We report the case of a 49-year-old patient with megalocornea and coexisting corneal astigmatism. The corneal diameter in the right eye was 15.0 mm and in the left eye, 14.9 mm. In both eyes, a nuclear sclerotic cataract developed, with the tendency toward cortical mass swelling in the right eye. The aim of surgical treatment was to remove the cataract with simultaneous correction of corneal astigmatism by implanting an Acrysof toric intraocular lens (IOL). Intraocular lens stabilization was obtained by suturing it to an capsular tension ring (CTR) in the anterior chamber. The IOL-CTR complex

The objective was to describe a new surgical technique to manage a posteriorly dislocated crystalline lens. Four patients with posteriorly dislocated lenses were studied. Two patients had dislocated lenses secondary to trauma, 1 had undergone retinal detachment surgery, and 1 had an idiopathic lens dislocation. Pars plana vitrectomy was carried out on all 4 eyes, followed by an injection of perfluoro-n-octane to float the lens off the retina. The lens was phacoemulsified through a limbal incision, and an intraocular lens was positioned in the ciliary sulcus with suture fixation. Perfluoro-n-oc

Advanced primary open-angle glaucoma (POAG) requires surgical strategies that achieve effective reduction of intraocular pressure (IOP) while preserving optic nerve structure and function. This study aims to compare the long-term clinical performance of ab externo canaloplasty (AC) alone versus AC combined with phacoemulsification (AC + phaco) in patients with advanced POAG. In this retrospective comparative study, 140 patients with advanced POAG were assigned to AC (n = 70) or AC + phaco (n = 70). Over 24 months of follow-up, both procedures significantly reduced IOP and medication use. Howev…
