of intubation should have benefits to the patient. A shorter period of intubation should provide less opportunity for complications to develop and allow earlier discharge.

Generalized and focal periodic EEG patterns were studied in 62 patients. Generalized periodic suppression bursts and generalized periodic sl … inical seizures in 24 cases and were noted almost exclusively in destructive hemispherical lesions. Periodicity represents a profound disruption of normal electrophysiological rhythms and indicates either structural or functional involvement of cortical and subcortical structures.

Two infants with idiopathic panhypopituitarism presented with severe neonatal hypoglycaemia, hepatomegaly and hyperbilirubinaemia (direct and indirect). Abnormal liver function tests returned to normal over a 5--8 month period. The growth rate in the absence of detectable growth hormone was 50% of normal during the first 6 months. The effect of growth hormone on somatomedin levels and growth rate during the first year of life in one of the infants is described.

The syndromes of Pickwickian, Ondine's curse, and primary alveolar hypoventilation are respiratory disorders manifesting increased sleepiness and irregular respiratory rhythms. These disorders are currently grouped as hypersomnia with periodic breathing (HPB). Polygraphic techniques have lead to a reasonable hypothesis as to the pathophysiology of the multiple variants of HPB. Discernible causes of HPB have been attributed to both central and peripheral factors. Peripheral factors encompass those conditions relating to upper airway obstruction. An acromegalic person suffering the HPB syndrome

To combat gastroplegia following vagotomy the authors recommend to perform open novocain blockade of the solar plexus by means of a microcatheter, inserted in the round ligament of the liver. The application of the blockade enabled the authors to manage the postoperative period without a sound, to reduce fluids and electrolytes loss by the organism and to minimize the use of anesthetics.
