Polymyositis and molecular mimicry, a mechanism of autoimmunity.

The amino acid sequences of Escherichia coli histidyl-tRNA synthetase and alanyl-tRNA synthetase, two proteins recently identified as autoantigens in polymyositis, were compared by a computer alignment procedure with those of the 3600 proteins tabulated in the National Biomedical Research Foundation protein sequence database. Both proteins contain sequences long enough to function as epitopes that match sequences on viral and muscle proteins. The homology thus revealed not only lends strong support to mechanisms of autoimmunity that invoke the theory of molecular mimicry of viral proteins, but…

Clindamycin for treatment of Toxoplasma polymyositis in a dog.

Toxoplasma polymyositis was diagnosed in a dog given repeated injections of glucocorticoids for a skin condition. The dog developed generalized lower motor neuron paralysis, with generalized hyperesthesia, myoglobinuria, and high serum muscle enzyme activities. Diagnosis of toxoplasmosis was confirmed by muscle biopsy and seroconversion. Despite supportive care, the dog's neurologic status deteriorated progressively until parenteral treatment with clindamycin was instituted. The dog made a gradual recovery, with complete return of locomotor function and muscle mass within 2 months.

Head and neck manifestations of dermatomyositis-polymyositis.

Dermatomyositis and polymyositis are rare disorders of connective tissue that manifest themselves primarily as muscular weakness. Fifty-five percent of 45 patients with this diagnosis at Thomas Jefferson University Hospital had symptoms referable to the head and neck. While dysphagia and the cutaneous manifestation predominate, such symptoms as facial swelling and oral mucosal disorders may require the treating physician to seek the advice of an otolaryngologist before the patient suffers any muscular changes. A thorough knowledge of this disease entity is essential to specialists in head and …

Esophageal abnormalities and dysphagia in polymyositis and dermatomyositis.

Structural or functional abnormalities of the distal esophagus were demonstrated by cineradiography in 14 of 16 patients with polymyositis or dermatomyositis who had been incompletely responsive to high dose corticosteroids and, in some cases, immunosuppressive therapy, or who had adverse effects associated with these therapies. These distal abnormalities occurred in the absence of proximal esophageal skeletal muscle dysfunction in 70% of patients, usually were more frequent with increasing disease duration, and were functionally similar to abnormalities reported in scleroderma. Symptomatic im…

[Electromyography in children with polymyositis (author's transl)].

Findings of repeat electromyography (EMG) in 15 children with polymyositis or dermatomyositis are reported. The data classically provided by EMG in these conditions is recalled. Emphasis is put on the contribution of stimulodetection to the diagnosis of primary disease of the muscle. With this procedure, EMG showed myogenic signs in all patients, at the first examination. Electrical anomalies are widespread from the start. A long-term study in 12 patients shows that electrical anomalies remain present long after clinical recovery. EMG disturbances are the last anomalies to disappear. EMG retur…

The relationship of dermatomyositis and polymyositis to internal malignancy.

The association of malignancy with dermatomyositis and polymyositis has been questioned. During the last 20 years (1956 to 1975), we have studied 58 cases of myositis that met predefined diagnostic criteria. These cases were analyzed for the frequency of malignancy, prognosis, and the value of a diagnostic test series for malignancy. A significantly greater frequency of malignancy was found with dermatomyositis than with polymyositis. The prognosis of dermatomyositis and polymyositis appears to be altered in the presence of malignancy. In the absence of malignancy, the prognosis is similar in …

Lymphocyte stimulation by acetylcholine receptor in polymyositis.

Lymphocytes of twenty-seven patients with polymyositis were incubated in vitro with cholinergic receptor rich membranes obtained from the electric organs of Torpedo Marmorata. Lymphocytes of polymyositic patients were slightly stimulated; positive responses were present mainly in patients affected from more than a year. Sensitization against the nicotinic cholinergic receptor may explain the occurrence of the myasthenic syndrome with polymyositis.

Survival in polymyositis: corticosteroids and risk factors.

Thirty-one polymyositis patients treated with low-dose corticosteroid were age and sex matched with 31 polymyositis patients treated with high-dose corticosteroid. Although disease severity at onset of the study was similar in the groups, no statistically significant difference in survival was found. The death rate in each group was greater than normal. The presence of dysphagia or severe muscle weakness lessened the chances of survival. Controlled trials with other drugs should be carried out.

Polymyositis as a cause of malabsorption. A case report.

We report on a patient with polymyositis which caused a malabsorption syndrome. The radiological findings included marked dilatation of the oesophagus and small intestine. The malabsorption syndrome appears to arise from bacterial overgrowth associated with intestinal stasis. The literature is reviewed. Polymyositis or dermatomyositis, like scleroderma, may involve the small bowel and cause the malabsorption syndrome.
