Moving forward on the road to recovery after concussion: participant experiences of interdisciplinary intervention for persisting post-concussion symptoms.

PURPOSE: Many factors contribute to persisting post-concussion symptoms (PPCSs), necessitating multi-modal treatment. Quantitative investigations have shown the potential of interdisciplinary intervention to reduce the burden of PPCSs and facilitate return to activities. There are often varied responses to intervention, warranting further investigation of potential factors underlying treatment response. This study aimed to explore participant experiences of i-RECOveR, an interdisciplinary intervention for PPCSs and its impact on symptoms, daily function, and concussion beliefs. MATERIALS AND

Growth hormone deficiency testing and treatment following mild traumatic brain injury.

Pituitary dysfunction, specifically growth hormone (GH) deficiency, can occur following traumatic brain injury. Our objective was to characterize the prevalence of GH deficiency (GHD) testing and response to recombinant human GH (rhGH) treatment in adults with persistent symptoms following mild traumatic brain injury (mTBI) referred for assessment of pituitary dysfunction. A retrospective chart review was conducted of patients seen at an outpatient brain injury clinic with a diagnosis of mTBI and persistent post-concussive symptoms who were referred to endocrinology. Clinical assessments of sy

Symptom Presentation After Concussion and Pre-existing Anxiety Among Youth Athletes.

Our purpose was to evaluate the effect of self-reported pre-injury anxiety diagnosis on persistent symptom development, vestibular symptom severity, and balance control among youth who sustained a concussion. We performed a retrospective study of patients seen at a specialty pediatric concussion clinic. Patients were 18 years of age or younger, examined within 10 days of concussion, and received care until full recovery. A questionnaire was used to assess pre-existing medical and psychiatric conditions, including anxiety. Our main outcomes were prolonged symptom recovery defined as persistent

Factors Associated With Delayed Concussion Reporting by United States Service Academy Cadets.

CONTEXT: Approximately half of individuals who sustain a concussion do not immediately report their injuries. Motivators for not reporting include thinking the suspected concussion was not a serious injury and wanting to continue participating in activity. Additionally, military personnel have concerns about how concussions may affect their careers. However, delayed reporting can prolong neurobehavioral recovery. Understanding the frequency of delayed reporting and contributing factors will aid in identifying individuals who may be more likely to delay reporting. OBJECTIVE: To describe the fr

Postconcussive, posttraumatic stress and depressive symptoms in recently deployed U.S. Army soldiers with traumatic brain injury.

Prior studies raise questions about whether persistent postconcussive symptoms (PCS) are differentiable from mental health sequelae of traumatic brain injury (TBI). To investigate whether PCS represented a distinct symptom domain, we evaluated the structure of post-concussive and psychological symptoms using data from The Army STARRS Pre/Post Deployment Study, a panel survey of three U.S. Army Brigade Combat Teams that deployed to Afghanistan. Data from 1229 participants who sustained probable TBI during deployment completed ratings of past-30-day post-concussive, posttraumatic stress, and dep

Associations between neuropsychiatric and health status outcomes in individuals with probable mTBI.

Mild traumatic brain injury (mTBI) is a common occurrence, and may impact distal outcomes in a subgroup of individuals. Improved characterization of health outcomes and identification of factors associated with poor outcomes is needed to better understand the impact of mTBI, particularly in those with co-occurring posttraumatic stress disorder (PTSD). Participants in a data repository of the Injury and Traumatic Stress (INTRuST) Clinical Consortium (n = 625) completed functional disability [FD] and health-related quality of life [HRQOL] questionnaires, and a subset completed a neuropsychologic

Rehabilitation Involving Tailored Subsymptomatic Aerobic Exercise in Adult Patients With Persistent Postconcussion Symptoms (REPCon Project): Protocol for an Assessor-Blinded, Parallel-Group, Randomized Controlled Trial.

BACKGROUND: Persistent postconcussion symptoms (PPCSs) represent a complex phenomenon following mild traumatic brain injury (mTBI), and this phenomenon is characterized by a diverse range of symptoms that greatly impact daily functioning, including the ability to engage in physical, cognitive, and social activities. Effective rehabilitation strategies and a nuanced understanding of brain function and patient perception remain insufficiently explored in adults with PPCSs. OBJECTIVE: This study aims to assess the physiological and neurofunctional effects of targeted physical exercise carried ou

Mechanisms of change during exposure therapy for persistent post-concussion symptoms: insights from session-level process data.

BACKGROUND: Persistent post-concussion symptoms affect a substantial subgroup of individuals following mild traumatic brain injury. Intensive exposure therapy may be an effective intervention by directly targeting psychological mechanisms underlying symptom maintenance. This study provides insight into the clinical processes of this therapy. MATERIALS AND METHODS: Therapy session-level data from two single-case experimental design studies with a total of eight participants was analyzed using mixed-methods. RESULTS: The most frequently targeted feared expectations included: cognitive failure,

The prevalence and clinical spectrum of post-Covid syndrome in patients with rheumatic diseases: a single-center experience.

Post-coronavirus disease (COVID) syndrome (PCS) is a term used to describe the clinical condition of patients who have recovered from COVID-19 but are still experiencing prolonged effects of infection or persistent symptoms for longer than expected. Although PCS has been previously studied in the general population, it has not been investigated in a specific population of patients with inflammatory rheumatic disease (IRD). This study aims to evaluate the presence and frequency of PCS among our rheumatology outpatients. This is a cross-sectional study of patients with IRD whose symptoms persist

Test-Retest Reliability of a Semi-Structured Interview to Aid in Pediatric Traumatic Brain Injury Diagnosis.

OBJECTIVE: Retrospective self-report is typically used for diagnosing previous pediatric traumatic brain injury (TBI). A new semi-structured interview instrument (New Mexico Assessment of Pediatric TBI; NewMAP TBI) investigated test-retest reliability for TBI characteristics in both the TBI that qualified for study inclusion and for lifetime history of TBI. METHOD: One-hundred and eight-four mTBI (aged 8-18), 156 matched healthy controls (HC), and their parents completed the NewMAP TBI within 11 days (subacute; SA) and 4 months (early chronic; EC) of injury, with a subset returning at 1 year

Hyperactive movement behaviour of athletes with post-concussion symptoms.

OBJECTIVE: Observations of hyperactive (/restless, agitated) behaviour as a consequence of mild traumatic brain injuries (mTBI) in sports are inconclusive as reduced or slowed movement behaviour is also commonly described post-concussion. This might be grounded in the fact that the movement behaviour of athletes has not been systematically investigated during standardized settings and with objective methods of nonverbal movement analysis. Thus, we investigate whether symptoms after mTBI in sports are characterized by a hyper- or hypoactive movement behaviour experimentally. METHODS: Three mat

Differential eye movements in mild traumatic brain injury versus normal controls.

OBJECTIVES: Objective measures to diagnose and to monitor improvement of symptoms following mild traumatic brain injury (mTBI) are lacking. Computerized eye tracking has been advocated as a rapid, user friendly, and field-ready technique to meet this need. DESIGN: Eye-tracking data collected via a head-mounted, video-based binocular eye tracker was used to examine saccades, fixations, and smooth pursuit movement in military Service Members with postconcussive syndrome (PCS) and asymptomatic control subjects in an effort to determine if eye movement differences could be found and quantified.

Integrated Blood Biomarker and Neurobehavioural Signatures of Latent Neuroinjury in Experienced Military Breachers Exposed to Repetitive Low-Intensity Blast.

Repeated exposure to low-level blast overpressure (BOP) during controlled detonations is an emerging occupational health concern for military breachers and Special Operations Forces personnel, given accumulating evidence that chronic exposure may produce subtle, subclinical neurotrauma. This study derived a latent neuroinjury construct integrating three complementary domains of brain health-post-concussive symptoms, working-memory performance, and circulating biomarkers-to determine whether breachers exhibit coherent patterns of neurobiological alteration. Symptom severity was assessed using t

Preliminary examination of symptom validity in veterans using the Rivermead post-concussion symptoms questionnaire.

OBJECTIVE: The Rivermead Post Concussion Symptoms Questionnaire (RPSQ) is commonly used post-concussive symptom measure. Simulation research has explored it as a symptom validity (SV) indicator. This project examined its use in a clinical sample using archival data from the Federal Interagency TBI Research Informatics System. METHODS: The sample (N = 133; mean age = 39.6; 96% male) included veterans with (n = 91) and without (n = 42) history of mild TBI. SV status was determined via MMPI-2-RF overreporting scales (Fr, Fpr, Fs, FBSr, and RBS). We created three SV outcome variables: possible (o

Acute interleukin-10 predicts persistent post-concussion symptoms in children with mild traumatic brain injury: exploratory findings from the t-BIOMAP study.

BACKGROUND: Children are a vulnerable population for mild traumatic brain injury (mTBI), accounting for a high burden of emergency department (ED) visits. A significant and unpredictable subset will develop persistent post-concussion symptoms (P-PCS), with consequences for daily quality of life. At ED discharge, no reliable tool exists to identify children requiring neuropsychological follow-up. Acute blood biomarkers represent a promising approach for early risk stratification. METHODS: In this prospective multicenter cohort study, children with mTBI had a blood sample collected within 24 h
