Postresectional pulmonary edema is a rare but potentially fatal complication of thoracic operations. In a retrospective study of 402 lung resections we have identified 11 cases of postresectional, noncardiogenic pulmonary edema. We have analyzed the individual data to test recognized hypotheses regarding this condition. Pulmonary edema occurred in 5.1% of right pneumonectomies, 4.0% of left pneumonectomies, and 1% of all lobectomies. In 2 patients the symptoms occurred immediately after operation; in the other 9 the mean interval to diagnosis was 43.4 hours. All 11 patients were in a positive

With the increasing applicability of cardiac surgical procedures, particularly in older age groups, increasing numbers of patients will be encountered with concomitant diseases amenable to surgical treatment.(1) We present three patients who underwent pulmonary resection associated with various cardiac surgical procedures. One patient had severe bron-chiectasis and mitral stenosis requiring mitral valve replacement with pneumonectomy. The other two patients had coronary artery disease and pulmonary neoplasms requiring pneumonectomy and bilobectomy concomitant with coronary bypass procedures. A…

Traumatic pulmonary artery and intercostal artery pseudoaneurysms are rare pathologies that can occur after a penetrating injury. Most times, only 1 pseudoaneurysm needs to be addressed. Options for management include simple observation, endovascular intervention with coiling, embolization, and stenting, as well as surgical intervention ranging from ligation to pneumonectomy. We present the case of a 20-year-old man who developed multiple pulmonary artery pseudoaneurysms and an intercostal artery pseudoaneurysm after sustaining a single gunshot wound to the chest. After multiple episodes of bl…

The complications following surgery for lung cancer vary depending upon the comorbidities and the type of surgery. Hemorrhage, infections and pulmonary edemas are not specific to the type of resection but frequently occur following pneumonectomies. Morbidity following pneumonectomies is related to the significant changes in the contents of the intrathoracic space. Pulmonary infarction and torsion are emergency situations that develop following lobectomy. CT shows features of localized congestion and stenosis or occlusion of a vein or bronchus. Rapid identification of severe events, in particul

A novel method for closure of the lower membranous trachea after right pneumonectomy using a flap derived from the cartilaginous portion of the right main bronchus is described in this study. This technique was used successfully in patients with tracheal stenosis due to a giant posterior mediastinal tumor known as schwannoma. Because of the severe tracheobronchial stenosis and destroyed right lung, tumor resection combined with resection of the lower membranous trachea and right pneumonectomy was carried out. We closed the defect in the membranous lower trachea with the flap derived from the r…

We report on the surgical treatment of a patient in whom hydatid cysts inside the right pulmonary artery and multiple right lung involvement were detected. Since the right pneumonectomy carried a high risk of cyst rupture, and migration of the cysts to the opposite lung during ligation of the pulmonary artery, a two-stage surgical approach was scheduled. Hydatid cysts located at the proximal pulmonary artery were removed by performing a longitudinal arteriotomy along the pulmonary artery to the bifurcation. Subsequently, a right pneumonectomy was performed on a safe right pulmonary artery in a…

Thoracotomy is not infrequently performed in patients with suspected pulmonary carcinoma but with no histological or cytological confirmation of malignancy. The intraoperative decision to proceed with major pulmonary resection (lobectomy or pneumonectomy) is difficult if a large or central lesion precludes total excisional biopsy. Incisional or needle biopsies violate the principles of good cancer surgery, and the results may be inconclusive if the tumor is missed and areas of associated inflammation or necrosis are sampled. Between January 1, 1970, and December 31, 1980, 303 patients underwen…

A paradoxical embolus associated with orthodeoxia-platypnea syndrome and intracardiac shunting is extremely uncommon. We present a patient who was found to have a positional change in desaturation after a right pneumonectomy who suffered from gangrene of the right foot and simultaneous deep venous thrombosis of the left arm. Workup revealed a patent foramen ovale as a cause for both the right-to-left shunt and the paradoxical emboli. After percutaneous closure the orthodeoxia resolved. This case highlights the necessity of heightened awareness of this syndrome in case of severe hypoxemia after…

Neoadjuvant chemotherapy combined with immunotherapy is a crucial treatment modality for patients with resectable non-small cell lung cancer (NSCLC). Whilst neoadjuvant chemoimmunotherapy enables tumor downstaging, the impact of reducing the surgical scope (such as from pneumonectomy to lobectomy) on safety and efficacy remains unvalidated in real-world settings. The present study aimed to use real-world data to compare the safety and long-term efficacy of several surgical methods following neoadjuvant treatment. Clinical data from 195 patients with NSCLC who were hospitalized between December…

BACKGROUND: Malignant pleural mesothelioma is highly aggressive and recurs rapidly despite radical multimodality treatment. Progression of mesothelioma is thought to be governed by various growth factors, including vascular endothelial growth factor (VEGF). Placenta growth factor (PlGF) belongs to the VEGF family, although no study has yet investigated its expression in mesothelioma. We hypothesized that PlGF is overexpressed in mesothelioma and could have prognostic value in patients treated by extrapleural pneumonectomy.
METHODS: We assessed by immunohistochemistry with semiquantitative cla…

Sleeve lobectomy for carcinoma of the lung was first described as a compromised operation for patients whose pulmonary reserve was considered inadequate to permit pneumonectomy. Since then, many authors have suggested that bronchoplasties may provide as good if not better results than pneumonectomy in selected cases of primary carcinoma of the lung involving the proximal bronchial tree. In all reported series, lesions in the hilum of the right upper lobe are the commonest indication for sleeve lobectomy, although all lobes and segments of the lungs may on occasion be involved with tumors that …

Numerous postpneumonectomy complications exist. We present a rare clinical case of postpneumonectomy exertional dyspnea revealing compression of the mitral annulus by the descending aorta. The patient was 42-year-old former smoker with pulmonary emphysema. He has been operated on, in 2012 (i.e., right pneumonectomy). Before the surgery, the patient was a recreational runner. However, after some months, it was difficult for the patient to resume running. Cardiopulmonary exercise testing indicated moderate exercise intolerance with important oxygen desaturation. More interestingly, a decrease of…

BACKGROUND: Methods such as the number of positive lymph nodes (nPLN), lymph node ratio (LNR), and log odds of positive lymph nodes (LODDS) … demonstrates superior prognostic power for patients with stage I-IIIA NSCLC undergoing lobectomy or pneumonectomy. By integrating both positive and negative lymph node information, LODDS offers a refined risk stratification that is particularly valuable in cases with high lymph node heterogeneity.

There is sparse information concerning approaches to metachronous lung cancer in patients who had a previous pneumonectomy for lung carcinoma. We describe the case of a 55-year-old woman who underwent a left pneumonectomy for lung carcinoma. Four years later, a radiological examination revealed a hypermetabolic nodule in the right upper lobe, which was located in the left hemithorax because of right lung hyperinflation and a mediastinal shift to the left. Wedge resection was carried out through a left anterior mediastinotomy. We believe that an anterior mediastinotomy represents a valuable opt…

Traumatic rupture of the right main bronchus with pulmonary artery injury is exceptionally rare and often fatal when pneumonectomy is required. An 18-year-old man presented with bronchial transection and pulmonary artery rupture after chest trauma. Venovenous extracorporeal membrane oxygenation enabled oxygenation during controlled thoracotomy. A parenchyma-sparing strategy was adopted: right upper lobectomy, pulmonary artery repair, and end-to-end bronchial anastomosis, avoiding pneumonectomy. Secondary bronchial dehiscence with bronchovascular fistula was successfully managed by omental flap…

Extended bronchoplasty combined with left lower lobectomy plus lingulectomy can be indicated for some locally advanced left lower lobe cancers to avoid pneumonectomy, but its outcome has not been well studied. We retrospectively analysed the surgical outcomes of 10 patients since 2008, when we introduced this procedure. The median age was 67 years (range 42-80). Squamous cell carcinoma was the most common histology. The indication for this procedure was bulky or extranodal metastasis in an interlobar node in 9 patients and massive tumour invasion to the lingula in 1 patient. Bronchoplasty was …
