Advancing Aerobic Exercise Training Intensity Prescription in Health and Disease Beyond Standard Recommendations: A Call to Action.

Guidelines for aerobic exercise training (AET) often recommend training intensities on the basis of a percentage of peak effort (e.g. %peak heart rate [%HRpeak], %peak oxygen uptake [%VO2peak]). However, such an approach could be associated with a higher inter-individual variability in acutely induced physiological responses (e.g. blood lactate, HR, VO2) when compared with threshold-based prescription (i.e. ventilatory threshold [VT] or lactate threshold [LT]). Therefore, we performed a literature review to compare the acute physiological impact of AET prescription based on fraction of the pea

Left ventricular endocardial pacing is less arrhythmogenic than conventional epicardial pacing when pacing in proximity to scar.

BACKGROUND: Epicardial pacing increases risk of ventricular tachycardia (VT) in patients with ischemic cardiomyopathy (ICM) when pacing in proximity to scar. Endocardial pacing may be less arrhythmogenic as it preserves the physiological sequences of activation and repolarization. OBJECTIVE: The purpose of this study was to determine the relative arrhythmogenic risk of endocardial compared to epicardial pacing, and the role of the transmural gradient of action potential duration (APD) and pacing location relative to scar on arrhythmogenic risk during endocardial pacing. METHODS: Computationa

Bayesian Population Physiologically-Based Pharmacokinetic (PBPK) Approach for a Physiologically Realistic Characterization of Interindividual Variability in Clinically Relevant Populations.

Interindividual variability in anatomical and physiological properties results in significant differences in drug pharmacokinetics. The consideration of such pharmacokinetic variability supports optimal drug efficacy and safety for each single individual, e.g. by identification of individual-specific dosings. One clear objective in clinical drug development is therefore a thorough characterization of the physiological sources of interindividual variability. In this work, we present a Bayesian population physiologically-based pharmacokinetic (PBPK) approach for the mechanistically and physiolog

Relationship between pacemaker dependency and the effect of pacing mode on cardiovascular outcomes.

BACKGROUND: A recently completed trial, the Canadian Trial of Physiological Pacing (CTOPP), showed that physiological pacing did not significantly reduce mortality, stroke, or heart failure hospitalization, but it did show that atrial fibrillation occurred less frequently in patients with physiological pacing. Many pacemaker patients experience only transient bradyarrhythmias with an adequate unpaced heart rate (UHR) and are not pacemaker-dependent. The purpose of the present analysis was to determine if pacemaker-dependent patients have an increased benefit from physiological pacing compared

Clustering trunk movements of children and adolescents with neurological gait disorders undergoing robot-assisted gait therapy: the functional ability determines if actuated pelvis movements are clinically useful.

INTRODUCTION: Robot-assisted gait therapy is frequently used for gait therapy in children and adolescents but has been shown to limit the physiological excursions of the trunk and pelvis. Actuated pelvis movements might support more physiological trunk patterns during robot-assisted training. However, not every patient is expected to react identically to actuated pelvis movements. Therefore, the aim of the present study was to identify different trunk movement patterns with and without actuated pelvis movements and compare them based on their similarity to the physiological gait pattern. Impact of physiologically shaped pancreatic stent for chronic pancreatitis.

Endoscopic pancreatic stenting is used to prevent main pancreatic duct obstruction and relieve painful symptoms of chronic pancreatitis. However, the stent typically needs to be exchanged and the rate of adverse events is high. Few studies have evaluated the effect of stent shape on those outcomes. We evaluated the adverse events, stent patency, and total medical cost within 90 days of patients who received an 8.5 French (Fr) physiologically shaped pancreatic stent by comparing these features with those associated with a conventional straight-type stent for ≥ 90 days. The total stent-related a

A histologic comparison of Hydron and zinc oxide-eugenol as endodontic filling materials in the primary teeth of dogs.

Currently available endodontic filling materials for primary teeth demonstrate several inadequacies. Therefore, this study histologically evaluates Hydron (2-hydroxyethylmethacrylate) and zinc oxide and eugenol (ZOE) as primary tooth endodontic filling materials in dogs to compare the materials' biocompatibility and to determine whether they undergo resorption in a physiologic manner. Primary molars of six mongrel dogs, 2 to 3 months old, received one-step endodontic therapy. By random selection, two thirds of the molars were filled with either Hydron or ZOE and the remainder were used as unop

A Ducted, Biomimetic Nipple Improves Aspects of Infant Feeding Physiology and Performance in an Animal Model.

Breastfeeding is widely regarded as the optimal form of feeding infants, as it provides both nutritional and physiological benefits. For example, breastfed infants generate greater intraoral suction and have higher amplitude muscle activities compared to bottle-fed infants, with downstream implications for motor function, development, and health. One mechanism that might explain these physiological differences is the structure of the nipple an infant is feeding on. Breasts in most mammals are ducted soft-tissue structures that require suction to be generated for milk to be released, whereas bo

The role of lactulose in the prevention of bacterial translocation in surgical trauma.

Surgical trauma (ST) is one of the causative factor of bacterial translocation. In this study we investigated the prevention of bacterial translocation with lactulose in a surgical trauma model. The study was designed in 3 experimental groups consisting of 15 rats in each. Group 1 was sham operated controls, group 2 was ST + physiologic saline treated and group 3 was ST + lactulose treated animals. Lactulose and physiologic saline were given by oro-gastric intubation in a dose of 2 ml of 33.5% solution/d and 2 ml/d respectively starting 3 days prior to surgery. Bacterial translocation was inve

Pathophysiology, monitoring, and therapy of acute circulatory problems.

Acute circulatory problems, including shock, traditionally have been recognized and treated by subjective symptoms and vital signs (BP and HR) which represent only superficial aspects of the underlying physiology. Shock, however, is objectively diagnosed, evaluated, and treated using invasive physiologic monitoring to measure cardiac function and output, pulmonary function, and tissue perfusion. Multicomponent noninvasive monitoring systems are now being developed to obtain comparable information and provide continuous display of data on-line, in real time. These monitoring systems may be used

Synthetic oxytocin prophylaxis vs physiologic third-stage care for postpartum hemorrhage prevention in physiologic births: A population-based cohort study (2015-2022).

OBJECTIVE: To describe third-stage care practices among midwives in Ontario, Canada and estimate the causal effect of synthetic oxytocin prophylaxis (synOT) vs physiologic third-stage care on postpartum hemorrhage (PPH) and severe PPH following physiologic birth. METHODS: This population-based cohort study included low-risk individuals in Ontario (2015-2022) with a physiologic birth who reached the third stage under midwifery care. Severe PPH was operationalized as PPH requiring transfusion. Adjusted relative risks (aRR) and 95% CI were estimated using modified Poisson regression with inverse

Distribution of cardiac output and oxygen delivery in an acute animal model of single-ventricle physiology.

BACKGROUND: When single-ventricle physiology is established acutely (ie, after a Norwood procedure), the combination of limited cardiac output and hypoxemia could result in limited oxygen transport to systemic organs. This study investigates the regional distribution of cardiac output and oxygen delivery after creation of single-ventricle physiology. METHODS: Single-ventricle physiology was created in 8 piglets, and 8 other piglets served as sham control animals. Aortopulmonary shunt, echocardiography-guided atrial septostomy, tricuspid valve avulsion, and pulmonary artery occlusion allowed t

Comparison of changes in diastolic functions after aortic valve replacement with freestyle stentless porcine xenografts in patients with restrictive and nonrestrictive physiology.

In this study, the effect of valve replacement on diastolic parameters was evaluated preoperatively and postoperatively at 3, 6, and 12 months by comparing diastolic parameters in patients after aortic valve replacement with freestyle stentless porcine xenografts for aortic stenosis. Depending on deceleration time (DT) and isovolumetric relaxation time (IVRT) with preoperative echocardiographic assessment, patients were divided into two groups: restrictive physiology (DT < or = 150 msec and IVRT < 100 msec, 20 patients), and nonrestrictive physiology (DT > 150 msec and IVRT > or = 100 msec, 27

PhysioJEPA: Joint Embedding Representations of Physiological Signals for Real Time Risk Estimation in the Intensive Care Unit.

Self-supervised learning of multi-modal, high-frequency physiological signals is largely unexplored, despite its potential for critical care applications. We present PhysioJEPA, a Joint Embedding Predictive Architecture (JEPA) designed for multi-modal physiological signals from critical care bedside monitoring devices. PhysioJEPA learns representations from 30-minute segments of physiological signals from three channels: arterial blood pressure, electrocardiography lead II, and photoplethysmography. Trained on over 10.7 million minutes of data from 4,282 intensive care unit stays (N=2,631 pati

The Effect of the Cervical Orthosis on Swallowing Physiology and Cervical Spine Motion During Swallowing.

Cervical orthosis is used to immobilize the neck in various disorders such as trauma and post-operation. However, it is still uncertain how cervical orthosis restricts the degree of movement of the cervical spine during swallowing and how they affect swallowing physiology. The purpose of this study was to evaluate these issues using the Philadelphia(®) Collar. We conducted videofluorography of swallowing in 39 healthy subjects (23 men, 16 women; mean age of 34.3 years) with and without cervical orthosis. To compare the two conditions regarding the cervical spine motion, we determined the angul
