Primary mucinous carcinoid tumor of the ovary.

A case of primary mucinous carcinoid tumor of the ovary occurring in a 37-year-old woman is described. The tumor, which replaced the left ovary, was accompanied by metastases in the contralateral ovary and para-aortic lymph nodes. Careful investigations excluded metastatic origin of the tumor. The tumor was solid, but contained an epidermoid cyst. There were no other teratomatous elements. The patient was treated with combination chemotherapy. A second-look operation 9 months following diagnosis revealed extensive microscopic involvement of the peritoneal cavity. The patient's condition deteri

Exfoliative cytology of clear-cell sarcoma metastases in pleural fluid.

Two cases of clear-cell sarcoma arising from the leg with widespread metastasis were reported. Exfoliated tumor cells in pleural effusions were round, oval, and slightly pleomorphic. They were seen singly or in clusters of variable size with marked cellular cohesiveness and molding. Their cytoplasm was well-defined, clear, or finely granular, and contained S-100 protein and periodic acid-Schiff positive material that was digestible by diastase. Tumor-cell nuclei were round and hyperchromatic with irregular chromatin clumping, and showed conspicuous nucleoli.

Extrapleural pneumonectomy for pleural malignancies.

Although originally performed on patients with refractory tuberculosis and malignant pleural mesothelioma, extrapleural pneumonectomy may be used to treat patients with pleural dissemination of other malignancies, including thymoma and NSCLC. Patients who present with stage IV NSCLC caused by malignant pleural effusion may be considered for EPP following induction chemotherapy if they demonstrate no mediastinal nodal or distant metastases and have adequate cardiopulmonary reserve. EPP for NSCLC should be performed by experienced teams at experienced centers to minimize the morbidity and mortal

[Malignant pleural mesothelioma].

Malignant pleural mesothelioma (MMP) is a highly aggressive tumor arising of the pleural mesothelium. Asbestos exposure is the main factor involved in the pathogenesis of MMP and according to the late ban of this agent in 2005 the peak incidence in Europe is expected in the next twenty years. The highly aggressive behaviour of this tumor results in a poor prognosis with a mean overall survival between 7 and 9 months. Despite the progress made in diagnosis and therapy of this entity the optimal treatment remains a subject of debate. In this article we review the current state of treatment and d

[Diagnosis of pleurisy].

Nowadays, the diagnosis of pleural effusion is greatly facilitated by thoracoscopy. Since most pleural effusions are due to neoplasias (notably in patients older than 40 years) and since the prognostic value of an early diagnosis is obvious in case of pleural carcinoma (notably mesothelioma), there is no justification in allowing the effusion to become chronic. If the diagnosis is confirmed by needle biopsy, thoracoscopy can rapidly be performed by skilled operators: in 95 p. 100 of the cases it provides the diagnosis. Five per cent of pleural effusions remain of unknown origin. In such cases

[Considerations on pleural puncturo biopsy, based on the results of an analysis of 100 samples].

Results are given on 100 cases in which pleural bioptic puncture (PBP) has been performed for diagnosis purposes. The data given in the study show the large possibilities to apply this method, readily acceptable by patients. The high diagnosis value of PBP method in based on strictly objective criteria. Stress is laid upon the contribution of the method in precising the etiology of pleural fluids, which, up to the application of PBP, were labelled as having an "undetermined" etiology. The right therapeutic measures to be taken on precise diagnosis are also stressed in the study.

The Role of CEUS in the Diagnosis and Follow-Up of Pleuropulmonary Diseases and Interventional Procedures.

Background: Contrast-enhanced ultrasound (CEUS) recently emerged as a valuable imaging modality for evaluating pleuropulmonary diseases. By combining morphological information from conventional B-mode ultrasound with real-time assessment of microvascular perfusion, CEUS can provide functional insights that improve diagnostic accuracy, guide interventions, and support patient surveillance. Methods: This review summarizes the current evidence on the use of CEUS in major pleuropulmonary disorders, including pneumonia, pleural effusion, pulmonary embolism, neoplasms, and COVID-19-related lung inju

Difficulty in positive diagnosis of ascites and in differential diagnosis of a pulmonary tumor.

Ascites is the most frequent complication of cirrhosis and occurs only when the portal hypertension has already installed but ascites is caused by neoplasms, heart failure, tuberculosis, pancreatic illnesses, as well as other kind of affections. We describe the case of a 67-year-old patient, a retired person, without significant personal or familial history, nonsmoker, infrequent alcohol and coffee consumer with following chief complaints at onset: loss of appetite, weight loss, serious physical asthenia, delayed intestinal transit, diffuse abdominal pain and increase of abdominal circumferenc

Pleuropulmonary desmoid tumors: immunohistochemical comparison with solitary fibrous tumors and assessment of beta-catenin and cyclin D1 expression.

CONTEXT: Desmoid tumors arising in the lung and pleura are extremely rare and can resemble other, more common neoplasms native to these sites. Alterations of the adenomatous polyposis coli/beta-catenin pathway have been detected in sporadic desmoid tumors and have been associated with nuclear accumulation of beta-catenin and overexpression of cyclin D1. OBJECTIVE: To analyze the expression of beta-catenin and cyclin D1 in desmoid tumors and solitary fibrous tumors (SFTs), and to compare the utilities of these substances for distinguishing between these entities with those of other, more commo

[Identification and characterization of microRNA involving in malignant pleural mesothelioma].

One of the research areas of modern medicine is to work on the identification of biological markers, such as biomolecular ones, for neoplastic diseases from occupational origin. MiRNA, short RNA no-codifing sequences, are recently identified such as diagnostic markers in several type of cancer. For this reason, the aim of our study is to analyze the possible role of miRNA in malignant pleural mesothelioma, a rare and aggressive tumor with a strong resistance to conventional therapies and poor prognosis. Total RNA, containing also miRNA, was extracted, and RNA was retro-transcripted with specif

Target Therapy in Malignant Pleural Mesothelioma: Hope or Mirage?

Malignant Pleural Mesothelioma (MPM) is a rare neoplasm that is typically diagnosed in a locally advanced stage, making it not eligible for radical surgery and requiring systemic treatment. Chemotherapy with platinum compounds and pemetrexed has been the only approved standard of care for approximately 20 years, without any relevant therapeutic advance until the introduction of immune checkpoint inhibitors. Nevertheless, the prognosis remains poor, with an average survival of only 18 months. Thanks to a better understanding of the molecular mechanisms underlying tumor biology, targeted therapy

Mysterious quad of constrictive pericarditis, recurrent pleural effusions, bone involvement and interstitial lung disease.

Erdheim-Chester disease (ECD) is a rare multisystemic non-Langerhans cell histiocytic neoplasm. The rarity of the disease and heterogenous clinical presentations often leads to delayed diagnosis. Historically, ECD lacked effective treatment and the prognosis was poor. Following the recent discovery of frequent BRAF-V600E mutation in patients with ECD, vemurafenib, a selective BRAF V600 kinase inhibitor has been approved for BRAF-mutated ECD patients. The prognosis of ECD has dramatically improved with early recognition of the disease and available treatment. ECD affects nearly every organ syst
